Job Description
Basic Purpose
Positions in this job family are assigned responsibilities for performing technical direct and indirect nursing care in a clinic, hospital, institution, or home setting for individuals under the supervision or direction of a registered nurse, licensed physician, or dentist. This includes taking temperatures, pulse, respiration, and blood pressure, and various other technical nursing tasks in providing health care to patients and others.
Typical Functions
* Performs nursing procedures or specialized functions in accordance with knowledge, skills, and licensure.
* Participates in the evaluation of responses to interventions.
* Teaches basic nursing skills and related principles. Delegates nursing care tasks such as: taking temperature, pulse, respiration, and blood pressure, giving enemas, douches, and sitz baths, collecting specimens, and measuring intake and output.
* Assists in assessing, reporting, and recording health status of patients, families, and groups in a variety of settings.
* Participates in developing and implementing appropriate aspects of a plan of care.
* Administers medications as ordered.
* Reports appropriate patient information to supervisor or appropriate health team member.
* Serves as Charge Nurse on a unit, supervising staff performing medication administration and patient care.
Level Descriptor
This is the specialist level of the job family where employees are assigned responsibility for performing the more complex nursing tasks and may serve as lead worker. This includes tasks involving the assignment and review of work, providing required training, and insuring that the quality of care meets established standards. Responsibilities will also include providing guidance and training to others in completing the more complex nursing tasks and activities.
Education and Experience
Education and Experience requirements at this level consist of possession of a valid permanent Oklahomalicense as a licensedpracticalnurse as approved by the Oklahoma Board of Nursing plus two years of practicalnursing experience; or LicensedPracticalNurseLicense recognized by the Enhanced NurseLicensure Compact (eNLC) plus two years of practicalnursing experience.
Knowledge, Skills, Abilities and Competencies
Knowledge, Skills and Abilities required at this level include knowledge of nursing techniques, procedures, practices, and theory; of medical terminology; and of infection control measures. Ability is required to exercise sound judgment; to follow detailed oral or written instructions; to establish and maintain effective working relationships with others; to provide nursing care to patients or clients; and to monitor and direct the work of others.
Special Requirements
Positions within the Oklahoma Department of Mental Health and Substance Abuse Services shall have a current certification as a Certified Alcohol and Drug Counselor (CADC) or be working toward CADC certification on all levels. ## Upon appointment, incumbents designated by the Oklahoma Department of Mental Health and Substance Abuse Services shall complete certification in one of the following required Departmental sponsored training in case management: Certified Behavioral Health Case Manager - Adult; Certified Behavioral Health Case Manager - Child, Adolescent and Family; or Certified Behavioral Health Case Manager - Dual Certification. Some agencies may have determined positions in this job family to be safety sensitive as defined by 63 O.S. ยง 427.8.

Shall read, understand, speak, and write English fluently.

2. Implements plan of care initiated by the RN Case Manager in collaboration with the physician. Assists the registered nurse or physician in carrying out the plan of care.
3. Develops, prepares, and maintains individualized patient/client care documentation consistent with the plan of care. Submits accurate documentation.
4. Assesses and provides patient/client and family/caregiver education and information pertinent to diagnosis and plan of care as established by the registered nurse. Documentation addresses problems identified in the on going assessment or provides documentation of rationale for not doing so. Documentation reflects communication and condition of the patient.
5. Participates in coordination of home health services, appropriately reporting the patient's/client's condition, status, and need for additional disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the RN Case Manager and/or Branch Supervisor on an ongoing basis.
6. Assists the patient/client and family/caregiver in learning home nursing procedures, self-care techniques and other care needs as appropriate to the patient's/client's condition. Provides medication, diet and other instructions as directed by the registered nurse and ordered by the physician.
7. Uses equipment and supplies effectively and efficiently.
8. Participates in personal professional growth and development. Responsible for keeping current in nursing trends and knowledge. Participates in In-service programs. Participates in Quality Improvement program of the agency. Can verbalize own responsibilities and activities.
9. Participates in public relations and community activities that promote the Agencies role as an effective member of the health care delivery system.
10. Responsible for following all policies and procedures of the Agency, regarding service delivery, documentation, and care coordination.

Performs highly complex vocational nursing work. Fosters interdisciplinary collaboration and integration. Communicates pertinent information orally and in writing to other team members, such as the RN, in order to ensure the treatment team has the best information possible to make decisions.
Serves as a resource/mentor for new LVNs. Assists the RN Supervisor with on-the-job nursing instruction and training of other new LVNs.
Assists RNs with implementation of protocols, procedures, and programs to improve overall health and safety of staff and individuals. Assists the Infection Control Nurse by implementing measures that prevent the spread of disease, assists in developing and implementing quality assurance programs, and assists in the implementation of therapeutic nursing programs. Registrations, Licensure Requirements or Certifications:

SUMMARY OF JOB RESPONSIBILITIES

RESPONSIBILITIES AND DUTIES
1. Develops, prepares and maintains individualized patient/client care documentation consistent with the plan of care. Submits accurate documentation within one working day of visit/phone call.
2. Assesses and provides patient/client and family/caregiver education and information pertinent to diagnosis and plan of care.
3. Participates in coordination of home health services, appropriately reporting the patient's/client's condition, status, and need for additional disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the RN Case Manager and/or Branch Supervisor on an ongoing basis or documents rationale for not doing so.
4. Uses equipment and supplies effectively and efficiently.
5. Provides those services requiring nursing skill, in accordance with the plan of care signed by the physician. Demonstrates competency in all skills required for the agency, which may include but not limited to, venipunctures, infusion therapy, infusion therapy device care, wound care, aseptic technique.
6. Assesses client's continual care needs. Documentation addresses problems identified in the on going assessment or provides documentation of rationale for not doing so.
7. Supervises home health aide at least every 14 days, teaches Aide in duties of client care as needed, assesses client's need for unskilled care and advises case manager of need for revision of plan of care as appropriate. In ACHC Accredited offices, supervises LVN/LPN every 55-60 days with or without LVN/LPN being present in the home. Supervises plan of care delivered by LVN/LPN during recertification/discharge visit and teaches LPN/LVN as needed.
8. Responsible for following all policies and procedures of the Agency, regarding service delivery, documentation, and care coordination.
9. Participates in on call rotation as assigned by supervisor.
10. Participates in Quality Improvement program of the agency. Can verbalize own responsibilities and activities.
11. Performs duties within the limits of preparation and experience.
12. Participates in public relations and community activities that promote the Agencies role as an effective member of the health care delivery system.
