What we're looking for
At a minimum, should hold a 2-year degree. A graduate holding a four-year degree from a college program with a bachelor's degree in a health related, business or marketing area of concentration, nursing preferred.

The Clinical Nurse Specialist (CNS) works collaboratively to improve the quality of patient care and promote desired patient outcomes in a designated population/specialty practice. Assist Registered Nurses in conducting patient assessments, including physical examinations and health history reviews, to determine their medical needs, with a focus on the common health issues encountered in a shelter environment.
Assist Registered Nurses in developing and implementing patient care plans based on assessment findings, in collaboration with other healthcare professionals and shelter staff.
Administer medications, vaccines, and other treatments as prescribed by physicians or within established protocols, including managing medication administration for a large number of shelter residents, under the supervision of a Registered Nurse.
Provide wound care, including cleaning, dressing, and monitoring for infection, with consideration for limited resources and potential environmental challenges in a shelter, under the supervision of a Registered Nurse.

As a Home Health LPN, you will:
· Provide professional nursing services under the direction of a RN in compliance with the state's NursePractice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
· Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.
· Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
· Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/NursePractice Act and communicate these orders to the RN Supervisor.
· Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
· Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
· Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is dearly in the best interests of the patient.
· Appropriately communicate to ensure adherence to professional standards in the provision of and availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.
· Prepare, submit and maintain documentation as required by the Company and/or facility. Visit/shift notes documented on day services are rendered.

Required Experience/Skills:
Graduate of an accredited LicensedPracticalNursing Program or accredited School of Vocational Nursing.
Current nursinglicense in the practicing state.
Valid drivers license, auto insurance and reliable transportation.
Current CPR certification.
Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

How you will contribute
You will determine donor eligibility; to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE).
You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.
What you bring to Takeda:
High school diploma or equivalent to including graduate equivalent of a recognized educational nursing program with state requirements
Currently licensed or certified in the state where responsibilities will be assigned: LPN or LVN
Current Cardiopulmonary Resuscitation (CPR) and AED certification
Fulfill state requirements (in state of licensure) for basic IV therapy
Satisfactorily complete the FDA approved training requirements for BioLife Medical Support Specialist
Two years in a clinical or hospital setting

Position Summary: An experienced registered nurse or licensedpracticalnurse with the Knowledge and Skills to coordinate and oversee all functions of the MDS and resident care assessment process to assure compliance with federal and state requirements, and to certify timely completion of MDS assessments. Able to collect resident assessment data in person and electronically. Able to care plan appropriate resident needs
Conducts Schedules or oversee scheduling of all resident MDS assessments to assure timeliness.
Coordinates and oversees all functions of the MDS assessment process to assure compliance with federal and state requirements.
Participates in the pre-admission process to determine potential residents' level of care.
Participate in resident level of care determination upon admission.
Participates in tracking changes in resident level of care to assure timely scheduling of MDS assessments.
Familiar with residents' condition and care needs.
Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
Review reports of timely completion of residents' MDS assessments and care plans and take follow-up action as required.
Analyze Quality Indicator reports for potential resident concerns and problems and review residents' care plans to be sure information is included.
Case Management duties involving sending updates to case managers, assisting with maintaining authorizations and keeping up a schedule for the patients so the case managers know when updates are due.
Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
Maintain confidentiality of resident and facility information.
Complete reports and assignments accurately and adhere to established time schedules.
Establish open lines of communication between all nursing personnel.
Observe infection control procedures relating to nursing services.
Observe all facility safety policies and procedures.
Come to work as scheduled and consistently display dependability and punctuality
Consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families and consultants.

Under the direction of a licensed registered nurse, provides practicalnursing services to students with medically complex health needs. Performs designated routine non-invasive and invasive procedures established by physician orders. Provides basic first aid and attends to students health needs as indicated in the School Health Services Manual.
Essential Functions
1. Performs routine and prescribed invasive and non-invasive procedures, including, but not limited to; gastric tube feedings; breathing treatments; diabetic glucose monitoring; urinary catheterizations; and tracheostomy care and suctioning.
2. Identifies and monitors student health status through needs assessments, screening procedures, examinations, observations, and analyses.
3. Provides basic first aid and attends to student health needs as indicated in the School Health Services Manual
4. Establishes and maintains appropriate medical records, logs, indices, and schedules on daily and school year diagnoses, procedures, treatments, monitoring, adverse reactions, and notifications.
5. Distributes/administers prescription and non-prescription medication to students in accordance with applicable laws and directives, following guidelines described in school health policy manual.
6. Assists the student, school personnel, and parents in understanding and coping with a students disability or condition.
7. Designs intervention in conjunction with other health and education team members to prevent further disability.
8. Facilitate Telehealth appointments for students.
9. Performs other duties as assigned.

Qualifications
Open Requirements: Licensure as a LicensedPracticalNurse (LPN) by the Florida State Board of Nursing; OR graduation from a School of PracticalNursing meeting the minimum requirements of the Florida State Board of Nursing with authorization from the Florida State Board of Nursing to practice as a LPN in accordance with Florida Statutes. Must maintain same during employment in this class.
