Summary/Objective:
The SocialWorker provides comprehensive counseling and related support services to assist in meeting the identified psychosocial needs of patients and families enrolled in hospice, home health, palliative care and other Partners In Care programs/services. This role ensures that patients and families receive palliative care that meets the standards of Partners In Cares mission statement, policies, and procedures. The SocialWorker offers psychosocial and emotional support and engages in interdisciplinary collaboration in both community-based and facility settings.
Essential Functions:
Conduct comprehensive psychosocial assessments for home health, hospice and palliative care patients/families.
Develop goals and interventions for the Interdisciplinary Plan of Care based on assessment findings.
Provide professional Social Work services, including ongoing assessments, counseling, emotional support, home care planning, education, resource identification, advocacy, and referrals.
Collaborate with the Interdisciplinary Care Team and other departments to ensure timely and appropriate delivery of services.
Work with Bereavement services to complete risk assessments and provide follow-up support.
Maintain relationships with community agencies/resources and promote education and support activities.
Perform initial and ongoing holistic assessments, utilizing therapeutic techniques to support decision-making.
Facilitate care coordination and make appropriate referrals.
Participate in IDT meetings and advance care planning, including end-of-life conversations.
Educate patients and families on health literacy, cognitive abilities, and medical care challenges.
Engage the community in education about palliative care services.
Support patients and families with anticipatory grief and bereavement counseling.
Set measurable goals to help patients achieve their desired quality of life.
Travel/driving required by personal vehicle throughout Central Oregon.

POSITION OVERVIEW:
Outpatient Counselors provide high-quality clinical treatment services tailored to individual needs, incorporating the latest research and evidence-based practices, while assisting clients in successfully completing their treatment. Competitive Compensation: Compensation is based on billable services rendered. Earning potential is based upon clinicians who work for us. Clinicians make from $82,660.00 to $120,000 per year (working 4 days per week). Income depends on your ability to engage with clients and the clients desire to continue to see you as their therapist as well as how many clients you schedule in your calendar. Your earning potential is up to you!
Client Caseload: Full-Load is defined as a 22hr-client caseload per week, consisting of variable length sessions equivalent to 22 hours. This is the minimum expectation for full-time status. You're welcome to see more clients if that aligns with your personal and professional goals.
Schedule: There is some flexibility in the evening hours and selection of weekdays on the schedule but there will be a requirement to work 2 weeknights and 2 weekend days (Friday and Saturday). Counselor will be required to maintain 25-28 available client hours for scheduling.

Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group

Requirements:
Must have a master's degree in social work from an accredited university.

You can work as much or as little as you like as an On-Demand Team Member (TM) and your schedule may vary depending on shift availability and store needs. This role is ideal for those that are looking for a personalized work schedule.
As an On-Demand TM, you will not be included on the posted weekly schedule, but rather will have the opportunity to create your own schedule by picking up shifts posted by leaders or other team members (via our my Time mobile scheduling app) that work best with your schedule.

DEPARTMENT SUMMARY: St. Charles Health Systems Acute Social Services and Psychiatric Assessment Team (PAT) positions may support anyone, or a combination of the following departments: Social Services, Home Health, Behavioral Health, or Hospice. Services provided to the community and region include the following: Acute mental health crisis and triage, inpatient psychiatric hospitalization, treatment for SPMI population, substance use triage, local, regional and state resources for long term care access needs, and collaboration with the outpatient community at discharge. These departments provide system-wide support including, but not limited to working with multi-disciplinary team to assess needs of patients and families, provide appropriate resource referrals, provide supportive counseling as needed, coordinate discharge planning and provide other identified services as needed to support those affected by chronic or acute illness.
POSITION OVERVIEW: The Mental Health Practitioner/Social Services Specialist provides counseling services including assessment, diagnosis, and treatment for patients experiencing medical/social/behavioral/mental health concerns. Discharge planning; supportive counseling and brief interventions. Assist in meeting the psychosocial needs of patients and their families to help patients utilize services to achieve optimum level of function and to coordinate care throughout the continuum of care.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Participates with team members in facilitating patient's and family's learning through the continuum of care. Reinforce patient's continued health care through teaching and/or referral to community agencies for follow-up needs.
Provides diagnostic, preventative and treatment services of a psychosocial nature pertaining to personality adjustment, behavior problems and interpersonal dysfunction.
Respects privacy and protect the confidentiality of patients and family members while maintaining boundaries of the professional discipline as outlined in the affiliated licensing board's Code of Ethics.
Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost-effective manner.

Responsible for maintaining knowledge of all state and federal regulations as they apply to the admission, care, and discharge of patients including all pertinent civil commitment procedures and protocols.
Coordinates after-care with outpatient providers including therapeutic appointments, warm handoffs, medication management, and follow up medical appointments that are in compliance with regulatory standards and/or other metrics.
Attend unit meetings and trainings as scheduled.
Identifies victims of abuse (child, domestic and elder). Make appropriate referrals and notification to appropriate services, including the Department of Human Services and/or law enforcement when applicable.

LICENSURE/CERTIFICATION/REGISTRATION:
Required: Code Grey Training is required with 3 months of hire. Ability to travel to business functions/training/meetings and all SCHS worksites.
Preferred: Current unrestricted Oregon LCSW, LMFT, or LPC licensure. Member of the National Association of SocialWorker's or another national association membership.
Bilingual in Spanish and English communication.
EXPERIENCE:
Required: One year experience in a healthcare/hospital or behavioral health inpatient/residential setting. Knowledge of community resources. Internship experience in an emergency department/inpatient behavioral health setting may be considered.
Preferred: Two years of experience in a hospital or behavioral health inpatient/residential setting. Postgraduate, clinical experience in inpatient/outpatient settings, crisis assessment and/or community organizations that work to support planning and administering of community issues and needs. Advocacy to promote social justice and change.
