Provide bereavement support and grief care to patients and caregivers.
Job Requirements:
Masters of Social Work degree from a school accredited by the Council on Social Work Education with a minimum of one year practical experience, preferably in the home health/hospice care environment.
Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting.

Delivery of Patient Care
Conducts psychosocial assessments of patients/caregivers and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
Develops, implements, and evaluates plans of care for patients/caregivers and families. Incorporates therapeutic, preventive, and other clinical social work practices that specifically address patient/caregiver and family needs for counseling and education while maintaining the dignity of the dying patient. If not an LCSW, consults with and involves a Licensed Clinical SocialWorker or Social Work Preceptor in cases that require clinical social interventions or support beyond their expertise or scope of practice.

Direct Patient and Family Support:

Bereavement Support:

Education and Advocacy:

Knowledges, Skills, and Abilities - Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework, groupwork, and community problems. Considerable knowledge of a wide range of behavior and psychosocial problems and their diagnosis and treatment. Considerable knowledge of family and group dynamics, a range of intervention techniques, governmental and private organizations, resources in the community, laws, regulations, and policies which govern the program, methods and principles of casework supervision and training. In certain settings, considerable knowledge of medical terminology, disease processes and their treatment as they relate to decisions regarding clinical interventions and appropriate therapies based on medical or psychological diagnosis. Skill in establishing rapport with a client and in applying techniques or assessing psychosocial, behavioral, and psychological aspects of client problems. Ability to supervise, train, or instruct lower-level socialworkers, students, or interns in the program; ability to establish and maintain effective working relationships with members of case load and their families as well as civic, legal, medical, social, and religious organizations. Ability to express ideas clearly and concisely and to plan and execute work effectively.
Minimum Education and Experience Requirements - Master's degree in social work from an appropriately accredited institution and two years of supervised social work experience as required by the North CarolinaSocial Work Certification and Licensure Board.

Position Summary
Under general supervision, the LCSW provides comprehensive case management services for all clients and their family members. Responsibilities include initial and on-going assessment of bio-psychosocial factors, healthcare factors and educations needs of these clients with a focus on enhancing our client's capacity for self-support and self-sufficiency.

Essential Duties and Responsibilities
Direct Patient Care - responsible for providing quality direct primary care to patients and family members.
Provide crisis intervention services to patients
Provides/ conducts psychosocial assessments and assists the patient in utilizing the support systems and community resources available.
Provide patient education
Provide counseling therapy
Implement treatment plans by identifying an ranking problems based on individual patient needs.
Support the medical treatment plan.
Demonstrates critical thinking skills by performing appropriate interventions.
Identifies the impact of cultural diversity and lifestyle choices on patient condition and makes accommodations for such differences.
Care Plan Development - responsible for developing useful treatment plans for patients and family members.
Meet with individuals to discuss current needs and create plans to address needs.
Utilizes knowledge and required skills to ensure appropriate assessments.
Consult and plan meetings with other professional to discuss patient's treatment services.
Prepare and maintain records.
Maintain and up-to-date referral system.
Contributes to optimal efficiency and productivity of the clinical care team by anticipating needs of patient and communicating with medical provider regarding patient care.
Community and staff support - ensure community outreach and education of community staff.
Participation and leadership in all center based activities Coach patients to engage in various wellness programs offered by center, based on care plan.
Administrative Reporting - Creates and delivers accurate reports to the Chief Medical Officer (CMO).
Meets productivity and billing goals set by organization.
Generates reports based on work with clients.
Work with the Clinical Services Department to promote ongoing performance improvement activities.
