The Hospice Social Worker provides professional social work services directly to terminally ill patients and their families. As a member of the IDT, the Social Worker provides consultation with team members about psychosocial care issues and coordinates social work services with the team. The Social Worker is responsible for continuing his/her own professional development to enhance practice. The Director of Counseling & Volunteer Services provides oversight of this position, via the IDT meetings. Performs other duties as assigned. PRIMARY JOB DUTIES 1. Assesses patient/family status to identify needs, provide supportive counseling, problem solving, service coordination, case management, discharge planning and placement within the scope of the social worker standard of practice. 2. In the office minimal. Must use own car when company transportation is not available for travel to remote sites, often in rural, isolated areas. Must be familiar with principles of growth and development over the life span and the ability to assess data reflective of the patient's status and interpretation of information needed for each patient's requirements relative to his/her age specific needs to provide the care needed. Schedule: PRN ( As Needed) Qualifications/Training: At least one year experience in social work preferably in home health, but must be in allied health care. Licenses/Certifications/Registrations/Education: Bachelors Degree from an accredited social work program. Must possess a current valid South Carolina driver's license and automobile insurance as required by law. Must be at least 21 years old to be covered under McLeod Health's fleet vehicle insurance policy. Must meet McLeod Health policy for driving eligibility. License and registration as required by state law. SC Social Work License, BLS Certification.
