* Set up and break down workstations

As a leading provider of Home and Day Services to individuals with special needs. As a TeamLeader, you will work with service recipients and staff in small groups and/ or individually in any of the programs under Home & Day Services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
(Other duties may be assigned).
Responsible for the supervision and oversight of staff under assigned caseload. A TeamLeader can have a caseload of up to fifteen staff. In addition, will be responsible for the supervision, safety and support of up to 40 clients under a designated caseload.
Provide ongoing supervision and support to assigned staff.
Review and monitoring Staff performance. Complete ongoing staff performance report.
Providing necessary support and training.
Responsible for the safety, oversight, and compliance of service recipients
Provide all services to individuals as required. Serve as a positive role model while serving as a DSP and when covering for staff in their absence.
Complete all required Home & Day Services billing documentation in an accurate and timely manner. Review and submit service documentation notes that are completed and accurate.
Report and document all incidents and maintain records. Complete Monthly Summary Reports of outcomes being worked with each individual in accordance with the client's Habilitation Plan.
Insures that consumer's chosen goals are implemented to improve or enhance the individual's independence, inclusion or integration into the community and that the outcomes are individualized.
Monitor activities and assist with social activities, grooming, feeding, and personal hygiene as needed; assist with ambulation and mobility; from total physical assistance, with or without adaptive equipment, to verbal support.
Supervises participants in the community, using various experiences to reinforce skills being taught in group sessions while ensuring their safety.

Maintains communication with the Care Manager to provide input and ensure compliance with the valued outcomes in the Life Plan and Staff Action Plan.
Provides weekly documentation of outcomes being worked towards with each individual in accordance with his/her Habilitation Plan.
Ensures follow-up of recommendations made during all reviews.
Serves as a positive role model.
Responsible for community outreach , maintaining volunteer site files, establishing relationships with staff at the volunteer sites.
Provide a full day of active programming for all assigned individuals based on individual preferences and interests using approved teaching strategies.
Initiates outreach and establishes linkages and relationships with community-based organizations to provide opportunities for community-based implementation of Day Services.
Implements the Individual's Staff Action Plan, Safeguards and assists individuals in meeting the objectives in their plans.
Assistance may be required in the following areas: cleaning, preparing snacks, social skills, budgeting, reading, math, computer skills, self-advocacy skills, volunteering, recreational and physical fitness activities, etc. Staff may need to provide hands-on demonstration, modeling, prompting, and instruction in the use of these (and other) skills.
Maintain records and statistics in accordance with the program and funding agency requirements and complete all reports and documents in a timely manner; these include bus arrival/departure forms, daily case notes, and individual attendance records.
When necessary, attends meetings for individuals receiving day services and reviews or revises the Staff Action Plan as needed.
Complete accurate Month Service Notes report.
Complete program/ unit progress report.
When necessary, manages a limited amount of petty cash funds
ADDITIONAL DUTIES AND RESPONSIBILITIES
As necessary, ensure that the grounds, furniture, equipment, and relevant property are kept in a safe, clean, functional, and sanitary condition at all times. Notifies Supervisor, CEO, and/ or designee immediately if a problem occurs that cannot be corrected by the Day and Home Service staff.
Responsible for obtaining and maintaining current First Aid, CPR, and other designated certifications. Also attends all required training and other meetings deemed necessary by the Supervisor and CEO.
Act as a professional role model for the individuals and staff; maintain a professional manner and presentation at all times. Since at times, work is done off-site, the Home and Day Service staff members represent Help On the Way and the program to the community.
Complies with OPWDD regulations and with all Help On The Way policies and procedures and site-specific program policies and procedures manual. Comply with Help On The Way corporate compliance, OMIG compliance, confidentiality, and HIPAA rules.
Attends and participates in in-service training.
Demonstrate the flexibility and willingness to adapt to changes in job duties when the agency's needs arise or change.
Ensure that program participants are transported in the program vehicle or agency van.
Visits appropriate volunteer sites or potential worksites to observe and develop possible volunteer or employment opportunities for participants. Performs any other duties deemed necessary by the Supervisor and/or Director of Home and Day Services, and Executive Administrative staff.
Additional responsibilities as deemed necessary and/or appropriate by the Home and Day Services supervisor.
SUPERVISORY RESPONSIBILITIES
Responsible for properly supervising up to 15 DSPs.

EDUCATION AND / OR EXPERIENCE
High School Degree,
Associate's degree in a health and human service field or 60 credits in a bachelor's program in a health and human service field and

A Master's degree in the health and human services field.
Language Skills
Written and reading fluency in English. Ability to read, and interpret documents such as medical reports, assessments, etc. Ability to write routine reports and correspondences. Ability to speak effectively with service providers and families.

The Assistant TeamLeader is responsible for supporting the Store TeamLeader and assigned store(s) in the processing and merchandising of donations and new good merchandise. When serving as Leader on Duty (LOD) is responsible for assuring safety, sales, production, programs and services and other standards are maintained. This position will also partner with other members of the Donated Goods Retail Administration team, People Resources, and Workforce Development to ensure that all activities performed are in a manner consistent with the mission, vision, and values of Goodwill of the Finger Lakes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Manages production and merchandising of assigned departments.
Consistently meets or exceeds daily, weekly, and monthly production goals for assigned operation.
Understands the production process and works to develop efficiencies or continuous improvements in coordination with Store TeamLeader.
Communicates production goals and ensures team members understand the relationship between store sales and Goodwill's mission.
Motivates team members to achieve goals.
Performs the duties of team members as staffing needs arise.
