Key Responsibilities:
- Conduct comprehensive assessments of clients to determine their needs, strengths, and challenges within the context of the criminal justice system.
- Develop and implement individualized service plans that address the behavioral health and legal needs of clients, ensuring alignment with court requirements and treatment goals.
- Coordinate and facilitate access to mental health, substance abuse, and social services, acting as a liaison between clients and service providers.
- Monitor client progress and compliance with treatment plans and court mandates, providing regular updates to relevant stakeholders, including legal representatives and probation officers.
- Advocate for clients within the legal system, ensuring their rights and needs are represented and addressed.
- Maintain accurate and up-to-date case records, documentation, and reports in accordance with organizational and legal standards.
- Collaborate with interdisciplinary teams, including mental health professionals, law enforcement, and community organizations, to enhance service delivery and client outcomes.
- Provide crisis intervention and support to clients as needed, utilizing de-escalation techniques and emergency resources.
- Participate in training and professional development opportunities to remain informed about best practices in forensic casemanagement and behavioral health services.
- Engage in community outreach and education efforts to promote awareness and understanding of the intersection between mental health and the criminal justice system.
Qualifications
Required Education:
- Bachelor's degree in Criminal Justice, Psychology, Social Work, or a related field.
Required Experience:
- Minimum of 2 years of experience in casemanagement, preferably within a forensic or behavioral health setting.
- Proven track record of working with individuals involved in the criminal justice system.
- Experience in developing and implementing individualized case plans.
Required Skills and Abilities:
- Strong understanding of the criminal justice system and its impact on individuals with behavioral health needs.
- Excellent communication and interpersonal skills to effectively interact with clients, law enforcement, and other stakeholders.

Overview
The Forensic CaseManager is responsible for monitoring and assisting individuals involved in the criminal justice system who exhibit serious impairment due to mental illness. The focus of the position is on serving individuals under court supervision adjudicated Incompetent to Proceed or Not Guilty by Reason of Insanity consistent with Florida Statute 3.212 and Rules of Criminal Procedure 916. The Forensic CaseManager's goal is to enhance the independence, autonomy and quality of life of the individual served, while ensuring compliance with all judicial requirements to ensure public safety.
Requirements
Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
Must have a minimum of two (2) years of full time or equivalent experience working with adults experiencing mental illness.
Must be eligible for enrollment as an Adult CaseManager by the State of Florida.
Must be a Certified Behavioral Health CaseManager (CBHCM) through the Florida Certification Board within six (6) months of start date
Must have knowledge of available resources in the service area for adults with mental illness.
Must have knowledge of and comply with state and federal statutes, rules and policies that effect the target population.
Must be able to travel locally to clients' homes, training locations and other locations as necessary.
Must be able to provide individualized, integrated mental health, substance abuse and co-occurring services to meet the needs of individuals served and their families.
Strong communication and interpersonal skills.

Our FIT team provides intensive substance abuse therapy, mental health therapy and casemanagement services to parents involved with the Florida Department of Children and Families.
Overview:
Primary duty is the performance of work requiring advanced knowledge (Master's Degree) in the field of Human Services, and to use such advanced knowledge to consistently exercise discretion and independent judgment in providing the highest quality client-centered therapeutic services. SUMMARY: The Targeted CaseManager III function as an advocate and support person to those individuals assigned to their assigned program and links them to appropriate services and monitors the progress or lack of progress by the individual and his or her family. The Targeted CaseManager III will work closely with the individual, the individual's family, LifeStream staff, and other community organizations to meet their needs, achieve their goals, and optimize their level of independence.
ESSENTIAL FUNCTIONS of JOB and RESPONSIBILITIES:

Advocates in the client's best interest
Link individuals served with needed services
Monitor individuals served progress and provide individualized supports
Conduct a thorough assessment of the consumer's home stability to establish significant changes in the consumer's home environment
Based on this assessment, develop and review the service plan as required to meet consumer needs when there are significant changes in the consumer's situation
Schedule face-to-face contact with consumer served, preferably in their natural living environment as required assigned by program guidelines; review and update service plan and provide encouragement and monitor progress
Involve significant others in the consumer's treatment to augment or create additional supports for the consumer
Convenes/attends staffing as necessary for assessing and coordinating services for the consumer
Assist the consumer when possible to improve the consumer's home environment
Assesses needs and develops a plan to meet the best needs of consumer
Monitors and evaluates service provision and individuals' progress
Work with consumer to develop service plans with long and short-term goals
If servicing youth, provide transition services for youth nearing 18 years of age to an adult system of care
Assists the consumer to develop connections to natural supports within their own network of associates, including friends and neighbors, community organizations, including service and religious organizations and through participation in clubs, organizations, and other civic activities
Arranges for procurement of incidental expenses of any additional temporary supports that may be necessary to move the family towards greater independence, stability and achieve resiliency goals as documented in their Plan of Care
Assists in coordination of any adjunctive services that are out of the scope of practice and therefore not provided by Staff
Assists with transporting consumer to medical appointments, court hearings, or other related activities outlined in the Plan of Care as needed
Communicate and liaison with the individual, family and service providers
Document accurately and remain current with all documentation
Attends team meetings as scheduled on a regular basis
Work flexible hours as needed
Maintain consumer files including all agreements, release forms, service plan and up to date progress notes at required by program guidelines
Maintain confidentiality at all times
Assist with transportation as necessary
Participate consistently in all in-service training programs
The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance

KNOWLEDGE AND JOB SKILLS: Must have a clear understanding of the characteristics and problems of individuals with a mental health diagnosis(es) or co-occurring mental health and substance abuse diagnosis with behavioral issues placing them at risk for an out of home placement. Must demonstrate excellent verbal and written communication skills and be able to manage computer-related tasks effectively. Displays excellent telephone and customer service skills. Must be able to function effectively within a team; however, must also be able to function autonomously. Must possess excellent problem-solving, advocacy, resource development, time management, and organizational skills. The ability to monitor clients' self-administration of medications is important. Crisis intervention skills are vital. Must have knowledge of life saving techniques including CPR certification and first aid.
PERSONAL QUALITIES: Patience and a high tolerance for frustration is essential. Initiative, dependability and accuracy are important. The ability to analyze situations and manage them in a proactive manner is important. Must be able to work within a team and be flexible in job roles.
SPECIAL FACTORS: Must be able to meet deadlines. Must have a valid driver's license and a clean driving record.

EDUCATION & EXPERIENCE:
Education:
Bachelor's degree with major course work in the human services field required or equivalent experience.
Targeted CaseManager certification required
Experience:
Minimum of Four (4) years of prior experience working with individuals with serious mental illness is required
Driving Requirements:
Must be Over the age of 21 required
Possess a Valid FL Driver's license (within 30 days of hire if currently do not possess)
Possess a continuously valid US Driver's license for the past three years, from date of issue required
Safe Driving record required
TRAINING & DEVELOPMENT: Completes all requirements according to Individual Training Plan within the first six months of hire and annually. Other training and/or professional development may be assigned due to evolution of programs.
