Job Purpose: - The Targeted Case Manager I at LIFESTREAM BEHAVIORAL CENTER is responsible for providing comprehensive case management services to individuals with mental health and behavioral challenges. The role involves assessing client needs, developing individualized care plans, and coordinating services to ensure clients receive the necessary support for their well-being and recovery. Requirements: • High School diploma or G.E.D. • One (1) year experience in social, economic, health or rehabilitative setting, medical, nursing, child development or childcare setting; OR one (1) year of post high school in education social services, economic, health, rehabilitative, medical or childcare services may be substituted for the year of experience required; OR an equivalent combination of education and experience. • Strong written and verbal communication skills. • Basic math skills. • Must be able to pass organization screening requirements, including state or federal background screenings as appropriate. • Must possess a valid driver's license from the state in which you reside, good driving record, and insurable under Center's policy.
