Purpose and Impact: The Case Manager provides services in our Guardianship program to assist in the coordination of direct services for up to 40 wards of the Agency to include: placement, monitoring of facilitating the provision of medical, social, psychiatric and financial needs of the ward. Essential Functions: Visit clients a minimum of one time monthly and monitor the care provided. Prepare the initial and annual plans for the court. Maintain client files and case notes. Assess the clients for needs, coordinate, facilitate and monitor services to meet client needs. Assess client needs and arrange for appropriate placement of our clients, monitor same. Review and monitor client needs, attend care plans, provide consents for medical treatment etc. Assist in marshalling assets and apply for benefits on behalf of clients as applicable. Document all client activity. Correspondence relating to the wards affairs and personal relationships. Advocacy on behalf of all clients. Conduct case conferences with Program Director, Client Assets staff and other caseworkers as needed. Interact with the volunteers of the program. Coordinate with other professionals involved in the continuum of care for all wards. Assume 24 hour emergency call for after hours, to be shared with the Program Director and other caseworkers. May be required to assist the Program Director with community outreach, education and marketing duties. Other Functions: Other duties as assigned by Program Director Physical Requirements: Hours may be irregular as one may be on call 24 hours. Spend a significant time out in the field monitoring the needs to assigned cases. Subject to unsanitary homes, physical exertion, and possible physical or verbal abuse from clients. Need to lift some heavy items at times. Work is performed with limited supervision. Case Manager is entrusted and required to communicate confidential and/or sensitive information outside the Agency. Work is performed on a highly independent basis and it is necessary to make client decisions and to continue to develop new methods and approaches in dealing with clients. Required to drive frequently using own vehicle. Must have Florida Drivers' License, reliable vehicle and current auto insurance. Education: BA or BS in Social Services, Accounting, or LPN, RN required. Experience: Experience in guardianship preferred. Background in the social services, medical nursing, legal, insurance or financial field required. Knowledge of community resources and services is helpful. Skills: Self-starter, whom can work independently and is unafraid to make decisions. Must be able to seek out supervision and help when needed. Interpersonal, organizational, and problem solving, written and verbal communication, and time management skills are required along with the ability to transfer knowledge to others. Computer knowledge preferred. Other: Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs. Adherence to Florida Law including but not limited to statute FS 744. Under Florida Statute 744.3135, I understand that I am required to successfully submit to a credit check every two years to continue employment with the agency. Essential Functions: Completes staff training as required by SNAP and agency; Completes intake assessments for SNAP including all required pre and post assessments. Responsible for the day-to-day case management and system navigation of SNAP Follows up on incoming referrals and schedules clients for intakes appointments. Monitors youth and family's progress through weekly compliance calls; Facilitates SNAP curriculum within the fidelity of the model; Liaises with relevant community outreach responsibilities including, but not limited to community relations with: agencies including schools, community-based agencies, local DCF and DJJ community partners, state attorneys' offices, and Police Departments to enhance outreach; Completes data reporting requirements for intake assessments and ongoing weekly data reporting requirements to all relevant systems in a timely manner (examples include: NETMIS, JJIS, Dropbox, and ASEBA, etc.) Ensures weekly materials are prepared and ready for SNAP groups including handouts, manuals, and all other required items; Supports the SNAP Coordinator with organization of food arrangements as needed; Participates in pre-briefing and de-briefing meetings weekly with other SNAP Team members; Participates in weekly consultation calls with the Florida Network; Willingness to work nontraditional business hours including evenings when Groups are scheduled. Purpose & Impact: The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency's core values. Essential Functions: Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing. Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child. Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced. Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Essential Functions: Conduct weekly in person trauma support for clients, focused on crisis intervention, safety planning and family preservation. Provide client centered ongoing intensive case management for especially vulnerable clients with special circumstances (medically or psychologically vulnerable clients, family conflict or crisis, education-related issues) Conduct comprehensive intakes for all participants determined eligible for the program Develop goal-oriented, client-centered self-sufficiency plans with each enrolled program participant, addressing their long- and short-term needs and goals. Maintain regular and timely communication with supervisor around any issues concerning the client's safety, stability, or well-being. The Case Manager maintains case ownership responsibility as the integrator of all services and supports identified for each child, including therapy, mental health services, health and dentistry, developmental services, educational support, permanency and safety. The Case Manager manages cases toward goals recorded in case plans while adhering to Florida Statutes, Administrative Code, written policies and orders of the Dependency Court. The Case Manager reports to the Case Management Supervisor. Essential Functions: Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing. Purpose & Impact: The employee in this position is expected to have knowledge of Florida Child Welfare Statutes, administrative code and operating procedures as well as any additional policy provided by the local Lead Agency. Department of Children and Families Regional Quality Assurance staff or Department memo is necessary for the proper performance of this position. The Supervisor is required to demonstrate advanced skills in communicating with, motivating, training and evaluating employees, and is expected to meet regularly with each worker under supervision to monitor and mentor performance. Primary focus is to assist Dependency Case Managers in the provision of services to families to assist with the retention of case managers. Essential Functions: Supports the case management team to ensure permanency, safety and well being of all children. Assists with daily tasks to ensure contractual requirements are met for all performance measures. Monitors and directs the day-to-day work of Case Management Support Specialist Team. Monitoring must include tracking of medication 5339's, referrals, transportation records, obtaining provider records and all performance contract measures. Reviews and assists with complex cases, provides guidance and support. Develops daily plans for high risk youth to assist with stabilization. Identifies performance needs of staff and develops and implements plans for performance improvement. Provides guidance to staff by coaching, motivating, training and providing staff development activities. Identifies and promotes outstanding performance. Utilizes software for creation of reports for various tracking needs, to include scheduling of appointments and other child well-being initiatives. Assist in completing outstanding issues on the Data Discrepancy report and maintaining FSFN education and medical tabs as requested. Assist in sending client service referrals, gathering documents for packets, Child Resource Record packets, drug screen requests, birth certificate, social security card request and any other administrative request Organize, prioritize and complete all work assignments by the established deadlines. Attends program Supervisor meetings as scheduled and shares information gathered with unit workers. Conducts regular unit meetings with staff to share information, provide training, discuss cases and participate in team building activities. Develops training and staff development plans with each staff person under his/her supervision. Conducts all required performance evaluations with assigned workers in a timely manner. Submits evaluations to APDs and Program Director for discussion and review. Participates in agency CQI activities as required. Supervisor carries an active cellphone during regularly scheduled work hours, as well as after hours and on weekends, in order to immediately respond to all calls. Other: Complete a minimum of 40 hours of relevant training each full year of employment. Complete agency web-based courses as required. Purpose & Impact: Key responsibilities include the direct supervision of youth residents, providing quality care, ensuring a safe and secure environment and transporting and accompany/supervising residents on outside activities. This is a direct service delivery position at a primary location, but the position may work at any one of a number of regional group homes within a reasonable radius as shift needs arise. These group homes house adolescent teens who have aggressive behaviors. The ideal candidate for this role is someone who would be comfortable working with at risk adolescent males and females. Purpose & Impact: The purpose of the Behavior Support Specialist is to perform professional work in behavior management of pre-school children, training and coaching for staff and parents, case management including referral for psycho-educational and mental health services. Responsible for developing, modeling interventions, and monitoring the child's progress and linking parents/families to mental health and/or psycho-educational services. Successful experience working with pre-school age children with behavioral concerns. The work involves linking parents/families to community services, providing support, making referrals and following up on referrals, designing and implementing classroom behavior support plans, providing mental health and behavioral management education to parents and staff, planning and consulting with the contracted mental health professionals. This work is performed under the supervision of the Licensed Mental Health Counselor and is reviewed through conferences, reports and evaluations of program effectiveness. Participates in the HS/EHS Quality Assurance efforts. Essential Functions: Provides parents support to access necessary resources and conducts in-home follow-up, monitoring and training to assure consistency between parents and teachers as needed Teaches effective parenting skills. Participates in the multi-disciplinary case management team meetings, individual and staff conferences, health services advisory meetings, team meetings and other appropriate meetings, as necessary.
