Job Duties and Performance Standards Functions as a Surgical Technician in 5 of the 6 specialties demonstrating responsibilities that include assisting in direct patient care in the operating room. Demonstrates appropriate aseptic technique, i.e. scrub, gown and glove, draping, counting and set up according to department policy Selects and prepares the appropriate supplies and equipment for the surgical cases. Operates all surgical equipment Organizes instrumentation and the sterile field. Anticipates needs of the surgeon, passes appropriate instruments, assists in exposure of the surgical field. Performs all surgical counts consistently, including the โ€œsafety countโ€. Performs wound closure (Subcutaneous and Skin) under the direct supervision of the surgeon. Participates in general operating room activities to create a highly effective team. Participates in PI activities, continuing education, planning and orientation of new personnel, and emergency preparedness. Demonstrates an awareness of the needs of the department and the assigned rooms and or tasks.

The cardiac cath lab physician assistant is a licensed provider who will be responsible for all aspects of pre, intra, and post procedure care of interventional cardiology patients. This includes, but is not limited to, pre-procedure assessment, informed consent, sedation assessment, assisting with procedures, post-procedure management, and discharge education. The PA will first assist in all types of procedures performed in the cardiac cath lab and will be comfortable with equipment required for the procedures. The PA will act as the responding clinician for patients on the PA Cath Service with responsibilities including admission H&P, daily rounds, progress notes, and discharge summaries. The PA must have knowledge of the most common diagnoses admitted to the Interventional Cardiology service, demonstrate competency to effectively diagnose, formulate an appropriate treatment plan, and implement the plan in conjunction with the attending physician. The physician assistant will be responsible for completing and maintaining procedural sedation training annually and completing fluoroscopic training and certification within 18 months of hire. While the PA can expect a relatively predictable work schedule, schedule variations may occur to cover clinical needs as they become apparent to the department. The position will include weekend and holiday coverage planned in advance. PA may be asked to take call, pick up additional shifts, work variable hours, and/or provide backup coverage as needed. Job Summary GENERAL SUMMARY/ OVERVIEW STATEMENT: Under the direction of the Lab Manager and with the general supervision from the Senior Tech Specialist, the technologist will be required to perform standard and complex all night sleep recordings as well as multiple sleep latency tests. Monitors additional physiologic phenomena by means of appropriate technique. The polysomnograms may measure oxygen saturation, EKG, EEG, respirations, ETC02 levels, upper airway resistance, CPAP and BI-PAP levels and other parameters necessary for a complete sleep study. After an initiation period of 4-6 weeks the tech would be required to work independently not necessarily alone for up to 12 hours. The technologist must be able to make appropriate decisions regarding the diagnosis and treatment of benign to the most complex patients. May perform clerical and other laboratory duties required. PRINCIPAL DUTIES AND RESPONSIBILITIES: * Examines physicians' orders and protocols for each sleep study. Reviews existing information in the patient's record and recognizes chief sleep/wake complaints. - Greets and interviews the patient to assess current medication, recent caffeine/alcohol ingestion, current sleep patterns and the need for treatment intervention during the study. Communicates with the ordering physician or unit director to clarify unclear of inappropriate orders and communications information regarding the patients' status. * Must spend appropriate time in servicing the patient regarding the possible use of CPAP/BI-level equipment. The patient's fears apprehensions concerning CPAP should e reduced or eliminated by the in-service. * Sets up/calibrates monitoring equipment with appropriate montage. Measures and applies necessary EEG electrodes using the 10/20 system. Applies appropriate EKG, EMG, and EOG monitoring electrodes. Places and securely attaches thermistor to measure nasal/oral airflow, respiratory effort, oximetry, and ETC02. Administers supplemental O2 under physicians' order. * Performs physiologic calibrations, verifies the integrity of the recording. Institutes "lights out" and notes patient data every 15 minutes on a tally sheet. Monitors the polysomnogram for clinical and physiological events. Must be able to interpret severely abnormal data/EKG rhythms and report such findings to the sleep physicians. Notes and corrects equipment malfunctions. Understands indications, contraindications and adverse effects of nasal continuous positive airway pressure (NVPAP, BI-level and oxygen administration), applies and titrates therapy based on physicians' orders and established protocols. * Recognizes and responds appropriately to the patient's medical problems and physiological events, especially cardiac arrhythmias, respiratory events and seizures. Alters the patient's environment or changes the patient's body position as needed to obtain the required sleep/wake data. Job Summary Summary The operating room is an extremely stressful and demanding environment, requiring the technologist to perform rapid and accurate evaluations of waveforms and the physiological and environmental forces that impact the waveforms. The Technologist II is also required to maintain multi-thread communication between medical teams while efficiently troubleshooting and analyzing the NIOM waveforms in real time. The technologist must perform these tasks accurately and efficiently, as communicated information regarding waveforms may have profound implications for the surgical course and safety of the patient. In collaboration with the IOM Supervisor and the NIOM attending neurologist, the technologist performs all entry-level NIOM cases independently and trains on Specialist I competencies as described by MGB Neuro Intraoperative Monitoring protocols. Technologist II works under the supervision of a more experienced technologist in cases where competencies have not been completed and as determined by the IOM Supervisor. Technologist II may be trained and supervised by Specialist I and Specialist II. Technologist II is responsible for the NIOM recording and documentation of all entry-level SSEP, TcMEP, EMG/Triggered EMG, BAEP, and EEG cases. The technologist acts as an interactive part of the operating room team, assisting in duties as appropriate and maintaining open channels of communication with and between medical teams. As the MGB IOM division continues to research and apply intraoperative monitoring methods and techniques to new procedures, the technologist will be required to learn new protocols as they are developed and may be asked to participate in the development of protocols. Does this position require Patient Care? Yes Essential Function * Provides monitoring for all entry-level surgical procedures requiring NIOM. Technologist II will be supervised by an experienced technologist, as determined by the IOM Supervisor, when performing cases that exceed their competency level. * The technologist collaborates with the attending neurologist, who provides the oversight for the case. The technologist monitors the surgical procedure and provides continuous technical interpretation to the surgical team. This may include, but is not limited to, changes in waveforms, special requests, changes to the surgical plan, and details for remote connection. * The technologist maintains detailed and accurate documentation, which requires knowledge of case types and of all modalities being performed. * The technologist maintains open channels of communication with the attending neurologist, as well as with all the medical teams involved in the case. * Technologist II is responsible for preparing patients prior to draping for surgery in accordance with MGB standards and for setting up for all possible/appropriate monitoring procedures. Patient preparation includes accurate and secure application of electrodes and stimulators to ensure quality recordings for long durations. The technologist measures the electrical impedance and examines the integrity of all NIOM electrodes, and when necessary, fixes issues occurring both prior to incision and throughout the procedure. The technologist also appropriately directs the surgeon in the application of sterile electrodes and stimulating devices as needed. * Technologist II notes relevant clinical information from the patient's chart and from the surgeon as necessary. * Technologist II prepares a preliminary technical report describing the recording throughout the procedure. While monitoring, meticulously documents surgical procedure, vital signs, anesthetics, and anesthetic changes, phases of the procedure, and communicates all relevant information with the surgical teams in a timely fashion. * Technologist II covers evening and weekend call shifts and evening shifts, as assigned by the IOM Supervisor. * Assists in the maintenance of databases and patients' permanent documents. * Keeps abreast of current developments in the IOM field and participates in educational conferences, meetings, and/or other activities that promote continuing education. * Applies infection control standards, aseptic, and sterile techniques, with particular attention to any monitoring instrumentation that enters the sterile field. * Technologist II is working towards completing Lead Technician competencies. Specific Urology Duties: * Principally focus on support of the procedural areas of the Urology ambulatory practice. * With the assistance of a peers, manage up to 50 procedural patients daily. * Develop and maintain an inventory of procedure-specific equipment and supplies to par with inventory status and location communicated to staff, care team, etc. * Monitor procedure schedules 48 and 24 hours prior to procedure dates to ensure proper scheduling of exams. Collaborate with procedure team to resolve any mis-scheduled exams. * Prepare procedure rooms with the necessary supplies, equipment, etc. for each unique procedure. * Maintains systems of care and preventative maintenance of all equipment. * Able to set up Urology procedure suites for varied procedures, including but not limited to cystoscopies, bladder and prostate biopsies, UDS, vasectomies and laser procedures. * Contacts vendor with equipment issues and communicates to staff and care teams in such scenarios. If needed, arranges for alternative testing times and locations for patient care impacted by equipment outages. * Maintains eye wash station and corresponding log sheet. * Prepares Urology procedure suites consistent with procedures scheduled to be performed (e.g. surgical clamps for vasectomy; ultrasound probe for biopsy; flexible cystoscope for cystoscopies). * Prepares for and assists in Urodynamic (UDS) testing procedures, including cystometrogram and uroflowmetry. Proficiently and safely operates UDS equipment. * Efficiently prepares procedure rooms for emergency and unscheduled procedures. * Within procedural support, observes all universal protocols and precautions. * Orders procedural and other supplies for Urology practice. * Understands the reprocessing of all utilized equipment.
