Matheny is a special hospital for children and adults with medically complex developmental disabilities Mathenys mission is to provide exceptional care and an optimal quality of life for children and adults with special needs and medically complex developmental disabilities. Each patient has a primary nurse who coordinates all his/her care needs. Our nurses and nurse managers work closely with our paraprofessional care staff for on-going communication and teaching about each individual patient. We maintain close contact with families and acknowledge their importance as a valued member of the healthcare team. Matheny currently has a career opportunities available for Full Time Registered Nurses for the 7pm-7am shift. Qualified candidates who are hired for Full Time employment will receive a Signing Bonus of $3,000.00. ($1,500.00 payable after completion of six months of continuous employment and the remaining $1,500.00 after completion of twelve months of continuous employment). Duties of this position include: Demonstrates effective assessment skills by collecting and analyzing data, interviewing, observing, and examining patients, and through review of the patients medical record. Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served Formulates a comprehensive and unique plan of care for each patient; monitors patient outcomes to treatments and revises the plan as needed. Demonstrates critical thinking skills to determine appropriate nursing interventions and/or the need for collaboration with the physician or other clinicians. Works with and supervises the PCA staff / unlicensed assistive personnels work, who do the direct patient care of ADLs, bathing, assisting with eating, etc. Oversees the proper infection control practices within the work environment especially now with Participates as a member of an interdisciplinary team which is comprised of all members of the patient care team- therapies, social services, physician, psychology, clinical nutrition and direct support professionals Communication with families/guardians on all patient-related issues is a large aspect of our nurses role. As a Registered Nurse in a special hospital, performance of additional responsibilities that are not outlined in this job description may be required in an emergency situation. Additional assistance as a result of staff absences or shortages is also required, including additional weekend and/or holiday time. Our NICU is a 13-bed unit taking care of infants born as early as 28 weeks. Patients are admitted after birth for any complications, genetic disorders, or prematurity. We also care for infants in need of respiratory support in the form of Nasal Cannula, High Flow Nasal Cannula, CPAP, SiPAP and SIMV. Our NICU also has a collaboration with the Children's Hospital of Philadelphia and has a Neonatologist in house 24/7. If an infant is born outside of our criteria, we will stabilize the infant and transfer to another facility. Scheduling Requirements (Tier 3): + Work a minimum of five shifts per four-week schedule + 24 of these hours must be fulfilled on a weekend shift. Weekend night shifts are considered Friday, Saturday, and Sunday. + Work two holiday per calendar year. Per Diem RNs will be paid at 1.5 times the rate for all hours worked on these holidays. + Based on your hired shift, work at least two 'difficult to staff days per calendar year (Easter, Mother's Day, Halloween, 'Black Friday', Good Friday, Christmas Eve, Valentine's Day or New Year's Eve) + Follow departmental call-out policy + Make-up absences within the same schedule period. Lactation services are provided by our board-certified lactation consultants and supported by our nurses who have extensive breastfeeding training . Mothers at risk for postpartum hypertensive complications are counseled and enrolled in our post-discharge surveillance program, Heart Safe Motherhood. Newborns are evaluated daily by pediatricians or neonatologists and can be taken to our Neonatal Intensive Care Unit (NICU) should further evaluation be needed or complications arise. Newborn care procedures include hearing screens, evaluation and management of hyperbilirubinemia, car seat testing, screening for critical congenital heart defects, and metabolic screening. Patient education is central to postpartum care and address topics such as newborn care, maternal care and recognition of early warning signs of possible postpartum complications. At Matheny, we focus on abilities in a warm and nurturing environment. Matheny Medical and Educational Center currently has an opening for a Full Time Transition Nurse. The Transition Care Nurse's primary role is to improve transitions to and from acute care hospitals for patients with multiple chronic conditions associated with developmental conditions such as cerebral palsy, spina bifida, Lesch-Nyhan disease and autism spectrum disorders. Transitions will be the first priority of this nurse's role; thus this nurse will work with the Medical Trip nurse and the Wound care nurse to provide transition services for the hospital patients in total. The additional role of this nurse will be to work with the Nurses on the Clinical Units to improve their skills when working with any new procedure or protocol that a patient has, who has been admitted with, or re-admitted with, to Matheny Medical and Educational Center. This role is essential in communication between the physicians from Matheny and the physicians from the outside hospitals when a patient of Matheny is admitted to an outside agency. Desired Qualities of a Transition Nurse: Respect Professionalism Integrity Compassion Duties of the Transition Nurse include: Advocates for patients residing in the Matheny Medical and Educational Center during transitions to and from acute care hospitalization. Consults with Matheny staff regarding the situation, background, interventions taken and plan for patients' health care needs. Ensures a smooth and effective transition among care settings and the transmittal of important information about the patient via the 'Transition Medical Record'. Ensures accurate medication reconciliation from the patient's home to hospital and on return to home. Follows the patient's post-discharge health status for one month and recommends additional consultations as necessary Fosters collaboration between Matheny, acute care hospitals and families. Conducts daily follow-up visits on all hospitalized patients, including consultations with their healthcare team members. Maintains communication with healthcare providers at the acute care hospital as to the patient's status during hospitalization and collaborates with discharge planners for the patient's return to Matheny. Provides in-service education and support to acute care hospital healthcare teams and families/caregivers regarding special care needs of patients with developmental disabilities, specific to the individual patient. Education of the Matheny staff regarding changing care needs post-hospitalization is a key aspect of his nurse's function. Participates in pre/post planning meetings and initiates post-hospitalization care plans with the primary care nurse on the clinic unit. The Transition Nurse/Nurse Educator will assure that we use evidence based practices pertaining to transitions, transitional care, reducing readmissions, and improving care for developmentally disabled complex care patients. directed through the Matheny Research Institute pertaining to transitions, transitional care, reducing readmissions, and improving care for developmentally disabled complex care patients. Will attend and participate in conferences pertaining to developmentally disabled and complex care patient needs as well as transition nursing to support the Transition Nurse role which could build capacity in the community for the proper health care of children and adults with developmental disabilities. Is a member of the Nursing Administration at Matheny Medical and Educational Center, thus taking call once every 7-8 weeks on the weekend as the other members of the Nursing Administration.
