Responsibilities:
* Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
* Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
* Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
* Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
* Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention

Requirements:
* Current Registered Nurse (RN) License for the state in which the nurse practices.
* Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
* Current PPD or Chest X-Ray.
* Current BLS card.
* One year prior Registered Nurse (RN) experience preferred.

Responsibilities for a PT - Physical Therapist include:
* Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.

Qualifications for a PT - Physical Therapist include:
A current OH Physical Therapist - PT license.
A minimum of one year of recent work experience as a Physical Therapist - PT.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.

Qualifications include:
* Four year college degree preferred
* Home Health experience is required
* Minimum two years of verifiable supervisory or management experience in the healthcare industry, preferably in home health care
* Knowledge of Medicare regulations, including OASIS and PPS
* Knowledge of Pennsylvania regulations
* Demonstrated record of goal achievement and of successfully taking on increased responsibility with positive results
* Proven interpersonal, recruiting and employee relations skills
* Proven ability to organize, manage, market and grow an office
* Demonstrated PC and communication skills, especially in regard to networking with the community and representing our organization to various groups and agencies
* Ambition to grow and advance beyond current position and responsibilities
* Bilingual in Spanish and English a plus

How you make a difference
The Medical Director of Utilization Management leads and oversees utilization review, case management, quality improvement, and related policy and practice initiatives within their assigned area. They provide guidance and direction to medical staff in their efforts to ensure quality patient care and the appropriate utilization of medical services. The Medical Director of Utilization Management serves as a key liaison with external partners and stakeholders, and works collaboratively with internal teams to optimize care delivery and achieve operational goals.
Key Responsibilities
Ensure the use of nationally recognized criteria and evidence-based standards for inpatient concurrent reviews and offsite service requests.
Manage UM staff including performance reviews, employee development, hiring, coaching, counseling, and retention.
Lead UM process improvements, facilitate patient care goals, and utilize data research to reduce length of inpatient stays and decrease ED admissions.
Evaluate and recommend policy improvement related to utilization review system, and provide UM updates and participate in meetings.
Establish and monitor progress towards UM program goals, maintain offsite service and UM tracking/reporting, and facilitate regular meetings with partner hospital clinical regarding offsite services.
Additional Details This is on onsite role based in Lemoyne, Pennsylvania at our Regional Office.
Qualifications & Requirements
Education
Medical Doctor (M.D.) or Osteopathic Doctor (D.O.) from an accredited university
Experience
One (1) year of experience in utilization management is required.
Excellent communication and organizational skills required.
