This role collaborates closely with a multidisciplinary team to support patient care through direct involvement in patient visits, education, and follow-up. Schedule: This PACU RN will be working a 0.75 FTE (60 hours every two weeks) on the rotating day/evening shift and 12 hour shifts. Start times vary with the earliest being 5am and the latest being 3pm. This position does have off site on call requirements. The weekend requirement is on-call every 6th weekend 12 hour day/night rotating and one weekday night per month. Call requirements are above FTE.

The RN Care Coordinator focuses on the needs of the patient and their family as they access care from the health care home. The RN serves as a resource and educator to the patient and their family, and coordinates care efforts for a safe, effective, efficient and patient centered transition along the health continuum. The RN provides pro-active disease management using evidence based practices; acts as a liaison between patient/family, medical staff and others involved in treatment (ie ER, Hospitals, nursing homes, home health care and the community at large) promoting patient/family-centered care based on jointly determined health care goals and priorities as defined in the individual care plan; and coordinates implementation of health care home improvements (patient registry, care planning, health outcome data management). As a member of the care team, the RN Care Coordinator supports quality patient and family-centered care principles through performing a variety of high standard functions within a care team to support the needs of the healthcare team and patients/families.
Schedule: This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the day/evening shift, weekends as needed. This position will mainly cover primary care. You will be primarily based in Hibbing, but may provide coverage in Nashwauk and Mountain Iron, as needed.
