Position Summary: Under a grant from the Office of Refugee Resettlement (ORR), NYAP's Home Study (HS) Caseworkers complete in-person visits to sponsor family homes. They conduct in-depth assessments of the potential sponsor's ability to ensure the child's safety and well-being, conduct face-to-face interviews with sponsors, other household members, and initiate background checks. HS Caseworkers must have experience in conducting HS according to the requirements in the state where services are being provided. They must be skilled at writing professional, thorough assessment reports. HS caseworkers must have knowledge of local community social services and should have specialized experience in conducting home studies and working with immigrant populations. Additionally, HS caseworkers should be knowledgeable about family preservation, kinship care, and/or general child welfare. Required travel includes in-person visits to sponsor family homes. Responsibilities: * Complete in-person visits to sponsor family homes. * Conduct in-depth assessment of the potential sponsor's ability to ensure the child's safety. * Conduct face-to-face interviews with sponsors and other household members. * Initiate background checks of the sponsor and adult household members. * Identify areas where additional support is needed to ensure a successful sponsorship. * Ensure completion of HS within ORR's designated timeframes. * Knowledge of UC Portal and Evolv. * Assists with post release services, if needed. * Provide well written and documented final HS recommendations of sponsor. * Carry a minimum case load of 3. * Have a case management approach that assesses, plans, facilitates, and advocates for unaccompanied children and their sponsors using knowledge of social services and human services. * Follow all ORR guidelines and policy requirements. * Reports to PRS Caseworker Supervisor. * Perform other duties as assigned. MINIMUM QUALIFICATIONS * Must be at least 21 years old. * Bachelor of Social Work (BSW) degree or an equivalent degree in education, psychology, sociology, or other behavioral science equivalent. * Experience in conducting HS. * Skilled at writing professional, thorough assessment reports. * Knowledge of local community social services and human services. * Have specialized experience conducting HS and working with immigrant populations. * Bilingual in English and Spanish. * Prior experience with ORR HS/PRS preferred. * Pass a criminal history screen, including state and local child protection agency registries, and C/ANs check. * Ability to travel up to 50% of the time with an active motor vehicle license, reliable automobile, a good driving record, and 100/300 insurance coverage on personally owned vehicle. * Willingness to work flexible and non-traditional hours in the service of persons served. * Must live within a 100-mile radius of the city they will be focused on supporting. * Travel required to states within assigned region. Treatment is individually designed for each individual based on a recovery model. Presenting mental health issues most often include: Schizophrenia, Schizoaffective, Bipolar, PTSD, and Major Depression. All clients are low income. Approximately 80% of area residents are ethnic minorities. Telecare's unique culture and approach makes it an amazing experience for students who are willing to PLAY HARD and WORK HARD! Direct Services Community Programs: include mental health rehabilitation individual and group style, as well as individual targeted case management. The majority of the services are conducted in the field, groups are done in the office. However, due to the COVID -19 crisis, many of the services are performed remotely via Telehealth. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Provides case management support to individuals engaged in behavioral health services, including community psychiatric support and therapeutic support, to promote the coordination and implementation of treatment goals. • Serve as a liaison between families and agencies to positively promote collaborative processes in client treatment planning by encouraging parent/guardian participation in treatment. • Identify and assess psychosocial and life skills needs through analysis of factors impinging on client success , including home, school, and community factors. • Provide parent/guardian educational workshops on identified issues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication. • Manage phone calls from families, guardians, agencies, etc. Set up meetings, obtain information, etc. • Develop programs to address parent/guardian participation in agency services and client engagement in the treatment process. • Attend meetings, training, and professional development activities as required. • Completes mandatory documentation within timelines established by applicable regulations and policies. • Mediates issues that arise among clients, families, and staff. • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational, transportation, healthy/safety, employment, etc.). • Provides clients and families with crisis management services as needed. • Work closely with an interdisciplinary team to support clients and coordinate care. • Develops a comprehensive discharge plan in collaboration with clients, client's families, multidisciplinary staff, and referral agencies. • Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills. • Advocates on a case, policy, and program level. • Meet productivity requirements as established by management. • All other assigned duties.
