Notify physician of changes in patient's clinical status and/or abnormal test results.
Collect urine samples and process specimens for courier transport.

Phamily is assisting Arizona Kidney Disease and Hypertension Clinic is hiring a Chronic Care Coordinator to support nephrologists and advanced practitioners in managing chronic care patients. This role involves using the Phamily Chronic Care Management platform to improve patient care, prevent unnecessary hospitalizations, and enhance communication between patients and their care teams.
Under the direction of the Office Coordinator, the Patient Care Coordinator/Medical Assistant will be responsible for performing general medical assistant functions in support of the care and treatment of patients while providing assistance to all office personnel as needed.Responsibilities:
Perform patient care activities, such as back-office check-in, check-out, prescription refills, anemia, and iron management, administer injections, schedule procedures and provide patient education.
Schedule and process 24-hour blood pressure monitoring.
Interview patients, measure vital signs, such as pulse rate, temperature, blood pressure, weight, height; and update medication list; record information in patients' charts/EMR to include urine tests per Clinitek.
Notify physician of changes in patient's clinical status and/or abnormal test results.
Collect urine samples and process specimens for courier transport.

Hospital Corpsmen (HM) assist health care professionals in providing medical care to Navy personnel and their families. They may function as clinical or specialty technicians, medical administrative personnel and healthcare providers at medical treatment facilities. Specifically, Hospital Corpsmen may be called upon to:
Perform emergency medical treatment on SEALs, Seabees, Marines and other military personnel injured in the field, as well as on Sailors aboard ships or aircraft
Perform emergency dental treatment as well as construct dental crowns and bridges, process
dental X-rays and operate X-ray equipment
Serve as an operating room technician for general and specialized surgery
Help administer a wide range of preventive care and medications, including immunizations and intravenous fluids
Conduct physical examinations and assisting in the treatment of diseases and injuries
Maintain patient treatment records, conduct research and perform clinical tests
Assist Navy Physicians and Nurses in a variety of medical fields, including, but not limited to: radiology, physical therapy, phlebotomy, dental, surgery, family medicine, pathology, womens health and more

Responsibilities
The Medical Assistant will be caring for patients in the following capacity:
* Assists provider with exams (including patient vitals), procedures and other processes related to direct patient care under direct supervision of the Provider and within the scope of practice of a Medical Assistant.
* Administers all forms of medications per policy and Provider direction and adheres to all safety protocols and procedures for medication administration.
* Will perform blood draws as per direction of the Provider
* May be responsible for urine collection for various testing to include employer drug screens
* May perform splinting and casting
* Conduct various type physicals such as sports physicals
* Performs other related duties specific to urgent care such as ear lavage, wound care and oxygen administration.
* Notifies the provider on duty promptly of all unusual patient occurrences including medication events on an incident report.
* Promptly notifies the Clinic Manager of patient or employee safety concerns.
* Follows guidelines and requirements of laboratory and clinical accreditation policies.
* Must be proficient in EMR/EHR
* Performs other related duties as assigned or described by Company policy.
As a Medical Assistant, you may also be asked to support Front Office Reception. When working in the front office you will perform such duties as: keeping patients and families informed of wait times, monitor the flow of patients, process patients for discharge including preparing charges, collecting payment, obtaining all necessary signatures and issuing receipts.

* American Association of Medical Assistants (AAMA) - administers the Certified Medical Assistant (CMA) exam and certification; or
