Job Title: RN PACE Job Summary Under the general supervision of the clinic manager RN and medical director of FLORIDA PACE CENTERS, provides nursing care to all persons enrolled in the program. Essential Job Functions * Performs comprehensive nursing history and physical on new referrals. * Creates appropriate care plan and participates in IDT. * Evaluates and triages participants with episodic illnesses * Refers participants to medical director or medical specialist as indicated * Actively participates in family conferences. * Participates in interdisciplinary team meetings, committees and work groups as assigned.
