RESPONSIBILITIES: Under the supervision of the Guardian Scholars Supervisor, support a caseload of approximately 25-35 high need youth requiring daily driving to meet youth where they are in the community. Implement evaluation and assessment tools to ensure programmatic data and outcomes can be collected and analyzed for outcome management. Support youth trainings and workshops, as needed. Ensure Salesforce is up to date with accurate files, notes, and documentation. Salesforce must be updated daily per phone calls, emails, and visits. Conduct monthly in-person meetings with youth to address challenges, track progress, and provide support, supplemented by additional check-ins via phone, text, or email as needed. This position requires regular evening and weekend hours to meet participant needs. Address any youth issues as they arise including career navigation, homelessness, and academic needs. As part of the team, support youth development program components including but not limited to college and career tours, Women's and Men's Leadership Network, Advocates for Change, care packages, Mentoring Department, internships, Guardian Scholars Resource Fair, and youth training and events. Assist in grant writing and fundraising activities as it pertains to Guardian Scholars. Provide support to volunteer mentors to ensure their effectiveness with youth and satisfaction with the program.     Amber House Crisis Stabilization Unit (CSU/CRT) Clinician Amber House CSU is a 10-bed voluntary-only CSU whose purpose will be to assess individuals who are having a mental health crisis and are in need of assessment, stabilization, and brief treatment. Amber House CRT has up to 14-beds for individuals in crisis who do not meet medical necessity criteria for hospitalization and would benefit from treatment and supportive programming. Build positive clinical rapport to maintain a productive and active caseload to promote medication adherence, increase stability and wellness, and apply harm reduction practices. Link and navigate MOUD clients to primary health care, appropriate mental health services, and assist clients in accessing housing and the necessary documentation to support stabilization and positive outcomes. Outreach, education and link client caseload to full range of Glide services as well as the relevant external community resources to achieve stability and self-sufficiency. Plan, conduct outreach for, and facilitate MOUD, treatment and other harm reduction groups on a regular basis. Document and maintain complete files on clients including updated intakes, assessments, case management notes, group attendance logs, referrals and other required data captures in a timely and accurate manner in accordance with program standards, organization policy, and applicable local, state, and federal laws for MOUD services. Submit monthly activity and data reports that capture contract deliverables in required database systems. Adhere to standard professional boundaries, understand and follow basic ethics and values of case management regulatory practices, and understand and adhere to HIPAA protocols. Develop and prepare outreach and educational materials targeted to engage the broader community in HEAT MOUD programs. Assist the leadership team in coordinating special events that promote HEAT MOUD efforts as well as the full array of HEAT and Glide wide programming. POSITION SUMMARY: Reporting to the Program Supervisor, the Early Childhood Educator is responsible for creating and implementing inclusive classes designed for parents/caregivers and children ages 0-5 to participate in together. (Please note this is not a childcare facility.) The Neighborhoods Ready for School (NRFS) initiative is a place-based collaborative, multi-year partnership between the Mandela Family Resource Center (MFRC) and First 5 Alameda County that supports equity and neighborhood-led investment, programming, and policy related to early childhood development, family health and well-being, and kindergarten readiness. ESSENTIAL FUNCTIONS: * Provides case management services to families in a tiered management model. * Creates and provides daily family literacy, early learning opportunities, and parent education classes. * Creates and provides pre-K transition programming that includes parent education and parent-child interactive classes that simulate preschool experiences. Programming is designed to help families transition their children to high-quality preschool programs. * Researches and develops curriculum based on best practices and evidenced-base early childhood programs. Also researches new programs based on target population needs. * Attends trainings on early literacy and parenting education to grow craft and understand best practices. * Provides support to families who come to the Mandela FRC and communicate with families as necessary to facilitate consistent attendance at classes. * Connect with local preschools, office of education, and other professionals in the preschool field to stay current with pre-K trends, issues, and educational strategies. * Conduct outreach and recruitment activities to the targeted population to effectively promote program services. * May develop agency material including project flyers and other outreach materials. * Complete and maintain all service documentation and statistics as required. * Participate in agency meetings and community meetings as directed. * Requires driving to conduct meetings at homes or other sites and may include transporting youth or families.
