Nascentia Health Options seeks a compassionate and detail-oriented RegisteredNurse to join our team as an Assessment RN. In this role, you will assess prospective enrollees and existing members to determine plan eligibility and develop person-centered care plans that support long-term health and independence. You will be a key liaison between members, families, and care teams, ensuring services align with regulatory requirements and individual care needs. This is a full-time role that supports business schedule of Monday through Friday from 8am- 4:30pm.
Performance Responsibilities and Standards:
* Conduct in-home and virtual assessments of prospective and current health plan members.
* Evaluate clinical needs, functional and behavioral health status, and social determinants of health using UAS-NY and other assessment tools.
* Educate members, families, and caregivers on plan benefits, rights, responsibilities, and services.
* Develop person-centered service plans that identify unmet needs and recommend appropriate supports and services.
* Use motivational interviewing and active listening to understand member goals and build trust.
* Review member materials, including handbooks, provider directories, and covered services.
* Maintain detailed documentation in the electronic medical record (EMR) and follow internal quality protocols.
* Communicate and coordinate with care managers, providers, and other interdisciplinary team members.
* Ensure all assessment activities comply with DOH and Medicaid Managed Long-Term Care (MLTC) regulations.
* Attend staff meetings, trainings, and participate in departmental quality improvement initiatives.
* Serve as a professional representative of Nascentia Health Options in the community.
* Meet or exceed productivity and scheduling standards.
Qualifications:
* License: RegisteredNurse (RN) licensed and in good standing in New York State
* Education: Associate degree in Nursing required; BSN preferred
* Experience:
* Minimum 1 year of nursing experience preferred
* Experience with UAS-NY assessments strongly preferred
* Knowledge of long-term care, home care, Medicaid/Medicare preferred
* Skills:
* Strong verbal and written communication skills
* Proficiency in Microsoft Office and ability to learn EMR systems
* Independent, detail-oriented, and able to manage a flexible schedule
* Bilingual abilities a plus
* Other:
* Valid NYS drivers license and reliable transportation
* Ability to drive and work in various environmental and weather conditions
Work Environment & Physical Requirements:
* Hybrid role: Primarily home-based with required in-home member visits and occasional in-office meetings
* Must be able to sit, walk, stand, drive, and perform data entry throughout the day
* Ability to lift up to 20 lbs frequently and up to 50 lbs occasionally
* Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
* Requires sensory acuity for communication, observation, and documentation
Compensation & Benefits:
Competitive Salary
This is a per-visit position with an estimated annualized range of $80,000$100,000+, based on visit volume*.
* Actual compensation will vary depending on the number of completed visits per pay period. A minimum average of 20 visits per pay period is required to maintain full-time status.

In this role you will be working in a general acute child/adolescent psychiatric inpatient unit with various types of psychiatric, behavioral and adjustment problems that affect their family, community and school life. Our purpose is to provide a safe and therapeutic environment for adolescents to work on psychiatric, behavioral and adjustment issues. 1. Ensure complete and accurate assessment and delivery of skilled nursing services.
a. Determine appropriateness of patient for home care.
b. Develop patient centered plan of care based on comprehensive assessment.
c. Develop a plan of care using interdisciplinary approach.
d. Ensure that all documentation meets quality standards:
1. OASIS evaluations are conducted per CMS guidelines.
2. Documentation reflects nursing care, treatments and responses.
3. Documentation is clear and concise.
4. Documentation is completed in timely manner.
e. Understand and follow nurse code of ethics and standards of practice.
f. Coordinate with internal and external resources to meet patient needs.
g. Maintain established productivity expectations.
h. Communicate client care issues to interdisciplinary team including physician.
2. Responsible for coordination of services.
a. Identify needs and refer to additional services as indicated to meet patient goals.
b. Report findings and plans to designated RN Case Manager/Care Team and/or Clinical Supervisor.
c. Work in conjunction with Supervisor and designated staff to efficiently manage patients using a team approach.
3. Responsible for development of the care plan for paraprofessional staff relevant to patient care as applicable.
a. Supervise home health aides in performing designated tasks related to patient care and per care plan.
b. Communicate appropriate information to home health aide department.
4. Comply with Nascentia policies and procedures and quality initiatives.
a. Maintain confidentiality.
b. Demonstrate knowledge and use of infection control practices.
c. Demonstrate knowledge of federal, state and local government rules and regulations.
d. Demonstrate knowledge and compliance with agency policies and procedures.
e. Adhere to quality improvement standards.
f. Keep personnel requirements up-to-date; i.e., health requirements,
licensure requirements, and mandatory in-services.
g. Demonstrate proficiency with skills competency through periodic evaluations.
5. Participate in personal growth and development.
a. Participate in educational in-services and meetings.
b. Maintain and update clinical knowledge on an ongoing basis.
6. Represent self and agency in a professional manner including compliance with agency dress code.
7. Other duties as assigned.
Requirements:
1. Current New York State registered professional nurse license.
2. Degree from an accredited school of nursing or college.
3. Current and valid driver s license and auto insurance, and a vehicle
which can be used for work.
4. Self-directed and organized with the ability to work independently.
5. Good oral and written communication skills.
6. OASIS Certification, or willingness to obtain within 1 year of hire.
7. Minimum one year experience in Home Health Care required.
Physical Requirements:
1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
2. Ability to perform tasks involving physical activity including:
- Pushing/pulling - Typical weight 50 pounds / Maximum weight 150 pounds
- Lifting and carrying - Typical weight 25 pounds / Maximum weight 40 pounds
- Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g. snow, rain, sleet, and going up and down stairs.
3. Ability to drive in all weather conditions throughout the working year.
4. Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
