    Title: Case Manager FLSA Classification: Full-time (35 hours), Non-exempt Role Summary: We are seeking a compassionate and dedicated Case Manager to join our team. The successful candidate will provide a comprehensive range of on-site, person-centered, and strengths-based services to a caseload of approximately 20-25 clients. Your primary objective will be to empower individuals to lead fulfilling lives within the community. As a Case Manager, you will play a pivotal role in client counseling, intakes, case management, and providing essential aftercare services. Collaborating with colleagues is key to meeting the diverse needs of your caseload and achieving program success while aligning with NAICA's core purpose, values, and vision. Primary Job Responsibilities/Duties: The Case Manager's role encompasses, but is not limited to: Conducting initial intake assessments and promptly addressing critical needs. Utilizing assessment tools, such as CARES, to identify support service requirements and make immediate referrals. Orienting clients to the shelter program, its requirements, and potential consequences. Developing Independent Living Plans (ILPs) in coordination with other Social Service providers, ensuring the delivery of comprehensive support services. Assisting non-working clients in accessing Public Assistance. Completing psycho-social assessments within the first 30 days of intake. Monitoring and tracking client progress. Empowering clients to actively participate in their own planning and goal setting. Conducting regular meetings with clients (weekly/bi-weekly or as needed).Referring clients for job development opportunities if appropriate. Connecting clients with suitable resources to help them achieve their goals. Meeting with clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency. Maintaining thorough client files, documenting conversations, warnings, and progress reports, while preserving confidentiality. Reporting critical incidents immediately to the Site Director/Director of Social Services. Collaborating closely with other Housing Specialists and Employment staff to identify client readiness for employment and housing. Treating all clients, visitors, and employees with kindness, respect, and dignity. Serving as an advocate for clients when necessary. Providing periodic case management and counseling, extending one year beyond the client's departure. Establishing and nurturing connections with other programs and units within the organization to facilitate inter-agency referrals. Escorting clients to housing viewings, DHS or HRA appointments as required. Facilitating suitable housing placements for clients. Ensuring that case notes are completed within 24 hours after client meetings. Offering support as a substitute for other staff when needed. Performing general clerical duties. Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Ability to sit for extended periods and perform repetitive tasks. Must be able to lift and carry up to 5 pounds. Must be able to travel to multiple NYC sites as needed. Must be able to access and navigate each department at the organization's facilities. Ability to climb stairs. Work Environment / Schedule Requirements: Office setting with regular exposure to computer screens and moderate noise levels. May be required to work shifts, including evenings or night tours and/or Saturdays, Sundays and holidays with holiday pay differential. Transitional Housing Program staff are required to continue their shift until relieved by another staff member or authorized to leave by a supervisor. Qualifications: Bachelor's degree in social work, psychology, sociology, or a related field with at least 2 years of direct social service experience. Associate degree in human services or a related field with a minimum of 4 years of direct social service experience. High School Diploma/General Equivalent Diploma with a minimum of 8 years of direct social service experience. Exceptional interpersonal skills, with the ability to balance compassion and firmness while always maintaining confidentiality. Proficiency in navigating community resources. Strong problem-solving abilities, decision-making skills, conflict resolution expertise, and active listening. Capacity to remain composed in crisis situations. Demonstrated ability to collaborate effectively with diverse groups. Proven capability to manage multiple tasks efficiently under pressure. Outstanding organizational skills, attention to detail, and efficiency. Possessing maturity, integrity, and sound judgment. Must be able to read, speak, write, and understand English for administrative purposes. Must pass drug screening to be appointed.     Job Description Title: Housing Specialist - Shelter Pay Range: $26.37 - $27.47 per hour (effectively $48,000 - $50,000 per year) FLSA Classification: Full-time (35 hours), Non-exempt Role Summary: The successful candidate will be responsible for providing housing services to help clients understand and overcome personal or social issues affecting their housing. The housing specialist will cooperate with case managers to develop service plans for clients to ensure permanent and stable housing and access to community sources. The specialist will be encouraged to be cross-trained and have an understanding of the job functions within the Social Services Department. Primary Job Responsibilities/Duties: The Housing Specialist - Shelter is responsible for, but not limited to: Serve as a case manager for clients with a goal of placing them in transitional and permanent housing Assist in client intake, service planning, advocacy and referral services in the absence of a case manager Meet weekly or bi-weekly to follow-up on ILP Maintain records and counsel session logs in DHS CARES system Conduct housing related activities and workshops Provide follow-up case management for clients placed in housing, to promote retention and document outcomes (aftercare mediation) Communicate with evening/weekend staff and with staff of other agencies to coordinate client services such as room and apartment viewings Develop a list of real estate agents and brokers; established relationships, and make daily contact with providers with real estate agents and brokers Submit applications for General Population, Housing Preservation Development, and Supportive Housing via the PACT System.     Title: Senior Case Manager FLSA Classification: Full-time (35 hours), Non-exempt Role Summary: The Senior Case Manager plays a pivotal role in overseeing the comprehensive operations of our program, collaborating closely with the Case Manager Supervisor. This entails planning, development, implementation, monitoring, evaluation, and continuous enhancement of procedures. The Senior Case Manager will provide daily supervision and guidance to the Housing Specialist, Employment Specialist, and Intake Specialist, ensuring seamless coordination under the guidance of the Case Manager Supervisor. Primary Job Responsibilities/Duties: The Senior Case Manager will be responsible for, but not limited to: Ensure strict adherence to organizational policies and procedures, with a focus on effective conflict resolution. Accompany clients to housing viewings, HRA appointments, and facilitate check pickups from DHS or HRA. Collaborate with the Housing Specialist to prepare and submit housing packets and applications. Participate actively in shaping departmental goals, objectives, policies, and procedures, offering insightful recommendations for improvements. Assist the Case Manager Supervisor in guiding a team of case managers in crafting independent living plans, discharge plans, and post-care programs. Provide direct supervision, monitoring, and evaluation of team members, ensuring proficient execution of daily, weekly, and periodic tasks. Deliver crisis intervention and emergency services as the need arises. Oversee case managers to ensure the provision or arrangement of all necessary and appropriate services, including housing, employment, and therapeutic placements, verified through thorough chart audits. Conduct monthly chart audits and diligently follow up to ensure compliance with OTDA, DHS, and NAICA regulations. Maintain accurate and organized supervisory case notes, referrals, and associated documentation. Tackle facility-related challenges diplomatically and facilitate conflict resolution. Generate timely and comprehensive progress reports on a weekly, monthly, quarterly, and annual basis. Collaborate with external agencies and individuals concerning resident records, rights, and responsibilities. Plan and execute community outreach initiatives and related activities. Act as a liaison and representative for engagement with community organizations, as needed. Review and assess initial intake and assessments for clients. Manage a caseload of clients based on the census. Review and approve work and dorm passes for clients. Contribute actively to case conferencing and client review processes. Provide ongoing case management and counseling, extending up to one year beyond client departure. Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Ability to sit for extended periods and perform repetitive tasks. Must be able to lift and carry up to 5 pounds. Must be able to travel to multiple NYC sites as needed. Must be able to access and navigate each department at the organization's facilities. Ability to climb stairs. Work Environment / Schedule Requirements: Office setting with regular exposure to computer screens and moderate noise levels. Qualifications: Bachelor's degree in social work, psychology, sociology, or related field with a minimum of 2 years of direct social service experience. Associate degree in human services or related field with a minimum of 4 years of direct social service experience. High School Diploma/General Equivalent Diploma with 8 or more years of direct social service experience. Excellent understanding of the challenges faced by homeless and special needs populations.
