MINIMUM QUALIFICATIONS:
Must have a current State of Illinois RN License. 1-2 years of related experience working with individuals with chronic mental and physical health conditions. 3-5 years of experience preferred working with vulnerable populations within Social Services and Healthcare industries. It is mandatory that the candidate completes all annual trainings. Must have a valid driver's license. Must have the ability to walk, bend, stoop, sit, stand, reach, and lift 50 pounds.
Must have excellent written and verbal communication skills, sufficient decision making, problem-solving, computer, and organizational skills. Individual must be able to work autonomously. Must have a valid driver's license and pass a criminal background check. Must show a willingness to work in the community and member's homes as needed.
GENERAL FUNCTIONS:
The MCM will provide pre- and post-transition services.
The MCM will adhere to all nursing timeframes in the Williams/Colbert Consent Decree.
The MCM will complete a physical assessment at the member's home within 7 days of discharge.
The MCM will be responsible for following up with the member, in person, after any crisis or unscheduled ER visit
The MCM will conduct weekly phone screenings to assigned members and any member who has questions about a diagnosis or test result or specialty visit.
The MCM will be responsible for conducting on-going assessments of CM's level
of functioning and update plans including (1) functional status, (2) risk of harm, (3) medication status, (4) healthcare utilization, (5) psychosocial status, and (6) community engagement as needed.
The MCM will conduct medication status evaluation and report the Healthcare
Coordinator the CM's ability to self-manage their medication and keep a tracking sheet.
The MCM will be responsible for assisting the care managers with development and implementation of the medical portion of comprehensive assessments and service plans. MCM support is needed in reviewing medical documentation for initial assessments and plans.
The MCM will conduct medication status evaluations and report and document the class member's ability to self-manage their medication.
The MCM will work with Managed Care Organizations to secure appropriate referrals, linkages, and appointments.
The MCM will meet productivity targets and complete documentation in a timely manner, including clinical documentation and service reporting.
In conjunction with the client's treatment team, the MCM will keep the client's family, guardian, and other Envision Unlimited staff informed of the client's progress, including emergency room visits, hospitalizations, and medication changes/adjustments.
The MCM will provide and document crisis interventions as needed, within 24 hours of an incident and complete the reportable event when necessary.
Other duties as assigned (i.e., RI follow-up, CM visit post transition, unscheduled ER visit, and ER to hospital transfers.
OVERALL JOB PERFORMANCE:
At all times maintain confidentiality, according to the Illinois Mental Health code and Confidentiality Act.
Responsible for participating in required in-service training and community training seminars at the discretion of the Director. Pursue and keep abreast of current trends, methods, and techniques in case management and counseling (minimum of 15 hours/year).
Be prepared to present information from outside trainings to clinical staff.
Attend/complete agency trainings as required.
Maintain a professional image and role as a representative of Envision Unlimited.
Inform supervisor of any condition that impacts the safety and wellbeing of clients served. Seek assistance as necessary to mitigate risk and seek out supervision as needed.
Attend individual supervision with the supervisor at a scheduled time weekly to review and report on progress, productivity, and concerns.
Demonstrate flexibility and adaptability with changes to program needs and schedule as the need arises.

About the Role
As a Belmont Village Caregiver, you will provide hands-on personal assistance to a group of seniors living within our large Assisted Living and Memory Care community. General Functions:
Part-time Resident reviewers will complete comprehensive assessments on clients residing in short- and long-term care facilities to determine their potential for community living. Clients may present with severe and persistent mental illness, substance abuse problems or medically complex needs. Assessments will include chart review, interviews with facility staff, family and other collateral contacts. Multiple virtual client interviews are also part of the process. Reviewers will make recommendations for needed services for clients transitioning to the community and work with case management staff to develop a treatment plan prior to discharge. Documentation required within an electronic health care record system.
Part-Time Resident Reviewers will be compensated $250 for each completed review. This is a contract position.
Key Responsibilities:
Engage clients in the assessment process
Secure appropriate consents for participation
Conduct record reviews for all assigned clients
Conduct virtual interview of all clients, family, facility staff
Work collaboratively with agency and facility staff to implement transition plans
Services, meetings, and interviews to be conducted virtually whenever possible.
Minimum Qualifications:
Position requires a master's degree in social work, psychology or related field, and 5+ years' experience in the social service field with a concentration in mental health. Knowledge of medically complex conditions and substance abuse needs a plus. Clinical licensure (LCSW, LCPC or LMFT) preferred.

Requirements
Must be at least 18 years of age
High School diploma or equivalent
Must be able to communicate clearly in verbal and written English
Experience as a primary caregiver or relevant training as a care provider
Ability to work the defined schedule for this position which may include weekends and holidays
Preferred Skills and Characteristics
You have at least one year of direct care experience in an Assisted Living, Memory Care or Skilled Nursing community

Envision Unlimited is seeking an In-Home Respite worker to provide habilitation services and relief to the primary caretaker of an individual with intellectual/ developmental disability. This is a great part-time job opportunity to earn extra income. Essential responsibilities include providing supervision and guidance to the client, ensuring their safety and well-being, and adhering to the member's daily routines and needs.
The hours for this position are limited to a maximum of 300 hours per fiscal year. Requirements include completing the Envision Respite Care Training Program, having a valid driver's license or valid state ID, and passing a criminal background check. Essential Responsibilities, include but are not limited to:
Provide supervision and guidance to client while primary caretaker is out of the home.
Provide a safe environment while primary caretaker is out of the home. Provide services to ensure the health, safety, and well-being of the client.
Follow the established routines, behavior management strategies, communication techniques, and other instructions provided by the family during their training sessions.
Maintain appropriate and up to date training as required by Envision and the Respite Care Program.
