    Under the supervision of a licensed practitioner, the Medical Assistant will perform non-invasive routine technical supportive services of clinic patient care relating to therapeutic and diagnostic functions, following established policies & procedures, and techniques. The supervising licensed practitioner must be physically present in the treatment facility for the Medical Assistant to perform non-invasive technical supportive services. The Medical Assistant will perform a variety of behavioral health screening tools to assist the LIP in identifying mental health and substance use disorders. The Medical Assistant also provides a variety of administrative, assistive & supportive clinical and outreach activities. The Medical Assistant cannot evaluate or synthesize data. Reports to: Direct & Administrative: Associate Director of Healthcare Operations or designee Technical/Clinical: Licensed Independent Practitioner in consultation with the Director of Nursing Services or designee Qualifications: 1. High school diploma or General Educational Development (GED) Certificate is preferred. 2. Completion of a secondary or postsecondary Medical Assistant education program from a public school authorized by the Department of Education, or an approved Medical Assistant program at a postsecondary institution accredited or approved by the Bureau for Private Postsecondary Education. 3. Some previous experience working in psychiatric, mental health or rehabilitation facility is highly desirable. 4. Knowledge of special population patient needs especially with persons experiencing homelessness preferred. 5. Must demonstrate within ninety days of employment, competency in the usage of clinical equipment and Point of Care testing e.g. pregnancy test, urine dip, etc. Have knowledge of instrument sterilization procedures and process. 6. Must be able to travel to all NEVHC Health Centers as directed by the supervisor. Medical Assistant II - Behavioral Health/Psychiatric (Floater) must meet the following additional requirements: 1. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy. 2. Displays qualities of dependability and reliability, be willing to accept a variety of tasks, and able to comprehend quickly and follow instructions accurately. 3. Be able to communicate tactfully and empathetically, with cultural sensitivity; with staff, patients, and family members. 4. Write and print legibly. 5. Be well groomed and display mature behavior. 6. Must possess excellent interpersonal skills. 7. Must be willing and able to work a varied and flexible schedule to accommodate the needs of NEVHC and its patients. 8. Must maintain or exceed Medical Assistant competency levels as established by corporate standards for clinical practice. 9. Must demonstrate through written and oral testing, the knowledge and skills necessary to provide care appropriate to the age of the patients served and has knowledge of the principles of growth and development over the life span. He or she can identify and categorize each patient's age-specific group needs, such as those for infant, adolescent, adult or geriatric patients. 10. Fluency in English (speaks, read and write). 11. Fluency in Spanish (speaks, translate, read and write) and must complete the mandatory Medical Interpretation course offered at NEVHC. 12. Ability to maintain absolute confidentiality about health care and another patient/client information. 13. Ability to utilize electronic medical records effectively to intake and document patient information and schedule appointments. 14. Current California driver's license, appropriate insurance coverage and a driving record acceptable to the NEVHC's insurance carrier (if required to drive on the job). Reporting to the Assistant Program Administrator, the Case Manager is responsible for assessing residents' needs, developing personalized care plans, and coordinating various resources to enhance their well-being. This position works closely with a multidisciplinary team to ensure effective communication, monitor progress, and advocate for optimal outcomes. Our program's mission is to delivery comprehensive care that improves health outcomes for those who need it most. Program Population: Program Population: Crocker Residential is a licensed Residential Care Facility for the Elderly (RCFE). We provide care and supervision for seniors aged 60 and older who require non-medical care and supervision in a home-like environment. We provide 24/7 assistance with activities of daily living (ADLs), including grooming, dressing, and mobility support-along with meal service, medication reminders, and general supervision. Our residents are adults living with chronic health conditions who cannot safely live independently and can benefit from personalized care plans that promote dignity, independence, and social engagement. We maintain a small facility of 48 residents to foster close staff-resident relationships and a strong sense of community. In keeping with our commitment to resident quality of life, we will accept a limited number of residents who wish to keep pets. Applicants should be aware that, on occasion, they may be working around friendly pets and should have no severe allergies to or significant fear of dogs. Principle Responsibilities: Collaborate with Providers, Managed Care Plans, and community service providers to ensure appropriate care coordination for eligible residents. Perform comprehensive case management services, including intake, assessment, personalized care plan development, plan implementation, and follow-up. Connect residents with essential social and support services, arranging referrals for specialized care (such as ophthalmology, behavioral health, podiatry, and nutrition), transportation, and more. This position includes Client Orientation to Comprehensive Peri-natal Services Program (CPSP) initial assessment, trimester reassessments, postpartum assessment, interventions, and follow-up services in: Obstetrics, Nutrition, Health Education, Psychosocial and Individual Case Coordination. * Schedule * Flexible, may require some evening and weekends * Primary Duties and Responsibilities * Orient the patient to comprehensive perinatal care at entry of care and throughout the pregnancy as needed * Complete assessments in the four components - obstetric, nutrition, health education and psychosocial using approved assessment forms * Complete the Individualized Care Plan (ICP) with the patient following the initial assessments. Prioritize problems and actions planned to resolve them * Provide individual Perinatal Education, counseling, referrals and instructions as appropriate to the needs and risks identified on the ICP * Reassess in each trimester to identify changes or new developments since the previous assessment and to provide continued support for the patient's strengths. Revise and update the ICP as needed * Postpartum Assessment and Care Plan * Assess the mother and infant postpartum and update the ICP. Provide appropriate interventions for the patient and her baby * Track patient's attendance at appointments, and identify the reason for any missed appointments * Responsible for issuance of Presumptive Eligibility if patient is qualified for the Program. * Maintains OB/EDC log, Newborn log and sends monthly report to Managed Care, QA Director and Clinic Supervisor. * Ensure that appropriate copies of the prenatal record are at the hospital during the Intrapartum period (34 weeks gestational age) * Assist the patient to obtain Medi-Cal and explain what documents she must have to apply for Medi-Cal. * Inform Supervisor of any issues or problems * Time Studies * Maintain a positive working relationship with staff * Other duties as designated by the Other duties as designated by the Clinic Manager, Clinic Director, leading Medical Provider, or Clinical Service Director Qualifications Minimum Requirements * Microsoft Office (Word, Excel, Outlook) * Excellent Communication Skills * Excellent Customer Service * Bilingual English/Spanish preferred * Ability to work as a team player and work independently * Reliable transportation * Must be able to travel from facility to facility * This position requires 10 % travel outside the local area. Demonstrate understanding of trauma-informed care. Demonstrate non-violent restraint methods, if needed and only as an absolute last resort. Monitor the floors by walking corridors and perimeter of the building. Maintain a front desk sign in/out sheet for clients, as well as visitors. Search clients and/or visitors belongings when they enter the building. Assist with clients' room searches and/or temperature control, as needed. Each CTS during shift is expected to use our electronic tagging system to check all perimeters of the building including the top floors. Program Population: The Pathway Home Housing Project is a HFH funded program, operated by JWCH. The Team consist of a cohesive team of MSW, case managers, site monitors and other professionals who collaborate to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. The clients/residents may come with behavioral and/or substance abuse issues. The program utilizes the Harm Reduction Model and we “do whatever it takes” to encourage success of the client. Principal Responsibilities: Performs job functions as prescribed by established policies and procedures within defined scope of education, training, and responsibilities. Establish a positive rapport with clients and express a concern for their well-being. Demonstrate understanding and practice harm reduction methods Demonstrate understanding of trauma-informed care Demonstrate non-violent restraint methods, if needed and only as an absolute last resort Monitor the floors by walking corridors as assigned, and or being rotated. Maintain a front desk sign in/out sheet for clients, as well as visitors. Monitor the front lobby on a 24 hour/7-day basis. Search clients and/or visitor's belongings when they enter the building Assist with clients' room searches and/or temperature control, as needed Assist clients with elevator access. Act as floor lead during fire drills and real emergencies, and as lead in performing necessary evacuations. Attend each shift change and contribute accordingly Document any client and/or client-staff incidents and submit to supervisor in a timely manner Answer front desk phone, as needed Escort clients to appointments, as needed. Use appropriate de-escalation techniques; call LCSW or Case Manager for assistance, as needed. Ability to follow all crisis protocol Demonstrate understanding and awareness of substance abuse and/or negative behavioral health symptoms Utilize agency van to transport staff to and from parking structure during dark hours and/or rain Provide supportive communication regarding incarceration, substance abuse, mental health, and other risk behaviors. Must be able to work extended shifts when emergency situations warrant in a 24 hour 7 day a week setting. On occasion will need to work an additional 4 hours after regular shift or 4 hours before the regular shift if situation warrants. Flexibility to adjust with a multiple schedule change, shift change, and days off. The work schedule is subject to change periodically based on the number of Care Team Specialists. Maintains the security of confidential documents. Work in various environments including adverse outdoor conditions such as cold, rain or heat. Must be able to work in all areas of the building. Work with administration to process the following: Performs other clerical duties Administrative tasks Assist clients in walkers and wheelchairs if needed. Coordinate with HFH warehouse for all deliveries, and U.S. Mail Maintains confidentiality at all times Discusses any client information only with appropriate personnel when related to the care being provided, and ensures these conversations are not near any clients Maintains confidentiality for all documents that contain patient/client identifier information, Observes and respects confidentiality of information in regards to fellow employees. Responsibility and accountability for adherence to organizational and department standards and policy. Observes department guidelines concerning absences and reports absences in accordance with departmental procedures, Self-identifies potentially unsafe systems/processes/situations and takes initiative to report to the supervisor, Observes department guidelines to conduct personal business during breaks and lunch hour, Projects a positive professional image and adheres to organizational dress code. Maintain shift reports and communicate with BH Team when client issues arise Participates in assigned staff and other meetings, and in-service training meetings Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors. Participate in required safety and compliance trainings and apply learned practices in daily work activities. Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace. Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards. Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role. Performs other duties and tasks as may be required or assigned. Physical Demands: Stand or walk constantly (for up to an entire shift) on various surfaces (tile, concrete, carpet). Climb stairs, ramps, or ladders occasionally during shift. Must be able to kneel, squat, bend for any duration of time and run if needed. Occasionally bend/twist at waist/knees/neck to perform various duties, including lift or carry up to 80 lbs. Constant use of hands, arms, and fingers in reaching/handling/grasping while using radio, phone, notepad, writing reports and other administrative tasks. Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors. Participate in required safety and compliance trainings and apply learned practices in daily work activities. Requirements: Minimum high school diploma or equivalency Two years' experience in behavioral health or social services environment. Previous experience working with homeless persons or population. Strong ability to communicate with homeless individuals who are actively participating in substance abuse and other factors related to homelessness. Desire to work with homeless population. Bilingual (English/Spanish) preferred Ability to assist with the collection of patient's valuables. Experience handling confidential documents and detail-oriented. Ability to use email, database program(s) of agency and working knowledge of word processing (Microsoft Word) and spreadsheet programs. Ability to communicate clearly verbally and in writing Effective written and oral communication and interpersonal skills with ability to deal with all levels of personnel, general public and patients in a professional and effective manner; must be able to use initiative and independent judgment within established guidelines.
