Borinquen Medical Centers is based in Miami, FL and is seeking to hire a full-time Outreach Specialist. Responsible for informing the community and creating awareness about Borinquen Medical Centers (BMC) services, including: substance use, mental health, HIV prevention, and prevention education on the risks of substance misuse, HIV/AIDS, and STDs. The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services. TASKS AND RESPONSIBILITIES Assist Project Director and Behavioral Health Lead Navigator in SAMHSA annual reports, continuation applications, etc. for Behavioral Health Department. Assist Project Director and Behavioral Health Lead Navigator with integration of behavioral health services and communication between disciplines (psychiatrists, physicians, APRNs, LCSWs, LMHCs, etc.). Attend weekly staff meetings, Behavioral Health Department meetings, and BMC general staff meetings. Communicate effectively with supervisor regarding caseloads, issues regarding supervision, and leave requests. Conduct street and community outreach during working hours, including some nights and weekends as needed. Assist Project Director and Behavioral Health Lead Navigator in the creation, scheduling, and presentation of education sessions/groups for HIV, STD awareness, mental health, Life Skills, substance use, and other BMC services. Distribute preventive materials including safe sex kits, condoms, and flyers. Educate target population, newly diagnosed HIV persons, and others about BMC's available services, treatment options, and assist with linkages to services. Engage and educate individuals aged 13 and up in risk-reduction discussions. Ensure all certifications required for your position remain active and that updated copies are submitted for your employee file. Ensure data collection is completed in a timely, accurate and consistent manner, including HIV and STD testing, follow-up questionnaires, and others. Make home visits and reach out to individuals that are high risk, hard to reach, and/or present an adherence problem to treatment. Navigate coordination and expedite services and linkages to additional wrap around care. Perform behavioral screenings and assessments including GPRA, questionnaire data, demographic data collection instruments, pre-post training assessments, and surveys of partners and staff. Assist Project Director and Behavioral Health Lead Navigator in performing periodic review of performance measures to assess progress toward goals, inform program development, and enable fine-tuning of implementation strategies. Present at internal departmental meetings and educate BMC staff about programs and services Provide HIV, hepatitis, and STD testing and counseling. Provide program with community referrals, providing name and location of available van and outreach sites. Recruit participants for all Behavioral Health programs, HIV/hepatitis/STD testing, and prevention services. Work with the Marketing Department to create campaigns and projects for social media. Perform all other duties as assigned by Director of Behavioral Health and/or Behavioral Health Program Managers. REQUIREMENTS Minimum High School Diploma, or equivalent. Outreach experience. Works within a multidisciplinary team to provide an array of direct (face-to-face) and indirect clinical services to adults in a person-centered, trauma informed, individualized focused approach utilizing evidenced-based practices delivered to fidelity. Specific Program Descriptions ABH Complex Care: Complex Care focuses on tailoring services to individuals that would benefit from a multidisciplinary, team-based approach. Our teams (AOT, ACT, FACT, and POWER) strive to build community connections and tackle stigma associated with severe mental illness. As a result, our teams work closely with consumers in a community-based approach, overcoming the barriers of multiple hospitalizations and treatment non-adherence. Complex Care team members are provided with the tools/skills necessary to advocate, assist, and motivate consumers to take part in their recovery. Complex Care Licensed Clinicians provide trauma informed therapeutic services utilizing evidenced based practices to empower individuals. Together, we work to integrate individuals back in the community, instill hope in recovery, and provide the additional support for mental health needs. MINIMUM ENTRANCE QUALIFICATIONS Education and Experience * Master's Degree in Social Work, Psychology, Counseling, or a related behavioral health field from an accredited university * Complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer. Licenses or Certifications * Must have and maintain current license in good standing in the State of Texas as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), and/or Licensed Psychologist (LPHD) * Must be able to be credentialed and enrolled with CHCS payors (determined by license type and subject to Provider regulations) Other Requirements Must maintain a valid driver's license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company's auto insurance carrier if you drive your vehicle during company business. NCI training provides staff with the skills to safely recognize and respond to everyday crisis situations that may involve more challenging behaviors. It focuses on prevention and offers proven strategies for safely defusing anxious, hostile or violent behavior at the earliest possible stage. Justice Intake & Assessment Annex (JIAA) Programs: Licensed Clinicians working within this program must be able to lift at least 50 lbs to maneuver boxes of completed assessments. Depending on assigned unit/program, applicants for this position must pass a Criminal Justice Information Systems (CJIS) fingerprint-based background check and maintain CJIS eligibility. Due to CJIS requirements related to system access, the following may result in being disqualified for this position: Felony Convictions, Felony Deferred Adjudication, Class A & B Misdemeanor Deferred Adjudication, Class B Misdemeanor Convictions, an Open Arrest for Any Criminal Offense (Felony or Misdemeanor), and Family Violence Convictions. The Occupational Therapist is responsible for interpreting all referrals information evaluating, adjusting, updating and discharging the treatment of all patient in accordance with the Ohio Revised Code section 4755. Conduct biopsychosocial assessments, weekly sessions, and discharges. Facilitate 5-6 daily sessions using TF-CBT, EMDR, or a psychoeducational curriculum. Travel to a satellite office once per week to conduct sessions in Charlotte County or Hendry County. Participate in weekly individual/group supervision. Complete quarterly and annual training. Provide knowledgeable of community resources available for residents and families. Makes presentation to residents on topics related to eating disorders. Assumes case management responsibilities for assigned residents as needed. Perform crisis intervention, including management of difficult or resistant residents, utilizing clinically appropriate accepted techniques and methods. Present resident cases for discussion with the multidisciplinary team and keeps current case presentation outlines for each resident. Develops, implements, and coordinates educational lectures and therapy programs, including conducting morning focus, community meeting and psych educational lectures. Responsible for resident, staff, and self-safety at all times. Takes responsibility for maintaining and monitoring resident records to ensure documentation is completed within program guidelines. Documentation meets standards, policies, and established timeframes. Maintains a good working relationship within the department and with other departments.
