Clean wheelchairs, beds, nursing equipment, resident personal hygiene items, and other items as directed. Requires the ability to lift at least 25 pounds and remove assistive devices to clean properly
Safely utilize equipment; report any equipment needing service or repair immediately
Participate in emergency fire or disaster drills and other in-service training as scheduled. Must be able to safely ascend and descend stairwells and assist in an evacuation
Follow Universal Safety Precautions in order to protect oneself, residents and other staff members. Rinse soiled linen in preparation to be sent to the laundry this involves the capability to withstand strong foul odors and the ability to tolerate wearing necessary personal protective equipment. Willingness and ability to wear, tolerate and use protective clothing and equipment needed in the carrying out of duties, i.e. latex gloves

Responsibilities
Performs ongoing clinical assessment, including OASIS assessments at appropriate time points to identify patient/client needs, determine the level of functioning and appropriateness of the patient/client for home care.
Develops and evaluates the plan of care in partnership with the patient/client, representative (if any), and caregiver(s).
Prepares and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner.
Analyzes patient/client needs and provides appropriate care that is ordered by physician as indicated in the plan of care. Revises the POC based on evaluation and ongoing assessment data.
Provides therapeutic treatments for speech, language, cognitive, and swallowing disorders with appropriate tests and measurements. Provides services that are ordered by the physician as indicated in the plan of care.
Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriately) related to the current plan of care.

QUALIFICATIONS:
Must have a current CNA certification

ESSENTIAL FUNCTIONS:
To ensure resident safety
Visually inspects van for any defects and/or vehicle damage; checks operating systems: Brakes, horn, windshield wipers, lights, first-aid kits, emergency flashers, and door-opening devices for proper operation before starting trips; validates registration and inspection stickers for the vehicle are state compliant; certifies working condition of cellular phone
Provides whatever assistance is required for boarding of the patient and any caregiver; ensures wheelchair and passenger are properly secured before starting the van
Upon arrival at the medical facility, assists patients in leaving the van and reaching proper offices or treatment areas; inspects van to ensure that no personal items have been left behind
Provides first-aid or CPR if required during patient transport; immediately notifies emergency personnel and supervisor if skilled assistance is required; completes report at end of day's run
Reports traffic accidents immediately to a supervisor, requesting police or ambulance service as needed; assists injured persons from the first-aid kit until these services arrive; completes company accident report
When not transporting residents, the driver will report to the supervisor for assignments
Promotes positive public relations with patients, residents, family members, and guests
Uses tactful, appropriate communications in sensitive and emotional situations
Use appropriate work place behavior and adhere to dress code at all times
Other duties as assigned
