Job Description
The Care Advocate Nurse oversees initiatives surrounding assessing the severity of the injured workers' reported injury(ies), reviews medical data in CareMC, validates and secures medical information, assesses and evaluates prescribed treatment of work related injury. The Care Advocate Nurse functions as a nurse consultant, supporting the goals of the Claim Management department and of CorVel.
This is a remote position.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
Initiates and receives telephonic contact with reported injured worker, employer and medical providers to obtain treatment plan, secures medical documentation, and assesses if injured worker has returned to work
Responsible for detailed documentation within the claim system focusing on medical condition, treatment plan and return to work status
Directs callers to appropriate medical and/or claim resources
Identifies and communicates urgent situations related to treatment or patient condition directly to the claim team
Functions as nurse consultant to the claim team
Assesses appropriate medical follow up and makes necessary recommendations to the claim team, working closely with team to assist in identifying potential red-flags from the injury that may require further follow-up or additional services
Adheres to the Care Advocate Nurse production standards and Account special handling guidelines
Additional projects and duties as assigned
KNOWLEDGE & SKILLS:
Fundamental concepts, practices and procedures of early intervention in the field of Workers' Compensation (WC)
Ability to write and speak clearly, easily communicating complex ideas
Ability to skillfully manage multiple, complex projects and competing priorities concurrently while working under pressure to meet production standards deadlines and maintaining strong customer service orientation

CorVel uses a market based approach to pay and our salary ranges may vary depending on your location. Pay rates are established taking into account the following factors: federal, state, and local minimum wage requirements, the geographic location differential, job-related skills, experience, qualifications, internal employee equity, and market conditions. Our ranges may be modified at any time.
For leveled roles (I, II, III, Senior, Lead, etc.) new hires may be slotted into a different level, either up or down, based on assessment during interview process taking into consideration experience, qualifications, and overall fit for the role. The level may impact the salary range and these adjustments would be clarified during the offer process.

Esrun Health is seeking Nurses to work part-time from their home office while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. You will be contracted to work a minimum of 20hrs/wk. Esrun Health wants its team members to have the flexibility to balance their work-life with their home life. Part-time team members will typically need to dedicate an average of 20-30 hours per week to care for their assigned patients. This unique business model allows you to choose what days and what hours of the day you dedicate to care for your patients.
The Care Coordinator will be assigned a patient panel based on skill and efficiency level and is expected to carry a patient panel of a minimum of 100 patients per calendar month. Care Coordinators will be expected to complete encounters on 90 percent of the patients they are assigned.

productivity-based pay structure

Payment tier increases require 2 months consistency to achieve. The role of the Care Coordinator is to abide by the plan of care and orders of the practice.
Ability to provide prevention and intervention for multiple disease conditions through motivational coaching.
Develops a positive interaction with patients on behalf of our practices.
Improve revenue by creating billable CCM episodes, increasing visits for management of chronic conditions.
Develops detailed care plans for both the doctors and patients. The care plans exist for prevention and intervention purposes.
Understand health care goals associated with chronic disease management provided by the practice.

What we are looking for:

A minimum of two (2) years of clinical experience in a Med/Surg, Case Management, and/or home health care.

About the role

REPORTS TO: Clinic Director
The CNP provides clinical, consultive and administrative support to improve patient outcomes. Work performed in this position is complex and requires a significant amount of independence, flexibility, attention to details and the ability to handle multiple tasks simultaneously.
Clinical services are guided by a Standard Care Arrangement (SCA) between the CNP and a designated collaborating physician. The SCA shall be reviewed and signed by the CNP and collaborating physician every two years (see attachment).
Clinical services are provided in conjunction with other health team members including mammography technicians, volunteers, interpreters, and other support staff. Clinical services focus on cancer risk assessments, cancer screening exams, health teaching, care coordination and referrals to other health care providers as indicated. Additional services may include diagnosis and treatment of selected minor acute and chronic stable health conditions. Services may be provided offsite in a variety of public and private settings including worksite and other community settings.
Consultant services may include providing medical guidance for managing selected problems for other LifeCare Alliance programs. This may include pre-employment physicals, immunization protocols and reviewing camp physical exams.
Administrative services include program planning to improve or expand clinical services, developing or updating medical forms and assisting in establishing Columbus Cancer Clinic goals and objectives.
PRINCIPAL RESPONSIBILITIES:
Performing physical exams and cancer screening tests following established cancer screening guidelines
Diagnosing and treating selected common health problems within the CNP scope of practice
Consulting with the collaborating physician for management of problems beyond the CNP scope of practice
Patient referrals and care coordination
Documenting patient encounters and phone calls in the clinic's electronic medical record
Assessing and evaluating patient outcomes
Other duties as assigned. This includes serving on agency committees, maintaining local and national affiliations and memberships, helping cover agency phone during staff shortages, attending clinic staff meetings and mandatory LifeCare meetings, assisting with conducting clinic tours and assisting with clinic events as requested.

Working within the Primary Care Team model of care, the RPN Team Lead facilitates the provision of safe, quality, client-centered health care services to the clients. Key Responsibilities:
Supervises the work of the team according to planning guidelines and geographical boundaries.
Works with team members and Management to ensure adequate staffing requirements are met.
Holds regular team meetings and huddles to keep team members appraised of Key Performance Indicators (KPI's) and information necessary for excellent service delivery and team effectiveness.
Utilizes appropriate communication skills to obtain information for the client and/or family and appropriate members of the service delivery team.
Provides complete, relevant assessments of the client/family and their environment utilizing established procedures.
In collaboration with the service delivery team, contributes to the delivery of the client's care plan.
Documents problems identified using a specified framework.
Selects and implements strategies or interventions from established care plans in consultation with client and/or caregiver and health care delivery team.
Describes, evaluates and documents client's responses to care utilizing established procedures.
Utilizes best practice guidelines at all times.
Utilizes adult learning principles and established training materials to teach and enhance skill levels of service providers including Client Care Coordinator as appropriate.
Provides hands on personal care as required.
Completes home visits with service providers for quality service and competency assessments and to ensure performance at optimal levels to meet client service requirements.
Receives and documents all employee, family and client complaints/issues related to service delivery and takes appropriate steps to ensure satisfactory resolution.
Meets with individual team members to build education and learning plans and provides input to annual PDP.
Acts as clinical resource to the service delivery team.
Demonstrates effective time management, planning and organization of day's work activities and of team members.
In collaboration with the Client Care Coordinator, assigns visits to team as appropriate, with attention to urgent care requests including sick calls, missed visits and extra visits.
Advocates on behalf of clients to ensure quality delivery of care is provided by utilizing established guidelines.
Promotes client-directed care, self-care and independence through teaching.
Common Responsibilities:
Promotes the goals and values of VON and their role as an integrated community care provider.
Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System, including all Policies, Safe Work Practices and Procedures.
Abides by all VON policies and work practices.
Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place within their role.
Works in collaboration with other staff in a team approach to service delivery.
External and Internal Relationships:
Works collaboratively with Directing Registered Nurses, Health Care Technicians (PSWs), Management and patients.
Education, Designations and Experience:
Registered Practical Nursing (RPN) diploma.
Minimum 5 years' experience in community based health care system.
Registered with College of Nurses in province of practice.
Team Lead or supervisory experienced preferred.
Proficient nursing skills with the ability to read and interpret technical and clinical instructions.
Clinical practice experience preferred.
Current CPR Certification.
Health teaching and facilitation experience preferred.
Skill Requirements:
Strong observation and assessment skills.
Advanced computer skills required, including Microsoft Office Suite and Windows OS.
Strong verbal and written communication skills.
Interpersonal relationship building.
Ability to problem solve and adapt nursing/unregulated service provider interventions as appropriate.
