    FAMILY SUPPORT WORKER JOB IDENTIFICATION INFORMATION Department: Family Support The primary responsibility of this position is to serve struggling families in the community through the Family Support Program. The Family Support Worker will assist families by engaging families in customized case management services, linking families to access tangible resources through the Compassion Center and community resources, and facilitating evidence-based interventions to strengthen families, and increase the safety, permanency and well‐being of children and their families. ESSENTIAL DUTIES AND FUNCTIONS Maintains a caseload families as assigned by the Family Support Program Supervisor (but not to exceed 15), as approved by the Senior Director of Family Support. Maintain weekly in-person contacts with assigned families by means of office visits and community visits (home, work, school, etc., as necessary). Work collaboratively with parents/caregivers to identify families' strengths and needs using assessments to develop and monitor individualized Family Support Service Plans. Assist families in accomplishing goals in Family Support Service Plans using relevant, clear, measurable, achievable, and time-bound action steps. Engage families in services and facilitate evidence-based interventions, to include but not limited to the Family Check-Up/Everyday Parenting and Parent Café. Assist families by linking them to community resources in order to address identified barriers. Assist families with accessing tangible resources from the Compassion Center. Assist families with scheduling and coordinating appointments with the Family Support Program. Complete data entry, maintain contract compliance, and required documentation and reporting in a timely, accurate, and efficient manner. Meet with Family Support Program Supervisor and the Family Support Services Team on a regular basis for supervision to review cases, training, and professional development and program development. Participate in continuing education opportunities including agency in-service training, professional seminars, workshops, conferences, etc. Other assignments as needed or requested by the Family Support Supervisor, Senior Director of Family Support, and Executive Director of Family Support. SUPERVISORY RESPONSIBILITIES • N/A REQUIRED EXPERIENCE AND QUALIFICATIONS A high school diploma or GED with a minimum 2-years of experience working with families in the welfare system or high-risk population families OR a bachelor's degree in the human services, social work, psychology or a related field. Valid driver's license and reliable transportation is required. Successful completion of required background screenings. PREFERRED EXPERIENCE AND QUALIFICATIONS Bachelor's degree in the human services, social work, psychology or a related field. Experience working with families in the welfare system or high-risk families Bilingual: Fluent in Spanish or Creole with English COMPENTENCIES Present oneself, both at and outside of work, in a manner keeping with One More Child's Core Values of Humble, Hungry, Wise, and Called. Ability to observe and make accurate assessments of situations. Ability to manage time effectively to ensure consistent and active engagement of families in the Family Support Program. Ability to work with frequent interruptions and in stressful situations; ability to respond in crisis situations. Demonstrates knowledge of available community resources within Polk County for families and expand knowledge through networking and partnerships. Key Expectations/Responsibilities: Provide ongoing counseling and skills training to assigned individuals and families in the office, via telehealth, and in client's residence based on client and program need. Provide strengths based clinical care. Complete CANS assessments as needed. Participate in the creation of collaborative therapeutic treatment plan goals and participate in service planning with assigned clients. Document and assess progress of each assigned client and maintain up-to-date client records that accurately reflect that progress, including timely documentation of client incidents, behavior issues and concerns in accordance with agency and licensing standards. Documentation is completed within two business days. Implement group programming to address the social and emotional needs of clients relating to the achievement of individual goals. Participate in the search and selection of residents for programs as assigned. Provide crisis intervention and stabilization to clients who are in psychological distress. Provide emergency crisis intervention as needed using the appropriate community resources and supports. Develop safety plans as necessary. Assist in the ongoing evaluation of evidence-based trauma informed treatment model and interventions. Meet treatment planning documentation, review, and timeliness expectations. Requirements: Qualifications: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Counselor (LMFT) required One year of experience in a foster care, residential treatment, or behavioral health setting required Experience in working with clients who have experienced trauma Willingness to become credentialed with insurance providers as needed and maintain active credentialing Working Conditions/Physical Requirements: May lift and carry up to 10 pounds frequently, up to 25 pounds occasionally, up to 40 pounds infrequently. The role manages the area foster care team and program, supervising and coordinating efforts to make it the most effective. The supervisor provides an additional layer of support for the local foster parents and homes and licensing requirements, spending time doing day travel in the field as necessary. ESSENTIAL DUTIES AND FUNCTIONS Achieve, maintain and aim to exceed local Foster Care Program capacity goals.
