Job Summary
This position is responsible for the overall leadership and management of the location, including meeting established goals and outcomes, ensuring regulatory and organization compliance, directing and coordinating work, financial and operational stability, and demonstrating leadership. Sets expectations for the collection and use of data and information to monitor and control infections. Responsible to create and maintain a culture of safety. Ensures a safety program is in place with scheduled inspections making corrections as necessary. Provides equipment and supplies to ensure a safe environment. Assures resident rights are protected and that all residents, families and significant others are made aware of those rights. Ensures HIPAA compliance. Responsible for ensuring a Quality Assurance Performance Improvement (QAPI) Program is in place. Assigns responsibility to an individual(s) for the daily management of QAPI. Ensures the leadership of monthly QAPI committee meetings. Sponsors performance improvement projects and reviews, approves or rejects performance improvement team findings and recommendations. Provides access to information needed to support quality assurance performance improvement and provides equipment and supplies to support QAPI efforts. The Administrator is the Compliance Liaison of the facility, with a responsibility to support the Chief Compliance Officer in carrying out the Compliance Program.
Manages Communication and Documentation: Expresses ideas clearly, concisely and effectively both orally and in writing. Requests ongoing feedback on task performance. Communicates upward and downward within the organization appropriately and in a timely fashion. Ensures documentation and reports are completed as required by regulations and/or policy and procedure. Seeks clarification of assignments and asks questions when needed. Keeps supervisor informed and follows chain of command as appropriate when reporting issues or concerns.
Manages Financial and operational objectives: Works with department teams, and consultants to oversee operations of the campus.
Qualifications
A Bachelor's degree is strongly preferred, preferably in healthcare administration, including course work in business and human resources.
Location will determine experience required.
The ability to be a licensed as a Nursing Home Administrator in the state of practice is required.
Other licenses as appropriate by state requirements and facility needs.

The School Youth Peer Support Specialist will aid mental health providers and youth to move toward self-advocacy in the attainment of their own, culturally specific life goals.
The Crisis Responder is responsible for providing 24-hour crisis coverage as part of the on-call rotation. This is a short-term service which aims to assist individuals who are in a crisis (e.g., suicidal, homicidal, psychotic). The Crisis Responder will screen, triage, and refer out to the appropriate level of care and services. The crisis program is designed to prevent hospitalizations and emergency protective custody by wrapping appropriate services around the individual to allow them the opportunity to remain in the community, therefore it is the Crisis Responders role to facilitate this.
DUTIES/RESPONSIBILITIES:
Collaborate closely with school based therapists to coordinate and enroll students in mental health services.
Meet with parents to complete documentation needed to establish mental health services for students.
Facilitate small groups each week in the school setting.
Interact, build rapport, and connect with youth by sharing common experiences and modeling resiliency and recovery.
Support youth involved with DHHS, the justice system, education system, youth, and family services.
Assist youth in understanding components of recovery and resiliency and in applying skills to achieve life goals.
Provide individualized direct support to peers according to that persons plan by supporting the individuals choice and building confidence, leading to a greater degree of independence.
Work cooperatively and effectively with individuals and groups from diverse populations.
Attend youth case meetings (when asked), other meetings as appropriate/assigned.
Locate available resources, training, or skill-building opportunities that will help youth to achieve his/her goals.

Maintains up to date client charts to include needs assessment, screening tools, treatment plan, progress notes, discharge planning, and crisis encounters. Provide crisis support and intervention services in order to prevent a crisis, during a crisis and after a crisis has stabilized. This includes screening, triage, and safety planning. If hospitalization becomes necessary, facilitates in cooperation with all necessary parties (e.g., mental health professionals, medical facilities, domestic violence shelters, housing, substance abuse treatment, food banks, law enforcement, schools, families, inpatient providers) and assists with the clients transition back into the community.
Will be a part of the on-call rotation to maintain coverage of the crisis line 24/7/365. This will include evenings and weekends.

+ Provides safe and direct patient care: ambulating, bathing, positioning, entering diet information into the computer system, passing meal trays, feeding and attending to other care needs. Responds promptly to patient call lights and special needs, ensures that patient valuables and personal items are properly handled and accounted for.
+ Greets all persons coming to the area assigned, answer inquiries, may direct and assist them, take and relay messages, assist with gifts, flowers, and mail for patients, run errands for department staff.

+ Requisition of supplies and equipment, maintenance and repairs services and prepares related forms.
+ Requests services such as the following as directed: Chaplain, social services, and pastoral care.
+ Performs clerical duties related to procedures such as admission, discharge, preparing appropriate charts and forms, transfer, and death.
+ Manages interdepartmental and patient mail.
+ Maintains patient rooms in an orderly condition, changes bed linens and provides routine cleaning tasks.

Job Summary
The Medication Assistant (MA) serves as caregiver who provides resident-centered nursing care and daily living assistance to assigned residents under the supervision of a charge nurse. Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per plan of care. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psycho-social support and other personal care to assigned resident. May assist the resident in transferring, re-positioning, and walking using correct and appropriate transfer techniques and equipment and provides range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. May assist residents with meals and snacks; provide water and supplements as appropriate.
Administers prescribed medications as delegated by a licensed nurse and within their scope of practice as defined by state regulations. Documents the outcomes related to resident care, medication administration, behavior, activity, and dietary. Communicates changing condition, medication follow-up and care related concerns/responses to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May participate in departmental/organization committees and task forces as needed.
Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum of 18 years of age.
Prior clinical or nursing assistant experience in long-term care preferred.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually based on state requirements. Maintains all department specific required certifications. When applicable, may need a valid driver's license and auto insurance.
Iowa: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained.
Minnesota: Must have nursing assistant certification, have completed the 75-hour course and be registered with the Minnesota Nursing Assistant Registry. Successful completion of a state-approved Trained Medication Assistant course.
North Dakota: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained. If working in an Assisted Living or Basic Care facility, Medication Assistant I certification required. If working in a skilled nursing facility, Medication Assistant II certification is required.
South Dakota: Must successfully complete the state approved Certified Medication Aide (CMA) course. CMA certification must be maintained with the State Board of Nursing.
For all Good Samaritan and Sanford nursing facilities: Must have the state required certified/trained medication aide certification (CMA or TMA). Active certified nursing assistant registration in state of practice, or ability to obtain within 120 days, is required.
