Responsibilities
Summary:
Team Rehabilitation is seeking out patient-focused PhysicalTherapists. Primary responsibilities are evaluating, diagnosing, and treating patients with the ultimate goal of having patients recover from injuries, manage pain, and regain mobility. The only way to drive success is in providing exceptional physical therapy with measurable outcomes. The ideal candidate will be passionate about patient care while regularly exceeding industry standards.
Responsibilities:
Provide excellent health care for our patients. Patients should achieve good outcomes and be happy with their experience of physical therapy.
Evaluate new patients - understand their current medical and emotional situations and then develop plans of care.
Treat patients and set measurable, functional goals. Making sure to answer their concerns and questions about their injury, progress and probable outcomes.
Educate the patient on treatment options and home exercises that can assist in complimenting their clinical sessions.
Supervising aides when they are providing modalities for non-Medicare, non-BCBSM patients, and also in their general set up and clean up duties.
Monitor and re-evaluate patient progress; revising plans of care and resetting goals when needed to ensure the best possible outcome.
Address pain management as a crucial component of care. Utilize techniques that can assist in pain reduction to increase their functions and everyday activities.
Determine when to discharge patients. Teach patients proper mechanics, ergonomics, and strategies to reduce future incidents.
Write timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
Achieve a reasonable level of productivity. This depends on the number of referrals and the type of patient presenting. However, with our normal outpatient orthopedic population, Team would expect 55 to 60 treatments plus evaluations a week from a full-time PhysicalTherapist.
Participate in research studies supported by the clinic and help develop protocols for the most common diagnoses.
Give input on policies, procedures and strategy to ensure that all Team Rehabilitation clinics provide the highest quality of care.
Handle patient feedback and complaints promptly and professionally.
Responsible for continuing to develop skills by staying updated in the latest research and techniques. Attend company in-house CEUs and use the continuing education allowance to attend seminars, workshops, and professional development opportunities.

Valid Physical Therapy License

The Family Therapist is responsible for providing mental health treatment and case management services. Services are delivered in the home or community based on the needs and preferences identified by the family. Must be willing/able to work a non-traditional schedule based on the needs of the families served. Must be able to deliver Treatment Foster Care Oregon (TFCO) clinical services and implement interventions that within the fidelity of the model. Must be able to deliver behavioral therapy that targets school behavior and family behavior. In addition to providing family-centered services, the TFCO Family Therapist is responsible to help families to develop collaborative relationships with other human service organizations and people or agencies in the community who can help to enhance and maintain a viable support network for the family after services have ended. The TFCO Family Therapist is responsible to help coordinate and integrate services that are provided to families by multiple service providers and to identify and eliminate barriers to effective service delivery.
Must maintain flexible work schedules and cell phone as needed. Will need to be available to TFCO clients 7 days a week, 24 hours per day as needed for on-call rotation. Must possess a “whatever it takes” mentality in delivering services to families.
This position comes with a $3,000 sign-on bonus.
Key Responsibilities include:
Provide weekly family therapy to families utilizing the TFCO treatment model. This includes introducing new behavioral coping and parenting skills and practices these in sessions utilizing strategic positive reinforcement and coaching interventions
Engage primary caregiver and other key participants in active change-oriented treatment;
Engage participants to identify and overcome barriers to engagement;
Develop therapeutic relationships with families;
Provide direct clinical treatment using methods compatible with TFCO model;
Provide family therapy;
Coordinate services with mental health and medical professionals, including psychiatrists, physicians, and nurses;
Facilitate collateral contacts with representatives from schools and other community organizations on behalf of children and families;
Advocate on behalf of families to help them meet their needs and achieve their desired level of independence and self-sufficiency;
Link families with resources available in the community;
Assist families in scheduling psychiatric evaluations and medication reviews with Psychiatrist once child returns home
Help to identify and eliminate barriers to services;
Help to enhance existing community services and develop new services based on the needs of families;
Develop and maintain collaborative relationships with people and organizations in the community;
Make referrals to Child Protective Services and Adult Protective Services as needed;
Participate in clinical supervision, team meetings, case consultations and training as needed;
Provide individual, family and group therapy; crisis intervention; case management;
Other duties as required or assigned.
Requirements
Must have a Master's Degree or higher and be a Psychologist (TLLP, LLP, or LP) TLLP supervised by a Licensed Psychologist, Licensed Professional Counselor (LLPC or LPC) LLPC supervised by a Licensed Professional Counselor, Licensed Master's Social Worker (LMSW) or Limited Licensed Master's Social Worker (LLMSW) supervised by a Licensed Master's Social Worker, and must possess and retain current licensure in the State of Michigan.
Must meet Michigan Department of Community Health requirements of a Child Mental Health Professional within 120 days of hire.
Must have at a minimum of 1 year of experience in the examination, evaluation and treatment of minors and their families.
Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain therapeutic relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative.
Must have a favorable driving record and be able to obtain a chauffeur's license within 30 days of hire.
Preferred experience includes working with families, working within a team environment, administering social skills assessments and interventions, direct use of pragmatic (i.e. structural, strategic, and functional) family therapies, cognitive behavioral therapy with children and adults, Wayne County CMH services, and electronic medical records.

What You'll Do

As a Full Time Clinical Therapist at Clinton Counseling Center, you will provide quality therapeutic treatment for substance use and mental health clients in a small non-profit outpatient clinic located in Mount Clemens, MI.
Job Responsibilities:
Provide quality outpatient treatment for substance use and mental health clients.
Complete Intake assessments, treatment planning, and provide group and individual therapy to assigned clients.
Discharge planning and thorough clinical documentation.
Advocating for clients needing higher or different levels of care.
Minimum Requirements:
Candidates must possess an LLMSW/LMSW or LLPC/LPC.
Must possess a CAADC or be willing to submit a MCBAP development plan.
Completion of a clinical degree at an accredited university.
Valid, state-issued clinical license.
Clear criminal record.
Refined interpersonal and diagnostic techniques.
Excellent verbal and written communication skills.
