The counselor will provide and coordinate the pre-vocational activities that help patients develop and achieve vocation rehabilitation goals and function successfully in the competitive workplace. Responsibilities: Perform intake of all patients to obtain vocation and educational background information to identify preliminary needs and develop an initial plan of services to meet those needs. Conduct a comprehensive vocational/educational assessment including patient history, test administration and the evaluation of the patients' capacity to function Prepare a vocational rehabilitation plan with short and long range goals and methodology to achieve goals Maintain a list of patients with updated status regarding involvement in work-related activities, work readiness, and employment status at 30-60-90-180 day intervals. The SAC serves as a member of the interdisciplinary treatment team. Job Scope: Initial and ongoing screening of resident and resident property in accordance with unit procedure. Facilitate group counseling formats on time and in a professional, ethical and caring manner and documents client's progress through the use of group notes. Supervise clients in self-medication (under supervision). Communicate all pertinent information, incidents and emergencies using the appropriate system including the SAC communication log, incident reports and 24 hour on-call supervisor. Provide crisis interventions and follow up including documentation of the incident, accident and/or elopement reports. The Vocational Counselor serves a member of the program's treatment team. Job Scope: 1. Completes vocational/educational comprehensive assessments and quarterly progress reports. 2. Develops and facilitates the implementation of vocational/educational treatment plans for all clients that are consistent with agency standards and comply with applicable administrative rules. 3. Provides individual vocational and educational counseling to adolescents to further assist them in reaching vocational and educational goals. 4. Conducts vocational/educational group counseling. The schedule for this position is 5p-10p What You Will Do: · Conduct assessments, intakes and participate in level of care determinations by gathering appropriate data, orienting client to his/her rights and responsibilities and program expectations. · Complete necessary administrative procedures for admission to treatment and as appropriate, assist in development of treatment plan. · Facilitate admission/referral to other programs or agencies for clients in need of services not offered at Unit. Provide individual, group, and/or family counseling services in a professional, ethical and competent manner. · Manage all aspects of assigned caseload during treatment, with special attention to appropriate levels of care and appropriate clinical interventions. · Provide backup coverage in the event of other clinical staff absences. Complete all needed documentation and reports in a timely manner and inform Program leadership (Director, Asst. Ability to work effectively in collaboration with diverse groups of people Position Status This is a full-time non-exempt position. Saturday-8am-4pm Sunday-Wednesday 3pm-11pm Thursday and Friday - Off Work Environment This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
