What would you do as a Firefighter and Paramedic
As a Full-Time Firefighter-Paramedic with Miami Township, you will play a vital role in providing fire suppression, hazardous materials response, rescue operations, and emergency medical services. Position Summary
The Community Paramedic will deliver in-home clinical care under established protocols, focusing on prevention, follow-up, and gap-closure in patient care. This role operates outside the typical EMS transport model. You will work with autonomy, using Total Care Connect's electronic patient care record system, following physician-led protocols.
Key Responsibilities
Perform patient assessments in the home environment: vital signs, health history, home safety, risk factors, etc.
Provide in-home follow-up care post-hospitalization or after urgent care visits to prevent complications and readmissions.
Deliver patient education on disease management, medication adherence, lifestyle improvements, and social determinants of health (e.g. nutrition, housing, etc.).
Work under clinical protocols to treat/manage health issues, escalate when necessary, consult virtually with advanced providers (NPs, RNs, physicians).
Coordinate care: schedule or facilitate follow-ups, referrals, community resources, and linkages with other health or social services.
Document all care, assessments, and patient encounters thoroughly, using the organization's mobile electronic patient care record (ePCR) system.
Participate in internal meetings, trainings, and continuous quality improvement activities.
Maintain all required licenses, certifications, and compliance with relevant regulations.
Qualifications
Valid Paramedic certification/license in the applicable state, in good standing.
Minimum of 2-5 years paramedic / prehospital experience; experience in community paramedicine or similar mobile health / MIH models is strongly preferred.
BLS certification required; ACLS and PALS preferred where applicable.
Excellent clinical assessment, problem-solving, and decision-making abilities.
Strong verbal and written communication skills.

ESSENTIAL FUNCTIONS: A Firefighter/EMT-P shall be able to operate all fire apparatus and ambulances and equipment carried on them to provide fire protection, and the care, treatment, and transportation for the sick, injured or incapacitated citizens of the city and surrounding areas. Performs fire prevention inspections, conducts public fire and EMS safety education programs, performs preventive maintenance inspections on all equipment, performs the testing of all fire apparatus, hose, hydrants and other equipment within the department needing routine testing. Complete written and/or oral reports for the dissemination of information based on the above activities. Maintain certifications as a Kentucky Certified Career Firefighter and State Certification as a Paramedic. Participates in drills, demonstrations and courses in firefighting techniques, equipment, and apparatus operation, medical aid, technical rescue, hazardous material, fire prevention, equipment maintenance and a wide variety of other subjects related to the fire and EMS service; studies city geographic, streets and conditions affecting fire operations; studies and learns departmental standard operating procedures; studies and learns inspection regulations and fire prevention laws, codes and departmental policies and procedures; studies and learns procedures and techniques for the mitigation of hazardous materials incidents. A Firefighter/EMT-P shall participate in the department's fire education and community outreach programs and activities. Assist in the inspection of residential and commercial business occupancies; assist in performing follow-up procedures to ensure compliance to Fire Codes, National Electrical Code, and Kentucky Building Codes. Cleans, services and makes minor repairs to ensure the operational readiness of vehicles, apparatus, hoses, and emergency equipment; participates in cleaning, repair and upkeep of departmental buildings, grounds, and facilities. SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES: This position carries a minimum of 48 hours per week; 24/48 shift. This position must respond to emergencies and perform Firefighter, Emergency Medical Technician Paramedic duties as trained or directed by the Incident Commander. Possess a valid driver's license. Obtain a good patient care report from the nursing staff. IE: chief complaint, treatment received, treatment needed enroute, acuity etc.
Gather all patient care paperwork and patient belongings if any.
Safely move patient to the stretcher and secure with all straps.
Complete a thorough patient assessment, obtain 2 sets of vitals at minimum. Patient vitals must be reassessed every 15-30 minutes depending on patients' current status, length of transport etc.
Must be able to give a good patient hand off report.
Complete an accurate EPCR, obtain all appropriate signatures and attach all appropriate paperwork to the EPCR. EPCRs must be completed by the end of each shift.
All patient care must be delivered appropriately and within your scope of practice or per Medical Control Direction.

Assess unwell children to determine if the child needs to leave daily programming.
Audit on-site First Aid kits and coordinate maintenance of supplies.
Coordinate with emergency services, as needed, in a crisis by providing first aid or care and communicating with EMS staff as necessary.
Interact with children in a professional, positive manner.
Adheres to the Positive Leaps Model of care.
Adheres to all Positive Leaps Policies and Procedures.
Communicate effectively with parents, peers, supervisors and visitors, and maintain confidentiality of children and families in treatment.
Creates an atmosphere of enthusiasm and commitment.
Performs other duties as assigned.
Minimum Requirements:
Must possess active State or National EMT Certification.
Must be at least 19 years oldĀ· Excellent documentation skills.
Must be willing to extend hours should a crisis arrive.
Must be flexible and adaptable to changes in schedule.
Must be reliable with documentation and adhering to expectations.
Be willing to collaborate with various team members internally and externally.
Must be able to work in a fast paced, stressful environment.
Exposures and Risks:
Potential for exposure to blood and body fluids.
Works directly with physically aggressive children.
Exposed to loud noise, such as repeated and prolonged screaming and crying.
