Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may impact recovery or the plan of care. Identify and escalate any deviations that could compromise patient outcomes. Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team. Maintain real-time communication with the remote command center nurse and physician to report findings, collaborate on care decisions, receive treatment orders, and ensure all aspects of the care plan are executed safely and effectively. Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions based on hospital orders and standing order protocols. Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and when to seek help. Use both verbal coaching and written materials to reinforce understanding. Documentation & Compliance: Document all assessments, interventions, communications, and outcomes in the electronic health record (EHR) system in real time.
