Amber House Crisis Stabilization Unit (CSU/CRT) Clinician
Amber House CSU is a 10-bed voluntary-only CSU whose purpose will be to assess individuals who are having a mentalhealth crisis and are in need of assessment, stabilization, and brief treatment. The service is available to individuals for up to 24-hours. Amber House CRT has up to 14-beds for individuals in crisis who do not meet medical necessity criteria for hospitalization and would benefit from treatment and supportive programming.
The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual's wellness and recovery, intake assessment, leading group couseling, milieu management.

SOCIAL WORKER DUTIES AND RESPONSIBILITIES:
Responsible for the development and delivery of a full complement of services, including individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals within the Clinical Care Coordinator's caseload as assigned.
Responsible for utilizing Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact.
Responsible for continuously assessing an individual's risk status including risk-based symptoms, substance use, environment, complex physical conditions, etc, and intervening appropriately.
Responsible for assessing the continued service necessity and creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
Responsible for continuous outreach and progressive engagement and reporting disengagement to the team immediately. Responsible for working with the outreach team and determining when to disenroll individuals based on program protocols.
Responsible for domain-based assessment, screening referrals, service planning, community planning, and benefits coordination which supports wellness and recovery principles. Responsible for care planning and therapeutic case management using a whatever-it-takes approach.
Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies. Responsible for producing the required number of regulatory units of service per month (65% of staff's time must be provided face-to-face or other eligible community-based support).
Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager.
Other duties as assigned.
SOCIAL WORKER COMPETENCIES:
Direct service experience working older adult, forensic/re-entry, TAY or adult through previous employment, internships, and/or lived experience.

SOCIAL WORKER QUALIFICATIONS:
Master's Social Work (MSW), Master's Marriage Family Therapy (MFT) or Master's Practitioner in Clinical Counseling (MPCC) required
Associate ASW/ Licensed LCSW or Associate MFT/ Licsened LMFT with current BBS registration;
2 years of social service experience required
Bilingual in a threshold language preferred.
Valid California driver's license, personal vehicle, and proof of personal vehicle insurance coverage are required. DMV printout required.
Local travel may be required, vehicle reimbursement is provided. Care Coordinators may be asked to transport clients in their personal vehicle.

GENERAL PURPOSE
Under the administrative supervision of the Clinical Director or Program Director, this position is responsible for insuring that the clinical and case management needs of clients with co-occurring psychiatric and substance abuse issues are met and that they achieve their goals through best practices such as motivational interviewing, harm reduction, and any other evidence-based practices available. Clinical Specialist will also be responsible for providing some case management responsibilities to a modified case load when fully staffed.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our members in meeting their expressed goals while living in the community. Additional support in areas of medication management, housing, counseling and advocacy will be provided as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES - (ILLUSTRATIVE ONLY)

Assists the Clinical Director in all areas of clinical program development as needed including KET tracking.
Provide prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
Problem solves communication difficulties between staff and clients by providing interventions to enhance communication.
Meets regularly with clients at Turning Point sites or in the field, at job sites, homes, board and care facilities, skilled nursing facilities, locked facilities, and other appropriate sites.
Develops financial management plans for clients who receive Representative Payee Services and authorize appropriate expenditures.

Scope of Position: The Licensed MentalHealth Professional will play a pivotal role on one of the Governor's newly launched innovative Safe Options Support (SOS) teams, which will provide comprehensive care to street homeless or subway-dwelling individuals. The role will require field-based work, periodic on-call coverage, and a willingness to work flexible hours.
Essential Position Functions:
Conduct persistent and assertive outreach and engagement using strength-based approaches, beginning at known “hang-outs” or “hot spots” within the transit system or during inpatient hospital admissions or emergency department visits.
Partner and collaborate with current street outreach teams, local police precincts, local hospitals, the MTA, the Department of Homeless Services, and family members/caregivers to identify individuals most in need of outreach and care.
Continuously assess the health and social needs of participants through SOS's conversational and observational assessments, as well as formalized risk assessment tools for those identified as high-risk.
Collaborate with the centralized SOR Hub to identify available housing and support participants throughout the process, including completing HRA 2010e forms, applying for housing, prepping for interviews, following up with housing providers, and assisting with moving in (day of move), obtaining housing supplies, and learning the neighborhood.
Participate in hospital discharge planning meetings to identify the best community resources for returning participants.
Provide short-term therapeutic counseling and support to participants both pre- and post-housing.
Collect and report data as required, and collaborate with the team leader, data analyst, and other SOS teams to use data to inform future care delivery.
Once participants are housed, work with them and their housing providers to resolve clinical issues impacting the participant's ability to manage and retain supportive housing.
Foster relationships with community providers to ensure that participants are connected to appropriate services as they transition back into the community.

Job Description
Community of Veterans (CoV) is the first and largest online social network exclusively for paperwork-confirmed Iraq and Afghanistan veterans. CoV provides its more than 24,000 members with access to message boards, affinity groups, resources, and live chats with experts on a range of issues - especially those related to mentalhealth. CoV provides veterans with a safe space to share what they're going through with others who “get it” because they've been there too. An informal peer-to-peer support system and means of connecting with resources and experts, CoV is an invaluable gathering space for veterans grappling with PTSD, Traumatic Brain Injuries, depression, and even thoughts of suicide.
The Online Community Intern will be responsible for supporting the IAVA Programs department to promote engagement within CoV through outreach and communications with members, posting content and resources, and scheduling live chats and other events. The Online Community Intern will report to the Senior Program Manager.
The Online Community Intern will:
Interact with Iraq and Afghanistan veterans online and off by conducting outreach to promote CoV sign-ups at IAVA events and interacting with veteran members in CoV
Learn about methods for digital outreach and engagement, through drafting social media announcements and blog posts, and through planning and executing online events, such as webinars and live chats
Learn about mentalhealth issues affecting this generation of veterans and gain exposure to key resources
Enhance skills in data analysis by monitoring and reporting on trends among CoV member needs and interests
Assist with creating and executing a crisis prevention and response system within CoV
Qualifications
The Online Community Intern should possess:
Excellent organization skills
Strong communication (written and oral) and interpersonal skills
The ability to work well with others and independently, with a sense of humor and a professional demeanor
A positive attitude and a passion for veterans issues and community building
Special Qualification Requirement
To maintain the integrity of the CoV site, the Program Intern, CoV must be a veteran of Iraq or Afghanistan. Evidence of service, such as DD214 forms, will need to be provided before an offer is finalized.

Job Description
The Licensed In-Home MentalHealth Counselor will provide counseling and mentalhealth services to youth and families. Mentalhealth assessment and counseling may be provided in the home or in the community for a youth and their family.
Major Responsibilities/Activities:
Provide mentalhealth assessment, diagnosis, treatment planning and psychotherapy services
Provide individual, family, and group counseling to a diverse population with many challenges
Provide individual and family interventions with the goal of developing and implementing social, interpersonal, self-care and independent living skills to restore and maintain stability, to support functional gains and to adapt to, and or maintain community living.
Guide families during and following a crisis episode experienced by a child/family with the goal of stabilizing the child/youth in the home and natural environment.
Maintain the crisis phone number, on call, for a week at a time (rotated amongst staff)
Collaborate with other treatment providers and collaterals, as appropriate
Complete and submit required documentation in a timely manner (e.g. progress notes, treatment plans, et cetera). Maintain positive working relationships with other professionals, community agencies and team members
Demonstrates sensitivity to cultural and ethnic norms
Follows agency's Worker Safety protocols
Participates in team meetings
Engage and guide the youth and family through active listening skills and therapeutic engagement methods
Maintain positive working relationships with program/agency staff, collaborating agencies, and stakeholders
Continue professional growth and training in best practices, as necessary/appropriate (e.g. cultural competencies)
Attend coaching/supervision meetings as required
Participate in the agency Quality Improvement Process and other agency meetings including in-service trainings, supervision and staff meetings as requested
Adheres to NYS and agency requirements including, but not limited to: HIPPA, ethical guidelines, confidentiality, child abuse identification and reporting, incident reporting, worker safety protocols
Possess a working knowledge of the Microsoft based products including agency programs such as Anasazi and Fidelity HER
Minimum Requirements:
NYS Education Department license in Social Work, MentalHealth Counseling, Psychology or Marriage and Family Therapy
At least one (1) year of experience providing direct services for children with one or more of the following primary diagnoses: mental illness, alcoholism, chemical dependency and substance abuse
Demonstrated cultural competence in understanding and working with disadvantaged/minority populations in community-based programs in an urban and suburban setting
A valid NYS driver's license is required
Must have reliable means of transportation to travel throughout Western New York area
Must be comfortable transporting clients in personal vehicle as needed
Computer literacy required, experience with Anasazi preferred
Excellent written and oral communication skills
Competitive Salary of $62,400 per year based on a 35 hour work week.

The Latino Heritage Internship Program seeks to engage young professionals in natural resource careers. Applicants must meet the following requirements:

Position Description:
The incumbent will collaborate with NPS Rangers and park partners to successfully present interpretive and community outreach programs that align with the six Interpretive themes identified in Golden Gate's Long Range Interpretive Plan, the agency's America 250 vision, and the commemoration of the Presidio of San Francisco's 250th anniversary.
The intern will have the opportunity to develop original programs and contribute to a variety of interpretive and community outreach offerings that will highlight overlapping history from California's indigenous cultures, Spanish colonialism, the Mexican Republic, US military expansion and the growth of San Francisco. The emphasis on the cultural history of sites such as the Presidio and Fort Point National Historic Site will be part of division-wide effort to provide meaningful recreational and educational opportunities for local audiences to connect with park resources and reflect on our shared heritage. The intern will be pivotal in achieving one of our goals to provide more impactful interpretive programs to the considerable number of Spanish-speaking and Latino members of the general public that visit fee-free sites like Fort Point.
Incumbent will collaborate with the Division's Media Branch and site leads to establish a social media campaign that advertises programming and highlights park stories that reflect the themes of the America 250, 250th anniversary of the Presidio, and Latino/Hispanic heritage. The intern will also develop digital content in Spanish in order to reach and engage with a wider audience.
Responsibilities:
Collaborate with NPS staff and park partners community engagement groups to support the facilitation of pre-existing programs geared towards target audiences connected to America 250th programming.
Tailoring programs to specific group needs and accommodations.

Lead Jr Ranger Day at Fort Point National Historic Site
Create social media content highlighting history of the Presidio and Fort Point NHS in collaboration with the park's Visual Media Team and Social Media Leads.

Position Summary
Brooklyn Community Services PROS is strengths-oriented psychiatric rehabilitation program devoted to helping adults increase independence and overall well-being. The PROS Counselor aids people living with persistent mental illnesses in developing life role goals, in managing symptoms, and in taking concrete steps toward a better life. The PROS Counselor does so through the provision of evidence-based individual and group services aimed at the development of therapeutic and life skills. Working in collaboration with an interdisciplinary team, they help program participants identify an individualized, meaningful course of care to become as self-sufficient as possible.
Responsibilities
* Manage a caseload of up to twenty-five participants, providing individual counseling and case management as clinically appropriate
* Develop and maintain collateral relationships with outside supports including family, case managers, housing program staff, and medical providers
* Conduct required assessments including the biopsychosocial, psychiatric rehabilitation, vocational readiness, cultural, and substance dependence screenings
* Develop Individualized Recovery Plans (IRPs) in collaboration with participants based on the above listed assessments on a semi-annual basis, or more often as needed
* Develop and facilitate approximately four to twelve skills-based workshops per week
* Document participants progress/regression towards goals in monthly progress notes and determine the need for service modifications on an ongoing basis.
* Provide services according to evidence-based practices including Wellness Self-Management, Family Psychoeducation, Integrated Dual Disorder Treatment, Individual Placement and Support
* Coordinate follow-up services once a participant is ready for discharge from the program.
* Act as a member of an interdisciplinary treatment team, including participation in weekly administrative and clinical meetings
* Possess/develop proficiency in Microsoft Word, Excel, and AWARDS electronic health record
* Manage responsibility for maintenance of records, including timely documentation of services as required by the Center for Medicaid and Medicare Services and the Office of MentalHealth
* Conduct intakes, as needed
Qualifications Required
