**PURPOSE AND SCOPE:**
Supports FMCNA's mission vision core values and customer service philosophy. Adheres to the FMCNA Compliance Program including following all regulatory and FMS policy requirements.
Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies procedures and training and in compliance with regulations set forth by the corporation state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).
**DUTIES / ACTIVITIES:**
CUSTOMER SERVICE:
+ Responsible for driving the FMS culture through values and customer service standards.
+ Accountable for outstanding customer service to all external and internal customers.
+ Develops and maintains effective relationships through effective and timely communication.
+ Takes initiative and action to respond resolve and follow up regarding customer service issues with all customers in a timely manner.
**PRINCIPAL RESPONSIBILITIES AND DUTIES**
PATIENTCARE:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC) Adequacy Monitoring Program (AMP) Urea Kinetic Modeling (UKM) and regarding disease process/access.
+ Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
+ Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
+ Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
+ Perform vascular access evaluation pre-treatment and report unusual findings to nurse supervisor.
+ Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.
+ Perform administration of Heparin as delegated or as allowed by state law.
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy and report any unusual findings to nurse supervisor.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures
+ Evaluate patient prior to termination of venous access - standing & sitting blood pressure.
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
+ Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
+ Report any significant information and/or change in patient condition directly to the nurse supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
+ Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application accurately and in a timely manner.
+ Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect label appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
OTHER:
+ Perform additional duties as assigned.
**ADDENDUM FOR PATIENTCARETECHNICIANS WORKING IN THE HOME PROGRAMS (NOT HOME ASSIST):**
Assisting the HT RN:
+ May assist HT RN on Home Visit as directed
+ Reinforces PD / HHD education under supervision of qualified HT RN
+ Schedules and contacts patients regarding appointments
+ Weigh patient and obtain vital signs
+ Collect treatment records and review for completion. Notify RN of incomplete / missing records.
+ Cleaning and prepping treatment room; prep charts
+ Preparing lab tubes and requisitions & assisting with lab draw days both pre and post draw (PCT certified phlebotomy or per state regulations)
+ Clerical duties as assigned (faxing mailing to physician offices etc.)
+ Obtain home hemo water sampling as directed by the HT RN
+ Set-up of the home hemo machine and PD Cycler.
+ Assist with exit site care when directed by HT RN.
+ Warm dialysate solution when directed by the HT RN
+ Draw dialysate solution for testing as directed by the HT RN
+ Creates and sends patient reminders (supply inventory for order Kt/V collection bringing meds to clinic visit for review)

Job Description
Perform first level of on-site technical, data recording and assessment within the manufacturing environment of Quaker Houghton customers. Follows standard procedures and has readily available supervision or management where necessary to provide a high standard of Fluidcare support.
Job Accountabilities:
Participate in addition of chemicals; including coolants, greases, oils, lubricants, cleaners and biocides as directed by site staff. Perform bio-stick dip tests of all machine sumps and central systems.
Assist in the maintenance of coolant filters, washers, paint systems, pumps, air handling systems, etc. for sump changes and cleaning, as required to keep fluids within proper limits.
Measure and record coolant parameters and maintain lubrication schedules to ensure parameters are met to maintain the highest coolant conditions. This may involve the dropping and cleaning of tanks when required.
Liaise directly with the customer so that changes in coolant parameters are highlighted and can then be acted upon and enter results in appropriate databases. Prepare detailed shift reports.
Assist in routine maintenance on customer's equipment and fluid systems including product additions, filter changes, emptying gons, sump cleaning, etc. as directed.
Assist in disposal of contaminated articles (i.e. pails, gloves, samples, etc.) in accordance with prescribed measures, including waste management and recycling initiatives
Assist in maintaining and calibrating all laboratory equipment; such as scales, pH meters, glassware, incubators, etc. in accordance with site control plan.
Assist in properly maintaining tools and utility equipment such as pumps, sump suckers, coolant dispensers, etc. in a safe and efficient manner.
Where appropriate: Cut-up, mount and prepare parts for metallurgical testing. Surface etching of parts for analysis. Measurement and metallurgical determinations consisting of hardness and microstructure analysis
Supports site staff general organization initiatives (inventory, storage rooms, labeling, etc.)
May work in manufacturing environment with union associates.
Interact with customer's production and technical personnel and other suppliers.
May be required to operate hand tools, shop tools and equipment. May be required to operate industrial equipment, such as: fork trucks, sump suckers, power washers, tuggers, etc.
May require working flexible hours.
Carries out other duties as directed. May have tasks specifically related to quality control, metalogical heat treat and the steel industry. Provide other specifically agreed Fluidcare services as and when required by the Site Manager and/or Lead Technician - this may include operating a fork truck, recycling equipment, etc.
Must be able to safely work in a variety of potentially hazardous situations while adhering to Quaker Houghton's Life Saving Rules.
Work Environment:
Work in a manufacturing environment; may be exposed to fumes or airborne particles and toxic or caustic chemicals. May occasionally be exposed to moving mechanical parts, risk of electrical shock and trip and slip hazards. The noise level in the manufacturing environment is loud.

Shift: 1st Shift 5:00 AM - 12:15 PM (Every 3rd holiday rotation) Position Reports to: CCU Inpatient Manager Department Description: Wilson Health's Critical Care Unit is a 7 bed Intensive Care unit that caters to patients with severe and life-threatening illnesses and injuries, which require constant, close monitoring and support from specialist equipment and medications in order to ensure normal bodily functions. SUMMARY: Prepares and compiles records in nursing unit of hospital facility by performing the following duties. Provides essential patientcare under direction of Registered Nurse. All team members of Wilson Health will promote our purpose to improve the health of our community so people can spend more time doing what they love with those they love. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Performs patient related clerical duties necessary to assist the health care team as they provide care. The patient population would include: the young-middle aged adult patient, and the geriatric patient.
The HUC shall be responsible for the computer entry of patient information/registration and physician orders to ensure continuity of care.
The HUC shall assist with the coordination of activities of the unit and ensures the necessary supplies are available to provide patientcare.
The HUC shall assure open and timely communication with the patient, their significant others and team members to ensure the occurrence of therapeutic interventions.
The HUC will be competent and current, adhering to established standards of practice and will engage in activities or behaviors that characterize professionals.
The HUC shall show respect for life, dignity, human worth and the individuality of each human being.
Under direction of the RN/LPN, assists in meeting the physical and emotional needs of the patient on his/her assigned unit. The patient population would include: the young-middle aged adult patient, and the geriatric patient.
The CCU P.C.T. shall, under the direction of the RN/LPN, carry out assigned treatments and procedures.
The CCU P.C.T. shall be competent and current, adhering to established standards of practice and will engage in activities or behaviors that characterize professionals.
The CCU P.C.T. shall provide care based on ethical concepts, respect for life, dignity, human worth and the individuality of human beings.
Performs additional duties as assigned within the scope of the department.
Qualifications:
HS Diploma or GED
One to three months related experience and/or training; or equivalent combination of training or experience.
Successfully completed an approved Nurse Aide Training or Competency Evaluation Program
Or 3 to 6 months related HOSPITAL experience and/or training
Current BLS Certification
