    **_What Health System Pharmacy contributes to Cardinal Health_** Pharmacy Operations is responsible for the safe, efficient and effective coordination of Cardinal Health's pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices as well as retail customers. Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes. **_Job Summary_** Cardinal Health manages the pharmacy at Lemuel Shattuck Hospital and we are currently recruiting for a Clinical Pharmacist. This is a 32 hour a week position that would consist of 4x8 hour shifts with rotating weekends. Hours of the facility are Monday-Friday, 7:30am - 10:00pm and weekends 8:00am - 4:30pm. This pharmacist will work closely with medical staff to ensure safe, effect, and appropriate drug therapy. Along with supporting daily operational responsibilities that include, but are not limited to order verification, inventory management, and medication dispensing. **_Responsibilities_** + Reviews, processes and monitors patient medication orders to evaluate the appropriateness of use, dosage form, route of administration, regiment compliance, therapeutic duplication, drug interactions, adverse reactions and age appropriateness. + Participate in multi-disciplinary rounds for a variety of patient populations. + Checks technician prepared or prepares for dispensing all medications for patient specific use. + Assists with drug procurement and inventory maintenance. + Assists with precepting experiential education students from Schools of Pharmacy. + Reviews non-formulary requests and prepares formulary reviews as needed for Pharmacy and Therapeutics Committee. + Participates in performance improvement activities using evidence bases practice. Pharmacists support the CMOP mission by preparing prescriptions and packages in an accurate, timely, and cost-effective manner, without disruption in services or production. Position Pays: $59.25/hr Responsibilities Provides pharmaceutical verification of prescription orders of legend, OTC, Medical Surgical (Med/Surg), and Controlled Substances to patients requiring care. Assesses drug orders and cross matches with medication presented at checking stations. Pre-verifies medications loaded into highly specialized mechanical apparatus to dispense medications into prescription bottles. Completes verification and fill of controlled substance orders, including Schedules III, IV, and V. Performs Quality Assurance functions and supervision of Pharmacy Technicians. Follows CMOP policies regarding documentation and reports to meet VA and TJC policy. Notes any unusual doses, wrong drug, wrong strength, or drug mix up, and reports the information in accordance with local CMOP policies. Provides full checking, labeling, and quality assurance within the guidance (written VHA handbooks, written CMOP policies, and training that will be provided by the CMOP) of the Department of Veterans Affairs regarding pharmaceutical medications. Compounds and dispenses medications within corporate and regulatory guidelines for controlled substances. Qualifications Licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Must be a graduate of an American Council on Pharmaceutical Education (ACPE) accredited school or university. Pharmacists who are licensed but have been subjected to disciplinary action of any type by their licensing authority within the preceding five years, must be referred to CMOP management before proceeding with the hiring action. Must be licensed for controlled substances, with qualifications to practice in any states. Must demonstrate effective interpersonal communication skills with co-workers and staff. Maintains basic typing skills, PC computer skills, and working knowledge of MS Office. Must read, understand, speak, and write English fluently under the authority of U.S.C. 7402(d) and 7407(d). Able to pass VetPro certification, which is a credentialing system that is required of all VHA licensed, registered, and certified health care providers. Able to sit/stand for long periods of time (8-10 hours), and lift totes weighing up to 30 pounds repetitively.
