Pay rate: $20-$22 per hour
Schedule: 8am-5pm M-F Normally close around 3 on Friday.
*The salary is determined by an individual's level of experience, as well as any relevant licenses and certifications they may hold.
Key Responsibilities:
Responsibilities:
Experience doing blood draws, labeling specimens, centrifuging specimens, recording maintenance data and decontamination, updating patient information, etc.
Collects and stores specimens in accordance with established procedures.
Properly explain in a clear but courteous manner the process of the venipuncture (blood drawing) and or other specimen collection (ex. urine or fecal) as required.
Demonstrate technique/s using straight needles and/or butterfly needles.
Fully understand all of the physicians' orders. Matches laboratory requisition forms to specimen tubes. Label, centrifuge, split, and freeze specimens as required by test order.
Check all test requisitions or computer labels against the script to ensure 100% correctness.
Package specimens for transport.
Stores specimens at the required temperature and places them.
Qualifications:
A High School Diploma or GED is required.
A minimum of 3-5 yr. of phlebotomy experience is required.

The Phlebotomist supports high-quality, Veteran-centered care by performing specimen collection and processing for a VA Community-Based Outpatient Clinic. Responsibilities include venipuncture and capillary collections, specimen handling/processing, courier preparation, point-of-care testing (as authorized), and accurate documentation in the electronic systems. The role emphasizes patient identification, safety, infection prevention, regulatory compliance, and excellent Veteran experience.
Verify patient identity using two identifiers; obtain informed consent/assent per policy and explain procedures to Veterans in a trauma-informed manner.
Perform venipuncture and capillary collections for adults, including special collections (e.g., timed, fasting, cold-protected) per requisition.
Select appropriate tubes/containers; follow order-of-draw to minimize hemolysis and cross-contamination.
Label specimens at bedside/point of collection with required identifiers; ensure complete, legible requisitions and orders.
Centrifuge, aliquot, and store specimens under proper conditions (temperature, light protection, stability) per test requirements.
Prepare specimens for transport (pack-out) following chain-of-custody, biohazard, and packaging requirements; coordinate courier pick-up and tracking.
Perform and document CLIA-waived POCT (e.g., glucose, urine dipstick, rapid tests) as authorized; run quality controls and maintain logs.
Maintain phlebotomy carts, supplies, and equipment; perform daily/periodic maintenance checks and troubleshoot issues; escalate as needed.

Coordinate with PACT teams, providers, and external laboratories to ensure timely, closed-loop results.
Accurately document collections, orders, and results in the VA EHR and/or Laboratory Information System (LIS) per policy.
Verify and reconcile orders; resolve missing/incorrect data through appropriate communication and documentation channels.
Protect PHI in all workflows and uphold privacy and information security standards.
Completes required organizational compliance education, including assigned requirements that are client-specified, for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.

Qualifications
Required Skills and Knowledge:
National certification (e.g., ASCP PBT, AMT RPT, NHA CPT) and prior VA/DoD, CBOC, OR 2 years reference lab experience within the past three years (preferred).
Proficiency in venipuncture and capillary techniques, specimen handling, and basic computer/EHR skills.
Strong phlebotomy technique with attention to patient comfort, specimen integrity, and order of draw.
Knowledge of CLIA requirements, specimen stability, and biosafety practices; competent with sharps safety and spill response.
Effective communication, customer service, and teamwork within a fast-paced clinic environment.
Organization and time management to prioritize STAT/timed draws and coordinate multiple providers/schedules.
Education Required:

Current BLS (AHA).

Demonstrates proficiency of proper collection methods and proper processing of all specimens.
Maintains required records and documentation.
Maintains daily records and all appropriate PSC/IOP Phlebotomy logs.
Assist with compilation of monthly statistics and data.
Performs basic clerical duties, i.e. filing, faxing, and preparing mail. The person in this position is required to perform electronic data entry.

Processes laboratory samples according to procedure. Centrifuges and prepares aliquots when needed. Maintains specimen requirements at all times according to protocol. Dispatches specimens to the main laboratory or other designated testing facility as per established procedure with fewer than two mislabeled specimens. Document all correct and accurate information in regard to the specimens collected to ensure best is being met daily, delivers collected specimens to the laboratory(window specimen receiving) and may receive specimen using Epic Beaker. May have to enter orders or look up orders in EPIC Beaker. May be required to contact the Core Laboratory for retrieval of specimens (e.g. for add-on test).
Adheres to Patient Safety Goals as defined by UTSW, TJC, CAP, and other regulatory/accreditation agencies. Performs inventory of supplies as assigned. Provides training to other Phlebotomists as needed. May provide team leadership for other Phlebotomists in the absence of the Lead Phlebotomist.
Ensures all tests are accounted for in the ordering system. If necessary, contacts physician's office or nurse to obtain order clarification/verification prior to collecting specimens. Tracks every specimen to ensure specimen integrity, accounting particularly for those samples which must be transported to other locations with fewer than three occasions of errors involving maintaining specimen batches (opening, reconciling and closing).
Orders and Monitors supplies and ensures all are within expiration date. Ensures that expired supplies are discarded and none are in the general supply, storage or on phlebotomy carts. Phlebotomy carts need to be fully stocked with required supplies; delivered to the specific staging area; document daily use on current phlebotomy log. Keeps work area well organized and free from clutter, stocked with essential supplies and decontaminated with proper cleaning solutions and disposal of wastes and trash in appropriate containers.
Utilizes the Risk Management and Process Improvement tools (Event Reports) in an accurate and timely way to identify issues concerning Quality Improvement and assists coworkers in this process as needed. Follows chain of command when reporting unsafe practices observed.
Actively participates in the review of the monthly schedule and work assignments in conjunction with the Supervisor/Coordinator. Know daily tasks and be accountable for the daily workflow, including the status of the phlebotomy tracker, work flow assignments, and priority test requests. Also, acts as a liaison/problem solver for phlebotomy issues.
Perform blood collections, blood cultures, and other special collections (Lactic acid, Ammonia, QFTs, to name a few), according to procedures defined by the LIS Collection Management with minimum errors.
Attendance Reports for work on the assigned days at the assigned times. Follows procedures regarding requesting and using leave in manner that consistently supports effective job performance.
