HEMODIALYSIS REGISTERED NURSE
REPORTS TO: FACILITY ADMINISTRATOR
SUMMARY OF RESPONSIBILITIES:
To assess patient's nursing care needs as it relates to the hemodialysis treatment, to plan and implement appropriate care each treatment, to make appropriate referrals to other health care disciplines or providers as indicated, to evaluate effectiveness of care provided, and to integrate nursing theory into clinical practice.
Patient Care:
Performs patient assessment: vital signs and all other parameters of the ESRD patients, review dialysis orders, determine general physical and mental condition, assist in determining compliance to dietary or medication regimen, document accurately and legibly.
Provides chronic hemodialysis therapy: formulate a dialysis plan for ultrafiltration, initiate dialysis via prescribed access, observe all safety measures, and monitor patient vital signs applicable to ESRD patients. Administer medications, fluid therapy and blood products or other treatments related to ESRD per physician's orders.
Performs analysis of patient data: assess and monitor status of vascular access, review lab work, patient assessments, and report any relevant changes or abnormalities to Charge Nurse, or Clinical Coordinator / Facility Administrator and patient's primary physician.
Develops patient plan of care based on patient condition, age, education and socioeconomic conditions that define interventions to attain expected outcomes.
Participates with the patient care team in the development and review of both the short term and long term care plan programs.
Planning, Coordinating, and Organizing:
Able to prepare and monitor dialysis machines and systems.
Able maintain dialysis records and prepare dialysis treatment system for use according to established procedure.
Informs technical staff of any malfunctions related to the dialysis treatment equipment.
Obtains machine and water cultures as requested.
Maintains patient medical records, documenting appropriately and legibly.
Quality Assurance:
Completes flow sheets verifying charges for the patient.
Monitor supplies and assure availability.
Participates in CQI process.
Monitors compliance with state or federal regulations, federal regulations for patient care and services.
Has the ability to implement emergency measures as situational conditions demand.
Communications and Patient Service Skills:
Reviews pre and post dialysis care with patient.
Discusses health concerns with patient; reviews with Physician and Clinical Coordinator / Facility Administrator.
Participates with orientation and training of new staff member.
Attends staff meetings and educational activities.
Other duties as assigned.
POSITION QUALIFICATIONS:
Current license to practice as a Registered Nurse in this State.
Graduate of an accredited school of nursing.
Current certification in Basic Life Support.
Experience in dialysis preferred.
At least one (1) year of experience in general nursing or a nursing specialty.

Partners with Restore Therapy to identify therapy needs for Medicare A & B recipients.

During facility visits, conducts an exit interview with the facility Administrator, Director of Nursing, and MDS Coordinator presenting findings from the visit and any recommendations for systems changes.
Formulates a visit report for each facility that summarizes issues that were identified during audits and/or visits and systems that were implemented to address the concerns. Reports will be submitted to the facility Administrator, Director of Nursing, Regional Administrator, Director of Medicare and Rehabilitation, Chief Operations Officer (COO), and MDS/Clinical Information Consultant.
Assists with the implementation of new NHS and regulatory policies related to the MDS or Medicare programs, in the region's facilities. Consults with the Director of Medicare and Rehabilitation and MDS/Clinical Information Consultant on the development and revision of policies and procedures, as requested.
Participates in daily PPS, Weekly Medicare/Rehab, and monthly Triple Check meetings during facility visits.
Assists with the implementation of facility medical record automation efforts as related to the RAI or Medicare process, if requested.
Stays current in MDS and Medicare related regulatory changes and revises practices consistent with those changes. Educates facility staff on these changes as directed by Director of Medicare and Rehabilitation and MDS/Clinical Information Consultant.
Reviews charts of residents being considered for discontinuation of part A coverage and discusses with facility staff.
Serves on committees as assigned.
Reviews recommendations of the PDPM group and coordinates with them to identify and implement needed changes and training needs.

Monitor residents' conditions, including vital signs, and promptly report changes to the supervising RN or physician
Assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding when needed
Collaborate with the interdisciplinary care team to support rehabilitation and recovery plans
Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR)
Provide emotional support and education to residents and their families
Promote a safe, clean, and therapeutic environment by following infection control protocols
Qualifications for the Licensed Practical Nurse (LPN):
Current and valid Licensed Practical Nurse (LPN) license
New grads encouraged to apply!
Experience in long-term care, skilled nursing, or rehabilitation preferred
Strong clinical skills, including medication administration and basic wound care
Excellent communication, documentation, and teamwork skills
Compassionate, patient-focused approach to care
BLS/CPR certification required
