Submits health care plan and goals of individual patients of periodic review and evaluation by Physician. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patient to Physician for consultation or to specialized health resources for treatment. Consults with patients with diabetic conditions, evaluating their needs for diet plans, blood sugar testing, etc. Responsible for the scheduling and implementation of the Synergy program.
