* Master of Nursing (MSN) or Doctor of Nursing (DNP) degree for Nurse Practitioner applicants.
* Master's degree for Physician Assistant applicants.
* Valid and unrestricted medical license to practice in the state(s) where care is provided.
* Certificate of Authority to practice.

Job Role Summary
A Primary Care Physician of Eskenazi Medical Group (EMG) and Eskenazi Health Center Inc. (EHC), provides the full scope of services which fall under individual field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance. In addition, a Primary Care Physician is a member of the Eskenazi Health Center team. In that capacity they are expected to be committed to the philosophy, goals and objectives of the Center and work collaboratively to achieve them.
It is the responsibility of the Primary Care Physician to model and uphold the mission, vision and standard of performance within the health center. The Primary Care Physician must be cognizant of, and comply with, all Center policies and procedures, EMG employment requirements and procedures as well as pertinent state and federal regulations.
Essential Functions and Responsibilities
Clinical Practice and Education
1. Obtains health history and perform examinations
2. Diagnoses and directs counseling for each patient on a plan for treatment
3. Prescribes medication in accordance with Indiana statute and professional guidelines
4. Performs minor procedures in accordance with professional credentialing
5. Arranges referral for patients requiring services outside of the scope of Primary Care, including transfer to inpatient care
6. Reviews In-Basket each work day and addresses time sensitive patient information with appropriate urgency.
7. Initiates coverage of assignments and ensures continuity of care prior to anticipated absences
8. Complies with EH medical staff bylaws, all applicable EH, EHC, EMG requirements, policies and procedures, and EH Infection Control plan.
9. Participates in all required EH, EHC, and EMG meetings.
10. Assists in updating protocols and principles of practice as requested
11. Actively supports and participates in all quality of care, performance improvement, and patient safety activities
12. Actively supports the achievement of identified performance and quality metrics including but limited to cycle time, capacity utilization, no show rates, missed opportunities, and patients per hour.
13. Supports patient experience and customer services initiatives, uses AIDET, and assure all patient interactions are professional
14. Assures patients are efficiently seen and dispositioned to assure their needs are met or exceeded
15. Accurately and consistently completes and attests notes from patient visits, signs and closes encounters preferably within 24 hours, but absolutely within 72 hours of the encounter
16. Meets the requirements and deadlines for all provider mandates from EH, EHC, and EMG, including but not necessarily limited to required CME hours, completion of annual PPD, annual flu shot, and any mandatory training assignments
17. When requested by leadership, represents EMG and/or EHC, and/or EH on assigned internal and external committees, task forces, commissions, agencies and promotional or public relations efforts regarding EH's primary care clinical affairs
18. Maintains flexibility (or availability) for the appropriate number of patient appointments in accordance with individual clinical FTEE. Is flexible to changing schedules to ensure access for essential patient needs
19. Works closely with the CPE or Medical Director for practice quality and performance requirements

This position offers the opportunity to work directly with a fellowship-trained breast and oncoplastic surgeon as we expand Hoag's Comprehensive Breast Program, building on our legacy of excellence in multidisciplinary breast cancer care. Our program integrates leading-edge surgical oncology with advanced oncoplastic techniques to provide the best possible outcomes and patient experience.
Position Overview
The Breast Surgical Oncology PA plays a vital role in the delivery of efficient, high-quality, and compassionate care for patients undergoing evaluation and treatment for breast cancer. The PA will collaborate closely with the surgeon in the clinic, operating room, and hospital settings, supporting both surgical oncology and reconstructive procedures. Over time, the role offers graduated autonomy in managing patient care.
Essential Functions
Participate in clinic, inpatient rounds, progress notes, and discharge planning.
First assist in surgical procedures including lumpectomies, mastectomies, sentinel and axillary lymph node dissections, and oncoplastic reconstructions.
Perform bedside and post-op procedures such as drain removals, injections
Conduct comprehensive pre-operative and post-operative evaluations, including histories, physicals, and documentation of care plans.
Write and implement orders for tests, procedures, and therapies per established protocols.
Review and sign informed consent forms and educate patients about surgical expectations, recovery, and follow-up care.
Interpret diagnostic tests and imaging in collaboration with the supervising physician.
Provide ongoing support and education for patients navigating breast cancer treatment and survivorship.

Education:
