Bereavement Coordinator Hospice - 2512483 Description Job Purpose: To plan and coordinator bereavement services for PruittHealth Hospice families and patients • Previous training and/or experience in death and loss issues. • Ability to interact with a variety of different age groups.
• Ability to work well independently and within an interdisciplinary team setting.
• Ability to communicate effectively orally and in writing with individuals and groups.
• Ability to handle confidential matters.
• Ability to work a flexible schedule.
• Possess knowledge of computer software, including proficiency in word-processing.
Other Division or Job-Specific Essential Administrative Functions:
• Plan, organize, implement and monitor the PruittHealth Hospice bereavement care.
• Supervise bereavement care volunteers and develop volunteer inservice trainings.
• Develop a bereavement plan of care for PruittHealth Hospice families.
• Review bereavement assessment.
• Refer families to other community resources as appropriate.
• Organize and facilitate bereavement counseling groups.
• Serve as a bereavement representative at PruittHealth Hospice interdisciplinary team meetings.
• Document interventions with patients and families and update bereavement care plans accordingly.
• Present training sessions on grief and loss to new hospice volunteers as part of the overall volunteer training program.
• Assist in conducting educational workshops for the community.
• Assist in providing bereavement support to staff members of facilities where hospice patients reside.
• Coordinate with the Director of Health Services (DHS) and the interdisciplinary team that the needs of patients and families are being met related to bereavement.
• Other duties as assigned by the Supervisor and/or the Administrator.
Qualifications Licensure/Certification/Education Requirements:
• Bachelor's degree in the area of human relations (psychology, religion, sociology, social work, counseling) required.
• Master's degree in Social Work or Chaplaincy required.
• Current Professional License as applicable
• Current Driver's License and Automobile Liability Insurance
Other Training, Skills, and Experience Requirements:
• Ability to work effectively independently and within an interdisciplinary team
• Orientation to PruittHealth Hospice and any other training that may be necessary to complete the assigned duties effectively.
• Satisfactory results of criminal history check and Office of the Inspector General Sanction Check
• Free from the illegal use of drugs, and free from the use and effect of drugs and alcohol in the workplace.
• Ability to work a flexible schedule
• Excellent Organizational skills.
• Has never been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected to any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application.

Master's Degree from accredited school of Social Work required; Valid license/registration of permit to practice in state(s) of agency operation if required
Medicare/Medicaid/Insurance specialty preferred
Unless otherwise dictated by the state, at least two (2) years of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferred
Experience or education in grief counseling preferred.
Discharge planning experience preferred.
Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Provide supervision of interns and/or social work fellows (SWF), as needed. If providing supervision of SWFs, must meet state regulations as a qualified supervisor.
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

+ Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.
+ Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
+ Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
**About You**
**The candidate must meet one of the following educational requirements:**
+ Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
+ A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.

Role:
The Chaplain provides a wide range of spiritual services to patients and families enrolled in any of the Company programs. The Chaplain also provides spiritual support to the staff and volunteers of the Company.
Qualifications:
If hired after 5/26/2024, Master's degree required.
If hired prior to 5/26/2024, Bachelor's degree in Divinity or related Degree from an accredited institution required.
Ordination or commissioning/endorsement from an ordaining body recognized by the Endorser's Conference, Veterans Administration that demonstrates the candidate has standing with the organization; ordination must be obtained within 12 months of hire
Documentation that demonstrates the candidate is in good standing with faith group or professional affiliation group with no ethical charges current or pending
One unit of Clinical Pastoral Education (CPE) accredited by the Association for Clinical Pastoral Education, Inc.; or in process within six (6) months of employment/position
Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system preferred; appropriate computer skills required
Health care experience preferred
Mobile Driver - Valid driver's license and automobile insurance per Company policy
Ability to travel and have reliable transportation
Must be able to physically access (i.e., navigate stairs and narrow spaces, endure heat and lack of air conditioning) any home, hospital, nursing home and/or assisted living facility (ALF)
Must have the endurance to manage demands of a hospice care field work position (i.e., time, travel, home/facility arrangements, stress, etc.)

Monitors implementation of the spiritual care plan and reassesses as needed.

Job Purpose: To plan and coordinator bereavement services for PruittHealth Hospice families and patients * Previous training and/or experience in death and loss issues. * Ability to interact with a variety of different age groups. * Ability to work well independently and within an interdisciplinary team setting.

Other Division or Job-Specific Essential Administrative Functions:
* Plan, organize, implement and monitor the PruittHealth Hospice bereavement care.

* Develop a bereavement plan of care for PruittHealth Hospice families.
* Review bereavement assessment.
* Refer families to other community resources as appropriate.
* Organize and facilitate bereavement counseling groups.
