    Description: This position provides custodial and personal care services to clients that contract with Assisting Hands for services. Education: High School Diploma/GED required; Current Nursing Assistant Certification with the Board of Nursing in good standing Qualifications: Individual must have successfully completed and provide evidence of formal training and certification as a Nursing Assistant per state law and regulation. One (1) year experience in the home or long-term care setting is preferred. Reports to: Director of Nursing Primary Duties: Maintaining a clean, safe, and healthy environment within client's residence. Duties may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing client's dishes or laundry, and other tasks to maintain cleanliness and safety for the client. The performance of all personal care activities contained in a written Plan of Care detailed by the agency's Director of Nursing which may include assisting the client with personal hygiene, ambulation and exercise, eating, dressing, shaving, physical transfer, and other duties as assigned. Providing nutritional support as needed. Reporting changes in the client's condition. Other activities as taught and overseen periodically by the Agency for a specific client and are restricted to the following: Assisting with the use of devices for aid to daily living such as a wheelchair, walker, or hoyer lift; Assisting with prescribed range of motion exercises; Teaching household routine and skills to well members of the family; Measuring intake and output of fluids; Measuring and recording temperature, pulse, respiration or blood pressure; Keeping records of personal health care activities; Observing appearance and gross behavioral changes in the client and reporting to the Agency. Assisting the client with the self-administration of medication, limited to the following tasks: Taking the medication in its previously dispensed, properly labeled container, from where it is stored and bringing it to the client. In the presence of the client, reading the label, opening the container, removing a prescribed amount of medication from the container, and closing the container. Placing an oral dosage in the client's hand or placing the dosage in another container and helping the client by lifting the container to his or her mouth. Returning the medication container to proper storage. Keeping a record of when a client receives assistance with self-administration of medication. The CNA may also provide the following assistance with self-administered medication, as needed by the client: Preparing necessary items such as juice, water, cups, or spoons to assist the client in the self-administration of medication; Opening and closing the medication container or tearing the foil of prepackaged medications; Assisting the client in the self-administration process. Examples of such assistance include the steadying of the arm, hand, or other parts of the client's body so as to allow the self-administration of medication; Abides by the agency's infection control policies, including proper hand washing techniques consistent with CDC Guidelines. Abides by all policies and procedures outlined in the agency's Policy and Procedure Manual, Caregiver Handbook, and Caregiver Training Manual. Hours: Dependent on assignment; Shifts available 24 hours a day, 7 days per week and based on availability. Assisting Hands does not guarantee hours. Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed. Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol. Participates in department and/or unit performance improvement activities. Conducts rounds with oncoming CNAs to assure continuity of care to residents. Provides direct physical care (bathing, grooming, feeding, toileting, transferring/ambulating) to residents to the extent the resident is incapable of performing those activities. Monitors residents and report all new signs, symptoms, resident complaints, alterations in skin integrity, and changes in functional status to licensed nurse or nursing supervisor. Assists with admissions to Assisted Living by preparing room, welcoming new resident and family, by providing for basic immediate needs, and by gradually introducing resident to schedule and activities for residents. Answers call lights promptly. Lets residents know if and how long a delay to expect; notifies charge nurse or nursing supervisor if he/she cannot complete work in a timely fashion. Transports residents to and from appointments and assists Life Enrichment personnel in transporting KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Knowledge of charting systems( Point Click Care) Familiar with patient care Proficient with Microsoft Office Suite or related software. Solid organizational skills including attention to detail and multi-tasking skills. Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees. Successful completion of a State approved CNA course and CNA competency testing. Basic Life Support (CPR) Experience with the geriatric population preferred. Minimum of one-year CNA experience desired. Strong communication skills both oral and written. Must be able to add and subtract, read a thermometer, and use a balance or digital scale. Salary determined based on qualifications and experience. * Works with staff, as a team, to provide high quality care to our patients by identifying and assessing health concerns, examines the patient based on the health concern, and creating a plan specific to the patient needs. * Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities. * Provides clinical consultation to other staff members and supervision of staff and students as assigned. * Demonstrates adherence to National Patient Safety Goals and plan. * Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development. Scheduled Hours: Based on Clinic Needs * 40 Hours per Week * Monday - Friday with a weekly evening rotation and a monthly Saturday rotation * Master's Degree in Nursing or postmaster certification as an APN or PA required. * Appropriate certification by National certifying body as required by state law (ANCC, NCCC, NAPNAP, AMCB, or ACC) is required. * Current Illinois licensure as a Physician Assistant (PA) or Advanced Practice License (APRN) is required. * Minimum of 2 years of experience in Primary Care is required.
