Our Company ResCare Community Living Works under the supervision of a Registered Nurse. Duties include providing routine care, general observation of client's health status, assists the RN and physician in treatments and communicating with clients and their families/guardians. Reports changes in condition and/or additional client needs to supervisor.
Responsibilities
* Communicates health/nursing needs and/or recommendations to RN supervisor
* Coordinates admissions, discharges, and follow-up as needed
* Assists in medical/health appointments as needed
* Ensures the provision of nursing/health services under the direction of the RN and as ordered by the physician, provides direct care as outlined in the Service Plan/Nursing Care Plan
* Takes and records vital signs, weights, wound measurements, etc. Identifies trends outside of parameters, or in need of attention and communicates to RN/physician
* Reports pertinent information and changes in individual health status to attending physician/health professionals and to the Team as needed in a timely manner
* Ensures the provision of needed adaptive equipment
* Medication Administration:
* Administers medications, verifying the correct medications are administered (per the six "rights" of medication administration), and that client's response to medications is noted and documented along with any noted side effects
* Works closely with home/facility/pharmacy staff in consulting on psychotropic medications, side effects, and medication reviews
* Orders, reviews, monitors, and discontinues medications as ordered by physician. Dispose of unused medication per policy
* Assures that monthly Medication Administration Records (MAR's) are accurately transcribed in each individual's record
* Checks the medication cabinet on a weekly basis to ensure medications are administered as ordered, appropriately documented on the MAR, and that missed medications are documented, and required follow-up occurs
* Returns completed med cards at the end of each month, along with expired and unused medications as applicable
* Checks program/service areas on a frequent but not less than a monthly basis to ensure the necessary medical/nursing/medication supplies are available and in ample supply
* Reports client illness, potential communicable diseases or infections and works closely with RN, direct care staff, other programmatic and health care professionals, and family in maintaining a safe environment for clients and staff. Implements isolation precautions as needed
* Ensures health status, care and services are documented accurately and timely per company and state requirements
* Provides supervision and education for non-licensed staff in the provision of care and services and reports any changes
* Complete a quarterly nursing evaluation/review of body systems on assigned individuals per program requirements. Refer to physician any assessment findings needing further review.
* Ensures incidents are recorded, the individual is safe, receives required medical care, communicates to RN, operations leadership timely
* Participates in clinics completed at the operation with PCP, psych, etc. as assigned.Participate in on-call duties to ensure weekend and after-hours access to clinicians by staff and individuals served (participates in on-call rotation as assigned)
* Participates in quarterly quality improvement activities, weekly team/staff meetings and Interdisciplinary Team meetings (IDT/PDP) Transcribes physician phone orders and other medically related reports
* Work hours may vary - weekends and evening shifts/visits may be required
Qualifications
Education: Completion of a state approved training program (e.g., diploma, associate's degree, etc per state requirement)
Qualifications:
* Must be a Licensed Practical/Vocational Nurse (LPN or LVN, per state licensure requirements) in good standing
* CPR certification
* Must have at least 6 months of direct nursing experience
* Experience in providing services and supports to individuals with developmental disabilities, or related disorders preferred
* Valid driver's license, with a satisfactory driving record, as defined by ResCare's vehicle policy
* Must have the ability to use a computer utilizing company documentation systems
* Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company

SUMMARY

ESSENTIAL DUTIES AND RESPONSIBILITIES

Assist in interviewing families and obtaining social histories.
Provide supportive back up to other Family Based team members.
Act as a liaison linkage to inter agency and intra-agency personnel.
Assist in coordinating referrals to other appropriate service providers.
Assist the family problem solving to focus on their strengths and collateral needs and service plan to address their identified strengths and needs.
Participate in screening, treatment planning and discharge process.
Rotate 24-hour emergency service seven days per week.
Attend and participate in appropriate conferences and training sessions as determined by the Program Supervisor and mandatory PCGC trainings.
Participate in staffing with the Mental Health Professional regarding the progress of the treatment plan.
Participate as co-therapist in all family therapy sessions with the Mental Health Professional.
Establish a close, extended family like relationship with identified families as a unit.
Ensure that all psychotherapeutic and case management goals will be linked directly to the treatment plan, treatment plan reviews and case-notes.

The BHT is responsible for:
Working with all members on the Team to support generalization and practice of new skills.
Completing the Social Determinants of Health (SDoH) screening tool and performing related case management activities.
Implementing crisis intervention strategies.
Applying and supporting clinical treatments, including Evidence-Based Treatments, Family Therapies, and the Core 4 Clinical Model.
Carrying out assigned components of the treatment plan across home, school, and community settings (e.g., behavioral plans, emotional support, positive reinforcement).
Providing continuous feedback to the Mobile Therapist throughout treatment to support improved outcomes.
Modeling positive affirmation for clients, families, and school staff.
Assessing strengths and therapeutic needs of the child and family through active listening, inquiry, and information exchange.
Working rotating schedules to meet the needs of clients and families during days, evenings, and weekends across settings throughout the calendar year.

* Accurately completes applicable paperwork; ensuring patient medical records are complete. Prepares all forms and requisitions as instructed by the provider.

* Reviews schedule daily and assists with office procedures to help maintain patient flow, and accuracy of scheduled procedure.
* Assists with inventory and order supplies for all procedures in the ASC, as directed.
* Sets up the sterile field on the Mayo stand with instruments, supplies, medications and equipment needed for procedures.
* Positions the patient for the procedures, establishes the sterile field, and drapes the patient for the procedures.
* Assists providers by passing instruments and supplies to them and holding retractors (as needed) during procedures.
* If assisting with procedures, adheres to all federal, state and nursing regulations and protocols including but not limited to patient safety procedures such as verification of patient identification prior to procedures
* Immediately prior to the start of any procedure in the procedure room, initiate/participates in a time-out and ensures correct patient, procedure, site and side, and that there are no additional concerns.
* Identifies every patient with two identifiers: full name and date of birth
* Documents this information in the Procedure Safety Checklist, including the time of the time-out
* Sterilizes and breaks down procedure room after each patient and ensures procedure room is cleaned prior to the beginning of a procedure.
* Keeps rooms clean and stocked with supplies.
* Maintains stock of medicines and supplies as necessary. Advises Office Manager when replacements are needed.
* Sterilizes and cleans instruments used in examinations and/or procedures; maintains clean and hygienic environment.
* Monitors personal and provider staff task buckets in the EMR.
* Sets up the sterile field on the Mayo stand with instruments, supplies, medications and equipment needed for procedures.
* Positions the patient for the procedures, establishes the sterile field, and drapes the patient for the procedures.
* Operates, sterilizes, and maintains necessary equipment, including but not limited to the autoclave, RFA machine, etc.

* Draws up injections to be administered (clinical office only).
* Verifies that radiological and laboratory records are received and in the patient chart. If radiological and/or laboratory records are not in the chart, takes necessary steps to obtain the records (clinical office only).
* Fits patients for durable medical equipment (DME) and completes all applicable paperwork, as directed (clinical office only).
* Obtains prior authorizations for radiology and/or prescriptions, as needed (clinical office only).
* Keeps rooms clean and stocked with supplies (clinical office only).
* Labels specimens accurately and distributes them to the appropriate personnel to ensure specimen integrity (clinical office only).
* Processes specimens and prepares them for shipping (clinical office only).
* Maintain lab area with ample supplies and in a clean and safe condition (clinical office only).
* Follows test requirements of laboratory and procures unaltered urine specimen for testing (clinical office only).
* Properly maintains examination rooms and all other clinical areas (clinical office only).
* Checks work e-mail on a regular basis throughout the workday.

Minimum Qualifications:
* High School Diploma, or equivalent.
