As a Pharmacist you will:
* Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
* Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
* Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
* Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
* Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
* Document all communications and clinical interventions in the patient's medical record, including interactions with families and caregivers.
* Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
* Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
* Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
* Educate and support facility/agency staff on safe and effective medication handling procedures.
* Perform other pharmacy-related tasks and responsibilities as assigned.

Required:
* Bachelor's Degree in Pharmacy, Pharm D preferred
* One year experience in long term care desired
* License/Certification(s): current and valid pharmacist license in the state of practice
* Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
* Proficiency in electronic medical records and pharmacy information systems
* Strong clinical judgement, and effective written and verbal communication

The Pharmacist in Charge position will be responsible for the regulatory oversight of KnippeRx's Patient Support Center in Lakeland, Florida. The PSC is a non-dispensing pharmacy responsible for the setup of the majority of orders dispensed by KnippeRx in Lakeland. This role reports directly to the General Manager of KnippeRx and will have a dotted line relationship to the Head of Pharmacy Operations while working closely with the KnippeRx PIC in Lakeland, Florida.
Responsibilities
KEY AREAS OF RESPONSIBILITY:
Operations Management - Works with Operations leadership to direct the activities of the non-dispensing Pharmacy in all aspects of its operation, including, , , customer service, quality assurance, regulatory control and short and long term planning. Achieve pharmacy cost reduction, operational goals, and productivity goals. Manage to achieve established quality parameters in, clinical services and patient service delivery. Assure compliance with all applicable operating procedures, accreditation standards and applicable federal, state and local rules, regulations, and laws. Support sales and marketing efforts in collaboration with other functional areas. Establish and maintain programs associated with ongoing facility operation including housekeeping, environmental monitoring, pest control, preventative maintenance, calibration, security and waste management. Develop and coordinate quality control activities. Support the clinical and financial information requirements of other functional areas within the organization. Develop and Monitor day to day activities of Pharmacy and Pharmacy personnel.
Personnel Management - Train and manage exempt and non-exempt pharmacy personnel. Direct the utilization of pharmacy personnel and resources to maintain a high level of customer service and satisfaction. Develop and manage safety programs for all pharmacy personnel. Direct and supervise the activities of pharmacy technicians and other supportive personnel in meeting the department objectives and care needs of patients. Evaluate performance and design development plans for all personnel.
POSITION RESPONSIBILITIES:

Establish and manage delivery services for all patients.
Develop and maintain clinical monitoring programs for all therapies and patients as appropriate.

Qualifications
REQUIRED EDUCATION AND EXPERIENCE:
Bachelor's degree with experience.
Current unrestricted pharmacy licensure in Kentucky
A minimum of 3 years of practical experience as a specialty pharmacist.
Proficient in the area of clinical interpretation; drug-drug, drug-disease, drug-food interaction and allergies
Experience with Specialty medications, Specialty disease management program, Oncology, REMS, and LDD programs
Demonstrated experience providing exceptional customer service.
Ability to read, write, speak and understand the English language.
PREFERRED EDUCATION AND EXPERIENCE:
PharmD from an accredited pharmacy program
Certified Specialty Pharmacist (CSP)
Board Certification by the Board of Pharmaceutical Specialties (BPS)
Previous HIPAA, Fraud Waste and Abuse and Confidentiality training
Clinical monitoring experience
Current CPR Certification
KNOWLEDGE, SKILLS & ABILITIES:
Significant experience in clinical monitoring services.
Excellent clinical pharmacy skills, laboratory value monitoring, and patient teaching skills
Ability to coordinate patient care and pharmacy operations.
Previous experience managing exempt and non-exempt personnel is preferred.

Prior pharmacy experience preferred
Ability to work a flexible schedule, including evenings and weekends

Responsibilities
* Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate
* Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies)
* Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team
* Collaborates with the healthcare team to provide clinical and technical support as needed
* Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
* Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs
* Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy
* Informs the Pharmacy Manager of medication related errors
* Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs
* Performs other tasks as assigned
Qualifications
* Bachelor's Degree in Pharmacy; Pharm D preferred
* One year experience in long-term care
* Licenses/Certifications: As required by government regulations and business needs
* Detail oriented; goal oriented, team player, oral communication
