Position Summary: The Pathways Case Management Team is designed to provide wrap-around case management referrals to youth with complex mental health needs. This position serves as a coordinator of this case management team. The team provides support and linkage to a continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Work within the community to identify and create partnerships that the team can utilize to assist clients achieve their treatment goals. Participate as a team member in the delivery of addiction and/or mental health treatment services to clients and their families and facilitate the issues of recovery into their daily living situations. Sign on Bonus Available! Masters Level $3,000.00 and Licensed LPC/LSW/LCPC/LCSW $4,000.00 Job Requirements: Master's Degree in Psychology, Counseling, Social Work, or other related field. Must qualify as a QMHP as defined by the Illinois Department of Mental Health Rule 132. Valid driver's license and ability to meet Agency insurance requirements Qualifications: Previous supervisory experience preferred Computer proficiency required. Ability to navigate and document in electronic healthcare records required. Ability to accurately complete required documentation within the time frames Essential Responsibilities: Ensure that the client service records are maintained consistent with required rules and regulations. Become competent in EHR and function as a trainer. Supervise assigned staff, including orientation, training, regular supervisory meetings and/or feedback, scheduled evaluations and disciplinary interventions as required. Maintain communications with all staff (e.g. admissions, and discharge plans). Assume on-call responsibilities per schedule or when designated. Present workshops, lectures and training to clients, families, and staff and community groups. Assist in treatment plan and assessment creation. Provide consultation to treatment team members to assist in assuring that client care needs are met. Identify and develop community treatment partners to assist in providing a comprehensive care program for designated clients. Assure ongoing communication and collaboration with community treatment providers once relationships are established. Provide case management and group coverage when needed. Ensure that a firm referral is established for each patient which includes date and time - supervisor role Act as point person when assigned Represent the department in PI committees and organizational initiatives as determined appropriate by leadership. Knowledge of psychotropic medications and ability to identify adverse effects of medications. Become familiar with all related state and county contracts, and complete monthly reports as required. Serve as a member of the department and participate in all team meetings and activities. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law. Deliver exceptional customer service consistently to every customer. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance. Job Type: Full-time Compensation & Rewards Base Pay: Masters - $55,161.60 (Plus eligibility for the operational leadership incentive of up to 10% of salary based on metrics/KPI achievement.) ESSENTIAL DUTIES & RESPONSIBILITIES * Assumes the leadership role within the program's assigned team and oversees the activities of crisis workers and engagement specialists performing Mobile Crisis Response (MCR) and assessment of members in the community to determine needs and make recommendations for treatment. * Works collaboratively community stakeholders and develops relationships with providers and partners in the crisis care continuum. * Assists the team in answering Thresholds crisis line and assessing caller to determine needs and level of intervention necessary. At implementation of the Statewide 988 crisis line, this position will assist the team in fielding these referrals and making determinations on the appropriate level of response. * Models person‐centered, recovery‐oriented care that is rooted in evidence‐based practices aimed at assessing crisis and determining appropriate safety planning. * Meets required targets and completes all documentation within assigned time frames. Monitors and coaches staff on meeting and documentation requirements. * Monitors and tracks member data, call flow, and insurance eligibility daily and collates this into monthly reports for distribution to Thresholds leadership and funders. * Represents Thresholds in monthly ED and behavioral health meetings to address ED operations, patient flow, monitor effectiveness of crisis services, and identify barriers and improve systems. * Reviews all required documentation. Maintains the quality, validity and confidentiality of individual case records according to CARF, Medicare, Medicaid standards and agency traditions. Assures that all member files and documentation are completed accurately, on time and is organized in a systematic manner. * Monitors employee compliance with required training. * Assists with special tasks as assigned, including but not limited to: representing the agency at meetings, conferences, supervising students/interns, orienting and training new staff. * Conducts annual evaluations in a timely manner for all team staff. Develops specific goals and objectives, and provide the method and opportunity for each team staff to succeed in their professional development. * Participates in the hiring sequence of the agency, and make recommendations on the hiring or termination of team staff. * Follows agency standards regarding clinical supervision. * Assists Program Director with the development and coaching of staff, to ensure they consistently meet agency standards. * Responsible for maintaining 24/7 coverage of the MCRT including the management of a substitute staff pool. * Conduct MCR on an as needed basis when coverage is necessary for crisis workers. * Required to be on‐call for assigned shift to assist in maintaining coverage of the MCRT and to provide clinical support. * Must be able to remain in a stationary position for prolonged periods of time and occasionally traverse long distances and ascend/descend multiple flights of stairs. * Shift: Sunday - Thursday 2:45pm-11:15pm MARGINAL DUTIES & RESPONSIBILITIES * Works flexible schedule including work on weekends or evenings as required. EDUCATION * Must, at a minimum, meet Office of Mental Health Rule 132 credential requirements for a Mental Health Professional (MHP): * Bachelor's degree in social work, counseling, psychology or closely related field or * Certified Recovery Support Specialist (CRSS) * Master's degree in social work, counseling, psychology or closely related field is preferred. SKILLS/CERTIFICATIONS * LSW, LPC, LCSW, or LCPC is preferred. EXPERIENCE * Two (2) years clinical experience. * Leadership experience preferred. * Experience with crisis assessment preferred. * Ability to synthesize and summarize information and make judgments regarding member care. * Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records and working knowledge of pertinent software. * Strong interpersonal skills and knowledge of group dynamics required. * May be required to provide transportation for members of the program using a Thresholds provided vehicle or personal vehicle. * Must have a valid driver's license, be at least 23 years of age, and must meet all insurance carrier's requirements. * Must obtain and maintain $100,000/$300,000/$100,000 bodily injury and liability. * Subject to validation every six months.
