The primary purpose of the LCSW (Integrated Care) is to provide clinical support in the areas of behavioral health. In this role, the LCSW will work with a multidisciplinary team to support patients with addressing their behavioral health and physical health conditions. We are looking for a highly motivated clinician who works well in fast-paced settings and is willing to grow in integrated health care. This position also provides technical assistance and monitoring to patients with high-risk behavioral health needs and intervention services. It is anticipated that the SocialWorker will facilitate and/or be involved in statewide initiatives to address integrated care and behavioral health expansion. This position provides behavioral health consultation to other departments at Advance Community Health by sharing best practice recommendations, providing auditing of clinical records, clinical policy review and training. In addition, the LCSW serves as a lead clinician to support behavioral health team members in their daily functions and supports the team by providing clinical supervision to provisionally licensed team members as needed.

Essential Duties and Responsibilities
Provide quality direct behavioral healthcare to patients and family members
Provides/ conducts psychosocial assessments and screening to assists the patient in utilizing the support systems and community resources available
Provide crisis intervention services to patients as needed
Provide brief counseling support to identified patients
Support the medical treatment plan to support patients with improving their chronic health conditions
Demonstrates critical thinking skills by performing appropriate interventions
Identifies the impact of cultural diversity and lifestyle choices on patient condition and makes accommodations for such differences
Assess, plan and implement individualized treatment plans for patients engaged in behavioral health services
Build and maintain relationships with caregivers and healthcare providers to support patients in addressing the physical and psycho-social barriers to optimal outcomes.
Identify any impediments to patients' progress and advocate for patients and families as needed.
Meet with individuals to discuss current needs and create plans to address needs
Utilizes knowledge and required skills to ensure appropriate assessments
Consult and plan meetings with other professionals regarding patient's treatment services
Prepare and maintain records within EHR
Maintain an up-to-date referral system
Contributes to optimal efficiency and productivity of the clinical care team by anticipating needs of patient and communicating with medical provider regarding patient care

ROLE OVERVIEW:
SocialWorkers at KIPP North Carolina provide crucial support and services to students. In this role, socialworkers will work directly with students, teachers, community partners and families to provide individual and group counseling, as well as link families to long-term solutions and external support organizations. SocialWorkers report to their school Principals and are coached by the Director of School SocialWork on the Regional Support Team.
Responsibilities of a School SocialWorker include, but are not limited to:

Work involves screening clients who are referred for addictive and/or psychological disorders. Employees use standardized tools to screen clients, conduct intakes and orientations, and complete assessments (social, educational, cultural, psychological, physical, career, and familial histories). Employees provide behavioral intervention therapy related to criminal related behaviors and attitudes for those clients involved with the criminal justice system. Employees may serve as an interface between mental health providers while continuing to provide substance use disorder counseling. Primary Purpose of the Organizational Unit:
The Child Services Division of the Johnston County Department of Social Services provides Protective Services Intake, Investigations, Assessments, Emergency On-Call, and Case Planning and Case Management; Permanency Planning; Foster Care Reunification and Placement; Independent Living; Recruitment and Training for Foster Parent Families and Adoptive Applicants, Agency Adoptions; and completion of Home Study requests from other North Carolina Departments of Social Services and the Interstate Compact.
The Staff Development Unit consists of CQI/Trainers who define, measure and improve outcomes on an ongoing basis in the Child Services Division. The CQI team completes case record reviews; coordinates State and Federal audits/reviews; coordinates and/or provides training to agency staff; compiles, analyzes and disseminates data; manages the agency's internship program; and completes other duties as assigned by the DSS Director or their designee.
Fifteen percent (15%) of this role involves training agency staff on essential policies, practices, and procedures to ensure consistent, high-quality service delivery. This responsibility includes developing and delivering training sessions that strengthen staff understanding and adherence to local, state, and federal mandates.
This employee's primary responsibility is to monitor performance, using case reviews, and process reviews and data analysis. This employee uses case reviews to evaluate adherence to federal, state and county policy guidelines and to assess the quality of work performed by individuals and teams within the specified Division or program area. This employee will also perform other duties as directed by CQI SocialWork Supervisor or the DSS Director as it pertains to assuring quality performance in the services provided by the agency. Candidates will have the opportunity to explore the entire range of services ITSS offers including, but not limited to, business Internet, managed IT services including active directory management, VoIP services, web design & development, network infrastructure installation, cisco networking, as well as OEM server and desktop manufacturing.
Requirements
High School coursework in the field of computers, marketing or business,
FBLA, DECA or other business club advisor recommendation. (alternatives include coach or school administrator)
Availability to work weekends
Must have valid driver's license or transportation to ITSS Corporate Offices on a regular basis
Position is located in Goldsboro, NC and is not a remote position.
