Responsibilities
Provide high quality patient care in conjunction with fellow team members during transport between facilities.
Perform assessment and stabilization of patients as well as any necessary diagnostic and therapeutic interventions.
Assure quality of care by adhering to therapeutic standards; making or recommending necessary adjustments by collaborating with team members and multidisciplinary members.

Requirements
Licensure / Certification / Registration:

Certification specializing in Neonatal Resuscitation credentialed from the American Academy of Pediatrics (AAP) obtained within 6 Months (180 days) of hire date or job transfer date.

PURPOSE AND SCOPE:
The professional registered nurse Inpatient Services RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the kidney disease health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and program operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations.
Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse.
Performs ongoing, systematic collection and analysis of patient data pre - during - post treatment for assigned patients and documents in the patient medical record, makes adjustments and modifications to treatment plan as indicated and notifies Supervisor, Physician, patient's primary nurse and others as may be indicated.
Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input including but not limited to Supervisor, appropriate physician, and the contracted facility primary Nurse.

Initiates and coordinates communication with FKC and Non-FKC dialysis providers and appropriate contracted facility personnel as needed to provide continuity of patient care.
Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier.
Administers medications as prescribed and documents appropriate medical justification and effectiveness.
Initiates and assists with emergency response measures.
Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
May be asked to provide specialized nursing care instruction to hospital/facility staff as stipulated contractually.
May be assigned to assist in an Outpatient facility on an as needed basis.

Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provides day to day direction and supervision to assigned direct patient care staff. Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. PRINCIPAL DUTIES AND RESPONSIBILITIES:
General and Staff Related:
Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Clinical Manager and initiate as appropriate.

Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication.
Technical:
Supervise the safe and effective use of all equipment involved in direct patient care.
Operate all dialysis related and emergency equipment safely and efficiently when needed.
Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual.

Works with the Facility Manager, facility staff and physician to coordinate the facility operations and patient procedures to ensure provision of quality patient care on a daily basis in accordance with policies, procedures and training. PRINCIPAL DUTIES AND RESPONSIBILITIES:
QUALITY:
Responsible for supporting and driving quality standards through meeting regulatory requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools.
Assists with implementing quality goals and develops facility specific action plans to achieve quality standards.
Accountable for outstanding quality of patient care, as defined by the quality goals, by working with management to ensure that policies and procedures are followed.
Assists with implementing appropriate training according to policy; ensure ongoing compliance with all risk management initiatives.
Responsible for aggressively addressing and acting on adverse events and action thresholds.

PATIENT RELATED:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge.
May confer with patients prior to the procedure to obtain necessary medical history, discuss procedure and answer questions, as well as, performs nursing assessment on the day of the procedure.
Ensures patients and patient's families are educated regarding post procedure care of vascular access.
Acts as a resource for the staff to address patient concerns and questions.
Ensures patient's procedures are scheduled in a timely manner and the facility capacity is utilized efficiently.
As needed, assist with patient workflow, including providing direct patient care and monitoring pre, intra and post procedure.
Identifies and communicates patient related issues to the Facility Manager.
Oversees and ensures accurate and complete documentation of patient treatments from admission to discharge.
Confirms written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions, delivery to appropriate labs, reporting and forwarding of lab results to appropriate physician.
Assumes primary responsibility in an emergency situation and must maintain competency with all emergency operational procedures, and initiate CPR and emergency measures as needed.
Administers medications and IV conscious sedation to patients as directed per physician's orders, and in compliance with, company, federal, state and local regulations.
PHYSICIANS:
Ensures regular and effective communication with all physicians on an as needed basis.
Attends and participates in Governing Body, assisting with meeting agenda and gathering of data/material for meetings.
Attends and participates in CQI meetings with physicians, reporting on assigned topics.
Works with the physicians and staff to ensure possession of all necessary instruments and equipment.
MAINTENANCE/TECHNICAL
Ensures appropriate operation of facility equipment and technology, including but not limited, patient monitors, defibrillators, medical recording devices and computers.
Ensures the inspection of all facility equipment on a regular basis. Report any malfunctions or maintenance issues immediately.
Ensures the inspection of all facility operating systems, including temperature checks in appropriate areas and inspect the physical structure of the facility.
Maintains a clean, safe and sanitary environment in the entire vascular access facility at all times.
Ensures all blood spills are immediately addressed according to company Bloodborne Pathogen Control Policies.
ADMINISTRATIVE:
Participates in cost control initiatives. Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the formularies.
Assists in the development and revision of any applicable policies and procedures for the facility as directed to ensure continuous process improvement.
Initiates, documents and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports and attend meetings as required.
Manages medications, including ordering, receiving, and inventory control and checking for expired medications, including appropriate recording of controlled substances as required by law.
Oversees maintenance of usage logs for all medications with appropriate documentation, as well as, records of drug disposal. Communicates with the pharmacist to assure medication accuracy, including appropriate recording of controlled substances as required by law.
Ensures medical records are accurate, complete and in compliance with policies and procedures prior to discharge of the patient. Performs chart audits on a regular basis.
Along with Facility Manager, coordinates the annual medical testing and competency testing for all facility staff.
Participates in all facility regulatory and accreditation surveys and action plans to address any deficiencies identified.
Verifies records for all implanted devices are completed accurately, timely and maintained according to records management guidelines.
Coordinates the collection and forwarding of all vendor invoices to the accounting department.
Acts as the Facility Manager in the absence of the manager or at the direction of management.
Ensure nurses' progress notes are accurate and complete.
Acts as the Patient Safety Officer and conducts quarterly patient safety meetings and fire drills.
Complies with HIPPA policies and standards regarding patient information and medical records.

Overview
BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country.
Evelyn's House is the newest addition to BJC Hospice and BJCs continuum of care for patients with life-limiting illness. Evelyn's House staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in pain and symptom management, counseling, and bereavement services for the dying. BJC Hospice services include alternative therapies such as music, art and massage therapy. BJC Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose
Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.
Responsibilities
Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plans and ensures communication of the plan of care across the continuum of care.
Promotes respect, equity and empathy in interactions with diverse and vulnerable populations through care delivery (e.g. support for emotional, spiritual, and cultural preferences of patient, family and/or caregivers). Practices collaborative problem solving, service recovery and advocacy for patient family centered continuity of care. Implements care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
Evaluates changes in patient's condition, informs and collaborates with family and/or caregivers, and communicates with interprofessional team as changes occur in plan of care, updates plan of care in EMR. Evaluates current nursing care to ensure evidence-based practice and quality patient outcomes.

A BJC Central Staffing Office nurse will be required to float to 12 BJC Healthcare Hospitals within their Medical, Surgical, and Telemetry Units. Responsibilities

Educates the patient, family, and significant other(s) about measures that promote, maintain, and restore health or promote comfort.
Uses standards of medical-surgical nursing practice to increase the quality of care and quality of life for the patient, family, and significant other(s) to acutely ill patients in a highly technical and ongoing monitoring environment. These patients are facing life-threatening problems with varying needs, which require extensive medication protocols and therapies. Demonstrates competency in evaluating intensive care patients, administering care and recognizing complications by systematically evaluating the quality and effectiveness of nursing practice. Establishes effective relationships and serves as an advocate to facilitate the development of the care plan unique to the patient's care needs. Uses the nursing process to develop and implement the plan of care, collect assessment data for each patient, determine appropriate nursing diagnoses, identify expected patient outcomes, determine the plan of care, implement interventions, and evaluate patient outcomes.
