Requirements for a RN: RegisteredNurse:
Current and valid NYS RN License

Responsibilities for a RN: RegisteredNurse:
Provides direct care and treatment to individuals in assigned programs through a variety of nursing procedures.
Assesses health care needs of individuals as required.
Assess an individual who becomes ill, administers first aid for inquiries and coordinates follow-up.
Keeps medical records current and complete in compliance with all regulations as they pertain to the program and works within the framework of the Nurse Practice Act.
Collaborates regularly with team members, caregivers and other professionals as needed.
Participates in training and development in accordance with agency policy.
Monitors safety and infection control standards and adheres to standard safety practices and job, department and Center policies and procedures related to safety.
Checks nursing equipment as indicated.
Attends nursing/program meetings as scheduled.
Documents all current health related information in nurse's notes and/or medical record.
Assist with occupational health duties for staff.
Communicates regularly with other service providers, program staff, administration, etc., re: changing medical issues.

Specific Duties and Responsibilities
Triage and deliver substance use/mental health screenings and clinical assessments to Latino and Black youth and their families, and provide/oversee warm hand-offs to services as indicated.
Ensure care retention through individualized case management, needs assessment, counseling (via text, phone, or in-person), and family support in navigating the behavioral healthcare system.
Provide overall coordination for a given family's service package.
Implement brief and extended interventions for substance use as indicated, appropriate for developmental age and aligned with level of risk.
Employ brief client-centered, non-judgmental motivational interview techniques that raise the participant's awareness of their substance use and that promote behavioral change and reduced substance use.
Work alongside community health workers and families in developing a tailored substance use prevention and treatment plan for the entire household
Minimum Qualifications
Graduation from an accredited school of nursing with licensure as an LPN.
Current CPR certification required. Must maintain current licensure and certification during duration of employment.
Completion of the JHU credentialing process to include verification of CPR certification is required prior to start date.

Job Description
We are seeking a skilled RegisteredNurse to join our Medical Surgical Telemetry unit. This role involves caring for patients in need of telemetry monitoring while providing comprehensive nursing assessments and delivering high-quality patient care. The RN will work collaboratively with a multidisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities
Assess and monitor patients' health status
Administer medications and treatments as prescribed
Coordinate patient care with healthcare team members
Educate patients and families on health conditions and care plans
Document patient assessments and interventions accurately in the electronic medical record
Required Experience / Certifications / Licensure
Minimum of 1 year clinical experience required
Minimum of 1 year experience in specialty (Telemetry/Medical Surgical) required
AHA BLS certification is mandatory
CMSRN certification is preferred
NIH Stroke Scale certification required prior to starting
Current New York RN license or proof of application required

OBJECTIVES OF THE POSITION
To deliver comprehensive, high-quality, trauma-informed nursing care and health education to youth at The William George Agency by the NYS Office of Professions' RN scope of practice and Article 29-I VFCA Health Facility guidelines. This role ensures routine, chronic, and urgent health services are provided effectively to youth in care while promoting best practices in documentation, infection control, education, and interdisciplinary collaboration in a residential care setting.
DUTIES AND RESPONSIBILITIES
Maintain a courteous, respectful, and professional demeanor at all times when interacting with residents, internal staff, nursing and medical personnel, external healthcare providers, hospitals, parents, and other professionals involved in the coordination and delivery of resident medical care.
Conduct medical assessments, including 24-hour screenings, intake evaluations, and HIV risk assessments per regulatory guidelines
Conduct triage and provide first aid, blood draws, and manage chronic conditions (e.g., asthma, diabetes), and provide reproductive health education
Administer and reconcile prescribed medications per NYS and Article 29-I regulations using blister packs, strip packaging, EMAR, and tracking logs
Maintain compliance with immunization schedules and vaccine storage protocols (e.g., daily temperature logs, VIS documentation)
Document all services accurately and in a timely manner in the Electronic Health Record (EHR) (Welligent), including medical appointments, session notes, and medication administration.
Maintain compliance with recordkeeping, medical safety plans, clinic logs, and discharge/travel medication packaging
Collaborate with physicians, NP's, behavioral clinicians, and residential staff to establish and monitor the youth's treatment goals
Coordinate transport and accompany youth to on-/off-campus appointments as needed
Deliver trauma-informed education to youth on hygiene, chronic illness management, and safe health practices
Support staff education on OSHA, infection control, seizure protocols, and medication protocols as needed
Maintain clinic cleanliness, secure storage of medications, and supply inventory
Follow all protocols for off-campus referrals, incident follow-up, and post-op care
Participate in internal audits, quality improvement, and safety initiatives
Rotate on-call cell phone coverage to support after-hours needs
Record and report after-hours contacts per agency protocol
Support staff training related to medical procedures and medication protocols
Attend nursing, administrative, and agency-wide meetings as requested to support the needs of the clinic and the Agency
Assist in staff physical exams, including BP, Height, Weight, and BMI, and maintain written records for the medical clinic/personnel.
Perform other duties as assigned by the Medical Director, Nursing Supervisor, or Assistant Nursing Supervisor.

GENERAL STATEMENT OF DUTIES
Performs professional nursing services at one or more schools in the maintenance of acceptable health standards, school health records and the emergency treatment of student and employee health problems; performs related duties as required.
COMPLEXITY OF DUTIES
Under the general direction of a School Physician or School Nurse-Teacher, this is a professional nursing position involving responsibility for giving care to students and for performing related nursing services such as pupil health screening and notification of defects and communicable disease control. This position differs from the certificated position of School Nurse-Teacher in that there is no responsibility involved for either classroom instruction or guidance. Supervision may be exercised over the work of clerical assistants.
TYPICAL DUTIES
1.
Assists the School Physician in physical, visual, and auditory screening examinations of students and employees.

Administers first aid and emergency treatment to students and employees.
3.
Prepares and maintains health records for school authorities.
4.
Assists at school immunization clinics.
5.
Arranges to transport sick or injured students or employees to hospital, home, doctor's or dentist's offices in cases of emergency.

Inspects the school plant, playground, and cafeteria and reports on general safety and sanitary conditions.

Orders, inventories, and oversees the storage of first aid and related health supplies and equipment.
8.
Consults with attendance teachers, staff members and School Nurse-Teachers concerning a variety of health factors related to non-attendance and communicable disease.

Requirements for an LPN Charge Nurse:
Current and valid NYS LPN License
CPR for the Professional Rescuer Certification
At least 1 year of job-related experience, specifically in field of nursing
High school diploma, or equivalent
Responsibilities for an LPN Charge Nurse:
Completes daily review of 24-hour report and follow up as needed.
Assists RN Unit Manager in preparing for medical provider rounds and communication to staff on who will be seen by medical provider on that day. May assist Unit Manager in preparing scripts that need to be signed by medical provider.
After medical provider rounds, assists Unit Manager ensuring all orders are completed and started as ordered. Communications to all necessary clinical team member for any follow up. Communication to all staff via 24-hour nursing report. If Stat orders, receive order from Pyxis cart. Ensure that Clinical Care Coordinator is aware of any appointments that need to be made and work with Staffing Coordinator to staff the appointment and attend appointments, as necessary.
Ensures daily that all labs and/or diagnostic reports have been received. Communicates any abnormal flagged lab results to RN Unit Manager / RN Supervisor and contact provider with abnormal findings as soon as possible.
Updates Electronic Infection Log in eMar as needed with all pertinent information. Notify Infection Control Preventionist of any new infections.
Participates in Wound Rounds as needed.. Notifies Unit Manager / RN who will assess the information for further treatment needs.
Collects incident reports; notifies appropriate persons as indicated on incident report. Attaches pertinent information, i.e., nursing progress notes, care plans, Kardexes, statement from employees if necessary. Logs Incident Report into logbook and files in book.
Makes unit rounds throughout the shift. Completes pertinent assigned audits. If problems noted at time of rounds, notifies appropriate Unit Manager or Department Head as needed. If training needed, notifies Educator/Infection Control RN.
Provides daily oversight of CNAs on unit.
Initiates and maintains unit assignments.
Provides medication coverage on med cart / treatment cart when needed.
Makes rounds and oversees mealtimes during meals.
Prepares individual resident clinical case conference sheets - ensures up to date prior to scheduled case conference - attend meetings in the absence of Unit Manager.
Attends daily morning report as needed or to cover in the absence of and at direction of Unit Manager.
Ensures supplies in back packs daily for residents that require them.
Works with Clinical Care Coordinators to manage supplies of the unit.
Ensures documentation for external MD appointments and consulting MD visits are complete prior to visit.
Checks dashboard on PCC - ensure C.N.A documentation is 100% complete before end of shift. If not, report to C.N.A prior to leaving facility / shift. Report to Unit Manager any supervision issues.
Follows a work schedule of 7am-3pm Monday through Friday and ensures/support coverage for the evening and OVN shifts
