    The Community Family Intervention provider, as a member of the Outpatient Family Services Team is responsible for providing intervention services to children and families. These services may be provided in various locations both internal and external to the organization and is driven by the clinical needs of the entire family unit. Services include, but are not limited to: individual, group and family intervention, crisis intervention, prevention consultation and education, and linkage to needed community resources. This position reports to the Program Director and training elements are provided by the CPST Coordinator. ESSENTIAL DUTIES & RESPONSIBILIES Contributes to creating a welcoming and inclusive environment for our clients, colleagues and community partners from diverse backgrounds and experiences. Integrates principles of Trauma Informed Care in all aspects of work. Is sensitive to the children's culture and socioeconomic characteristics that delivers the best possible care to the youth and families served. Provide case management (advocacy, monitoring and linkage services) to children and families participating in the program. Provide prevention, consultation, and education services to school staff, youth, and families. Assist in the management of crisis situations with the exclusion of restraint. Assist students and their families with social, emotional, education adjustment and development. Provide billed services at a minimum of 100% of the stated Agency expectation on a monthly basis. Transport clients and family members occasionally on an as-needed basis. Maintain accurate documentation of direct and indirect services provided in accordance with ODMH, ADAMH, and agency standards. Gather statistical and research information to be used for outcome purposes. Participate in staff and team meetings for the schools and agency, staff development and in-service training, planning interventions and regular supervisory conferences. Bilingual (English/ Spanish). Fluent in both spoken and written communication for both languages. Knowledgeable of community resources that can support individuals in the Hispanic/ Latino community. Able to demonstrate cultural competency as it relates to working with individuals from different Hispanic or Latino cultural backgrounds. Other duties as assigned or requested. EDUCATION & EXPERIENCE Bachelor's degree in social work, Counseling, or other related discipline is required. Minimum of two years of experience working with children and families preferred. Rendering Provider number required based on level of licensure. National Provider Identification number required. Excel, Word, Outlook, MapQuest and phone skills needed. Valid Driver's License and Proof of Auto Liability Insurance with required limits needed. REASONING ABILITY Ability to maintain high degree of empathy and compassion in meeting the needs of agency clients and client families. Ability to build strong bonds with employees to foster open, honest and candid communication. Ability to multi-task and maintain organization in a fast paced, changing environment. Ability to manage change in an organization reengineering its culture and approach to workload management. Ability to successfully operate with ambiguous guidelines where ethical decision will be required. Ability to create and maintain highest levels of confidentiality when dealing with client information, SVFC proprietary information and sensitive situations. LANGUAGE SKILLS Ability to effectively communicate plans, goals, directives and diagnosis information between clinician and clients. MATHEMATICAL SKILLS Ability to calculate figures and amounts using basic math skills. Assist the Clinicians with the children's discharge process to include passing of information on follow-up appointments. Coordinate with Medical Services for children's routine and emergent health care, and hygiene supplies for children. Coordinate youth transports which include but are not limited to, medical appointments, hair appointments, court appointments, home visits, external counseling and agency visits. May also provide transportation to appointments. Gather and maintain clothing inventories of children in care. Ensure completion of clothing vouchers as needed. May assist in medication administration as needed. Meet established Community Psychiatric Supports and Treatments (CPST) productivity requirements for services pursuant to OAC 5122-29-17. Makes recommendations to the psychologist regarding further student/client testing (i.e. if the child needs an IEP). Collect data and documentation then communicates as an active member of the treatment team. Attend development classes as requested by supervisors. Assists the Adaptive Behavioral Specialist with mental health interventions, as needed. Implements and uses trauma informed care services. Is sensitive to the cultural and socioeconomic characteristics of the children and families served. Performs general office duties to include, filing, copying, faxing and customer service. Other duties as assigned or requested. Education and/or Experience: Bachelor's degree in education is required. Master's in special education is preferred. Current Ohio special education/intervention specialist licensure in pk-3 is required/k-12 preferred. Language Skills: Excellent verbal and written communication skills are required. Writes and updates IEPs for students on caseload. Holds meetings for updates, transitions, or other district needs for special education students. Writes progress notes every 4.5 or 9 weeks for IEPs. Maintains student data for goals on IEPs for students on caseload. Communicates with parents, on an as needed basis, regarding student/client academic concerns. Tracks absences and tardiness of students/client. Completes and disseminates grade cards four (4) times per year. Conducts a minimum of two (2) Parent-Teacher conferences during the school year. Makes recommendations to the psychologist regarding further student/client testing (i.e. if the child needs an IEP). Collect data and documentation then communicates as an active member of the treatment team. Attend development classes as requested by supervisors. Assists the Adaptive Behavioral Specialist with mental health interventions, as needed. Implements and uses trauma informed care services.
