Your Specific Responsibilities:
This position provides case management to persons infected and affected by HIV/AIDS within the two-county area of Volusia and Flagler Counties (Area 12). This position provides case management activities through office or field visits according to the guidelines established by the Florida Department of Health Bureau of Communicable Diseases HIV/AIDS Section and the Area 12 HIV/AIDS Program Office. This position reports to the SENIOR HUMAN SERVICE PROGRAM SPEC-SES (64067641). Specific duties are as follows:
Completes Comprehensive Needs Assessments of individuals presenting themselves for assistance through the agency. Develops individualized care plans to address identified needs. Responsible for the implementation and oversight of care plans. Ensures Ryan White services are consistently used as the payer of last resort. Updates all required information regarding payer source eligibility on his/her clients in the FL RW Portal, CAREWare and HMS Databases within three business days. Completes and maintains accurate documentation of all client-specific activities. Case notes, scanned documents and all required information on clients are entered in the FL RW Portal and CAREWare database three business days. Promptly links clients to appropriate agencies for services and activities required to assist the client in achieving the goals and objectives identified in their care plan. Facilitates linkage and re-engagement activities for clients who appear to be out of care. Authorizes for transportation for clients to receive medical services and other social aid services (including provision of food vouchers) as needed. Educates clients concerning HIV, its transmission and complications, HIV risk reduction, insurance and entitlements. Advocates for clients and assists with navigation, referrals and enrollment.
Completes Acuity Assessments including the following areas:
Basic needs (i.e., financial, nutrition); Co-morbidities; Dental; Family planning. Health insurance/medical care coverage; HIV disease state; Housing/living arrangements. Mental health; Risk behaviors; Self-sufficiency (i.e., activities of daily living, literacy. Substance use; Transportation; Treatment adherence Clients are contacted based on their acuity level and monitored accordingly. Acuity Level - Reassess acuity annually unless client situation changes or if service requests become frequent. Recommended minimum contact (phone or face-to-face) every six months.
Miscellaneous Activities:
Attends and participates in general staff meetings, staff development, such as team/staff meetings and in-service/continuing education.
Attends computer skills training classes as needed.
Performs related duties as necessary including public health services during disaster.
Maintains timely submission of EARs and timesheets.
Performs other duties as assigned.
Required Knowledge, Skills, and Abilities:
Ability to work with HIV/AIDS clients
Skilled at verbal communication, telephone etiquette, and use of DOH-Volusia telephones
Ability to provide guidance and assistance to clients, coworkers, and community partners

Job Summary:
The Adoption and Guardianship Subsidy Case Manager provides senior-level case management with direct/indirect assistance to hard to place adoptive children and their families. This position reviews applications to determine eligibility for the respective programs, and creates cases in the software systems to allow for payments to be processed and the child to receive insurance coverage. In addition, this position provides ongoing support/advocacy to the family.
Job Duties:
* Determines if hard to place children in adoptive/guardianship homes are eligible for adoption or guardianship assistance respectively using completed federal and state criteria. Supports the integration and stability of the child with the adoptive family. Provides adoptive parents with resources available through public and private entities. Clearly and accurately documents each "special condition' of the child on the adoption subsidy agreement.
* Provides case management and crisis intervention services to adoptive families of children who meet the "legal definition of a child with special needs". Provides comprehensive senior-level case management services and supports to prevent these adoptions from disrupting and to support the family unit through the provision of needed social services. Consults with the child's behavioral health and medical professionals, adoptive parents, school personnel and Department of Child Safety (DCS) staff to identify the level and frequency of services required to maintain children in their family unit.
* Completes all required documentation on approved cases to comply with state and federal requirements. Assures that adoption subsidy agreements are executed prior to the finalization of the adoption. If case managers reduce, terminate or deny services for the child, the case manager must inform the adoptive parent of their appeal right/due process, and prepare the case for the appeal hearing.
* Provides training/technical assistance to adoptive parents and adoption workers on department policies/procedures. Provides DCS permanency staff with ongoing information regarding the adoption subsidy application process, documentation requirements, statutory time frames and prior authorizations requirements. Ensures that all processes are completed timely and accurately in computer software systems that allow for payments to be processed and medical coverage to be in place.
* Reviews and resolves high profile case-specific issues and concerns. What will you do:
This is work responsible for providing residential care, assisting with enrichment activities and supporting the treatment, psychiatric rehabilitation, stabilization and recovery of people with a mental illness. As a Human Services Worker II you will treat people in a respectful manner that preserves their dignity and rights and maintain a professional caregiver relationship with people served.
How you will make an impact:
The incumbent is responsible for preserving the rights, safety and security of people served by observing them:
* At all times to learn what type of environment and interactions make them feel safe, secure and calm and to identify distressful stimuli or triggers:
* When providing ward coverage.
* When escorting to and from activities and appointments to ensure that they arrive safely.
* During meals to monitor intake and ensure that they don't choke or trade food.
* When a person served is placed on special observation status to address dangerous or high risk behaviors.
* When a restrictive intervention such as seclusion or restraints is used to assess for release criteria and immediately identify health needs that may arise.
Communicating with them:
* At all times to develop a trusting, caregiver relationship and identify signs of distress or health concerns.
When a person served is placed on special observation status, in the seclusion room or in restraints to assure the person that he or she is safe, help them develop trust in you as a caregiver and identify signs of distress or health concerns.
Intervening:
* To separate them from distressful stimuli and assist in resolving conflict.
* To ensure that people served are not neglected, abused or exploited; or to ensure that it is reported to the abuse hotline if they are.
* To ensure safety during medical or disaster emergencies.
* To ensure safety when a person served is exhibiting a behavior dangerous to self or others, which may involve the use of physical holds and placing people served in a seclusion room or mechanical restraints.
Qualifications:
* Must be 18 years of age at time of hire
* Must have six months of experience working in a medical, psychiatric, nursing, or childcare setting, customer service experience or in working with developmentally disabled persons; or in working in a correctional facility OR college or vocational/technical training from an accredited institution at the rate of 15 semester, 22 quarter or 360 classroom hours; OR have a valid CNA license.
* Ability to attend a 1 - 2-week mandatory training course that will include completion of a CPR certification. This training could be scheduled during day, weekend, and evening hours.
Physical agility requirements for Direct Care Staff:
* Must be able to ambulate up and down a stairwell.
* Must be able to assist with evacuation of persons served during an emergency situation.
* Must have normal vision and hearing, including use of corrective devices.
* Must be able to work with or around cleaning chemicals and supplies.
* Must be able to do repetitive movements with arms, hands, neck and head.
* Must be able to demonstrate competency, as appropriate, in all areas of training, some of which requires physical activity such as;
CPR and MANDT:
* Must be able to bend, twist, turn, kneel, stoop, squat, push, pull, lift and walk extended distances.
* Must be able to physically operate all equipment necessary to do job tasks.
* Must be able to lift and carry 20 pounds.
* Must be able to sit and/or stand for one hour without a break.
* Must be able to remain alert while on duty.

* The incumbent in this position will work with residents who have been charged with felony offenses and may have psychiatric disorders.
* The incumbent in this position will assist residents in completing oral care, bathing, and showering as well as driving them to facilities and medical appointments etc.
