Qualifications:

* Answers general inquiry eligibility questions related to program requirements, application processing, case status, and benefit information.
* Processes a variety of eligibility related work activities within designated time frames
* Educates customers on the benefits and features of My ACCESS Account including account set-up and password resets.
* Resolves Electronic Benefit Transfer (EBT) ACCESS card issues.
* Conducts abbreviated interviews for the purpose of collecting and updating required information on applicants and recipients and their household members for all government assistance programs.
* Requests all additional information/verification required to establish or continue eligibility for government assistance programs. Ensures electronic case records are documented thoroughly and properly.
* Assists customers with referrals to other agencies and community resources.

Working under the direct supervision of the HEALTH SERVICES SUPERVISOR - SES the incumbent in this position serves in a developmental capacity, providing direct disease and public health awareness intervention activities to CHD STD Prevention and Control Programs including PROACT, epidemiological case analysis and investigations, notification, counseling, and referral services of individuals exposed to or at-risk of infection for HIV/AIDS.
* Communicates with public and private clinics, clinicians, hospitals, laboratories, and other health care providers to obtain demographic, medical, or other specific client information including address, telephone number, place of employment, social security number, race, sex, date of birth, test results, diagnosis, treatment, etc. and/or to arrange medical evaluations and treatments for a client.
* Conducts field investigations in accordance with current STD Program Policy to locate and provide education and linkage to care to individuals infected with HIV/AIDS and/or to refer individuals infected with or suspected to be at-risk of infection for STD's including hepatitis, HIV/AIDS or other bacterial, biological and/or viral agents for counseling, medical evaluation and/or treatment. This position will contact clients that have been lost to care and have not picked up their medication or have lost their Ryan White certification.
* Performs case management activities as directed in the process of discovering and/or identifying individuals infected with HIV/AIDS. These activities will include but not limited to: Work prioritization and records documentation and management, clustering, HIV pre-and posttest counseling, visual case analysis and epidemiological techniques, STD*MIS data entry and/or other related activities.
* Completes required records and reports including case management forms, laboratory request forms, travel reimbursement forms, HMC/CIS, etc. in accordance with current CHD and STD Program Policy and/or procedure.
* Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
* Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency. This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
* A valid driver's license, free of major infractions and access to an automobile to be utilized for work purposes.

Job Summary:

* Collects and records TB data into Health Management System (HMS) electronic health record or patient chart and assists with TB reports and Quality Improvement (QI) as directed.
* Assists with coordination of respiratory fit testing program to include performing fit testing and documenting results in employee health chart and maintenance of fit testing database.
* Sets up and maintains remote video monitoring equipment to facilitate DOT. Maintains documentation on video monitoring equipment assignment.

Assist in preparing and perfecting" appeals", provide representation at fee basis, hardship eligibility hearings, request for waivers of federal entitlements, military discharge review hearings or Regional Office VA hearings as required.
* Review and interpret VA laws, rules and regulations affecting veteran's entitlements in applying for federal and state veterans' benefits. Respond to telephonic or written inquiries from citizens, veterans, dependents, Post, Chapter, County, and State Veterans' Service officers with accurate and complete information. Provide accurate and current information relative to problems with the administration of benefits with the local agency of jurisdiction, process daily incoming mail and correspondence. Complete forms and documents utilizing the Department's Systems. This includes entering all clients' information into the computer database. If working in a medical center or hospital, visit each new admission to the medical center at the request of the veteran or health care provider within 48 hours of the request.
* Assist all accredited organizations, state, county or others in obtaining medical information necessary to support claims for benefits. Maintain rapport and liaison with VA outpatient staff regarding case development and treatment to support claims processing and advocacy issues. Confer with clinic, service center and medical center staff as required. Safeguard and protect all private medical information and claims information in accordance with appropriate regulations and statutes.
* Represent the Florida Department of Veterans' Affairs by providing briefings and information on State and Federal veterans benefits at local events, military bases, medical center meetings, and community fairs and other events as determined your supervisor and the department. Represent FDVA on committees, advisory boards and consumer service programs established at VA medical centers and satellite outpatient facilities. Deploy and provide assistance during State emergencies such as providing assistance at disaster recovery centers (DRC) throughout the state after natural disasters.
* Assist in preparing forms, obtaining signatures, financial data, and personal information for veterans requesting admission to State Veterans' Nursing Homes. When necessary visit and provide assistance to the home in completing applications, verifying eligibility and financial data and assisting family members in obtaining state and federal veterans' benefits.
* Visit and provide on-site assistance, claims preparation and submission of claims to all Florida National Guard units in the geographical area. Maintain contact with all guard and reserve units and active military installations in the Florida geographical area.
* Perform other related duties to include but not limited to, attendance at conferences, seminars, schools of instruction and upon request serve as an instructor, or speaker at public events.
