Position Overview
Under general direction, functions as a designated lead worker for a team of casework staff; serves as functional resource person for team casework staff; conducts periodical determinations of eligibility; identifies training needs and conducts on-the-job training for assigned staff.
Essential Functions
* Serves as functional resource person for team casework staff; answers questions, explains official policies, program rules, and procedures for team staff; instructs and assists in handling difficult and complex cases; hears and investigates clients complaints; reviews initial responses and recommends Department's position to client appeals.
* Serves as a designated lead worker for a team of casework staff; assigns and reviews work; provides guidance and training to assigned staff; advises immediate supervisor regarding performance of staff.
* Conducts periodical determinations of eligibility; updates the system with changes to clients information.
* Identifies training needs and conducts on-the-job training for assigned staff; recommends formal training courses for team members; gathers, organizes, distributes, and explains operational and administrative policies and procedural materials and information for staff.
* Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
* Requires knowledge, skill and mental development equivalent to completion of four (4) years college with emphasis on the behavioral sciences and two (2) years suitable professional casework experience in a social service agency; or one (1) year of agency casework experience; or any combination of equivalent training and experience.

SGA's Early Head Start Home-based program serves children 0-3 years within the city of Chicago through weekly home visits engaging both the caregiver and the child. Additionally, families participate in bi-monthly group socializations, parent trainings, governance, and field trips. As part of the research-based Head Start program, all children and their families receive wraparound services to ensure the healthy development and success of the child. The Health & Mental Health Specialist is responsible for monitoring all of the health physicals, hearing and vision screenings, developmental screenings, individual health plans, and follow up care for the children in the program.
MAJOR DUTIES AND RESPONSIBILITES
Child & Family Engagement
Provide and maintain 90-minute home visits for 10 children and/or pregnant women on a weekly basis. Provide make up visits for those canceled by the program or by the Home Visitor
Conduct at minimum 46 home visits for each child during the program year
In cooperation with the parent, plans and implements developmentally appropriate home-based activities based on child's assessment and identified family needs
Demonstrate commitment to the process of developing and maintaining quality relationships with children and families.
Complete weekly child observations and enter into COR
Assist parents with understanding and implementing the Family Partnership Agreement
Complete developmental and social-emotional screenings using the ASQ-3, ASQ-SE and ESI-R tools according to screening schedule

The Parent Educator/ Home Visitor provides home visiting services and parent education to children and families in the CPS Prevention Initiative program. Works from a team approach utilizing principles of Family Support Practice to enhance the overall development of children and families in the program.
Duties include, but are not limited to:
* Provide home-based services including parent/child interaction activities to families.
* Provide support, education, and information on relevant topics to including child development, parenting, and family education through home and center-based services.
* Follows PAT curriculum guidelines along with additional requirements from the funding partner.
* Conducts family assessments to determine needs for case management and other referrals as well as the development of an IFSP.
* Conducts developmental screening, home-based and center based services, referrals and advocacy.
* Supports the family in reaching their identified goals and supports parents identify tools and skills to foment their child's health development.
* Prepare and submit appropriate documentation and reports, maintain client records in accordance to professional standards and funder requirement.
* Present case material and request information from other center staff during weekly meetings.
* Represent organization at meetings and attend conferences as requested Assist in recruiting eligible families for home visiting program and maintain caseload.
* Attend the PAT Training for certification and training on Principles of Family Support.
* Collaborate with other agencies and services.
* Assist childcare staff with planning center activities and services, which reflects families' needs and interests.
The ideal candidate will have the following education and experience:
* Minimum of a Bachelor of Arts degree (B.A.) in Human Services, Early Childhood Education, or a related field.
* Experience in child development, early childhood education, social work, or related field.
* Experience in PAT curriculum guidelines along with additional requirements from the funding partner (preferred)
* Bilingual strongly preferred - English/Spanish
* Knowledge and understanding of community.
* Must have access to reliable and efficient transportation that will allow for the completion of caseload responsibility (Drivers' License and Car Insurance).

MAJOR DUTIES AND RESPONSIBILITES
Recruit and maintain a caseload of up to 20-24 children/pregnant mothers.
Conduct biweekly (weekly if needed) visits and submit weekly schedules.
Work jointly with parent to obtain the necessary documentation needed to complete intake and other program requirements.
Assist in maintaining at minimum 100% enrollment rate throughout the program year utilizing various outreach and recruitment strategies.
Complete, update and maintain all required Parent as Teacher (PAT), ISBE, program/agency forms.
Work jointly with parents to develop home visit and activity plans based on each child's assessment and identified family needs.
Collaborate with the team, plan and facilitate monthly parent and child activities for Group Connections.
Maintains families engaged. Encourage parent participation in literacy activities, parent workshops, group connections and other events or trainings.
Responsible for data entry into the programs data system and maintaining all required documentation in child's/pregnant mother's binders; keeping all information confidential.
Complete developmental assessments/screenings and family needs assessment with families as needed to meet program's requirements.
Complete required ASQ screenings to identify children with special needs and makes the necessary referrals to appropriate staff.
Develops with parent input, an individualized family support plan.
Serve as an advocate for families and as a liaison between families, the program, and the larger community.
Work in collaboration with other staff for the recruitment and enrollment of eligible families.
Collaborates with team to create new MOU's/Partnerships with agencies/organizations that provide services to pregnant women, children and their families.
Maintains communication with supervisor and agency staff.
Participate in on-going training and staff development provided by SGA, Program Director, other consultants, workshops and conferences.
Attend regularly scheduled team and agency staff meetings/trainings.
Complete other duties as assigned.
Requirements
QUALIFICATIONS FOR THE JOB
Education:
High School Diploma / GED
Associate Degree in Child Development or related field and/or CDA preferred.
Gateways credential level 3 preferred
Experience:
Minimum of 3-5 years of experience working with young children ages 0-3 years old and/or parents.
Other Requirements:
Must successfully complete Parents as Teachers Model Certification training.
Must be familiar with sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
Must be able to make home visits to present, prospective and former clients in the clients' own home.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of customers or employees or the organization.
Exercises discretion and independent judgment in executing tasks.
Must demonstrate the ability to establish helping relationships with families of varying educational, experiential and socio-economic backgrounds and handle emergency and crisis situations.
Valid driver's license, car insurance and access to automobile.
Bilingual preferred (Spanish)
KEY COMPETENCIES:
Competent in Microsoft Office tools and inputting services in an online database.
Home Visitors must receive training in education, nutrition, and other topics relevant for the position, and must plan and implement appropriate family/parent educational activities and function as a member of the program team.
Schedule may be flexible and may include some evening and weekends.
