ESSENTIAL FUNCTIONS: Case Management: Assist with the creation and implementation of individual case plans designed for clients self-sufficiency and success. Collect supplementary information needed to assist clients, such as employment records, medical reports, and other reports as deemed necessary to comply, and to determine the eligibility of programs offered in the community.
