Role Scope:
We are looking for a dynamic, service-oriented Interdisciplinary Team (IDT) Coordinator who is passionate about supporting elderly persons in their journey. Role Scope:
This is an opportunity for a mission critical leader. The Outreach & Enrollment Supervisor is responsible for closely developing and managing a team of Habitat Health Outreach Specialists, Enrollment Specialists, and Intake Coordinator, as well as key community partners.
